Severe Chronic Traumatic Encephalopathy in a US Naval Special Warfare Combatant Crewman.

IMPORTANCE: Chronic traumatic encephalopathy (CTE) is a tauopathy mostly observed following a history of repeated impact-type head injury, usually from contact sports. The overwhelming majority of published, high-stage CTE cases are in older, former high-level contact sport athletes. CTE was infrequent in a large case series of brains donated by military service members, wherein it was most often of minimal severity, while being unanimously present in former contact sport participants. OBJECTIVE: To describe the neuropathological examination of a brain donated from a US Naval special warfare

Patterned injuries from a modified sickle: Forensic observations and insights-Case report.

Sharp weapons are common homicide weapons. However, in rare cases, unusual weapons are used for crimes and can pose a challenging task to the forensic pathologist during autopsy. Darat is an indigenous agricultural tool used in the hilly regions of India. Darat is a modified sickle, and literature on injury patterns caused by it is exceptionally rare. This case report describes a double homicide involving a 72-year-old man and his 65-year-old wife, who were killed by a Darat. Autopsy findings revealed multiple chop, stab and incised wounds to the head, neck, and shoulders, consistent with heav

Exploring superior capsular reconstruction and tendon transfers for massive irreparable posterosuperior rotator cuff tears.

Rotator cuff tears are prevalent, affecting 20% of the general population, with massive tears accounting for 40% of these cases. Massive tears, those larger than 5 cm or involving several tendons, pose substantial clinical problems, including poorer surgical outcomes and increased recurrence rates. Multiple classification systems offer varied definitions, complicating treatment strategies. The irreparability of these tears, exacerbated by conditions such as tendon atrophy and advanced imaging abnormalities, can further complicate management. Surgical options include superior capsular reconstru

Advocates, Academics, Survivors and Clinicians to END Intimate Partner Violence (ASCEND-IPV) initiative: a prospective observational case-control study protocol to identify plasma biomarkers of intimate partner violence (IPV)-caused brain injury (BI).

INTRODUCTION: Although as many as 92% of survivors of physical intimate partner violence (IPV) report impacts to the head and/or non-fatal strangulation (NFS) that raise clinical suspicion of brain injury (BI), there are no evidence-based methods to document and characterise BI in this vulnerable population, limited clinical practice guidelines and insufficient understanding about long-term risks for conditions including Alzheimer's Disease and Related Dementias (ADRD). This leaves most survivors of IPV-caused BI (IPV-BI), overwhelmingly women, without adequate access to medical care and suppo

Multiple Ipsilateral Acute Epidural Hematomas: A Case Report and Literature Review.

Acute epidural hematoma (AEDH) typically develops from a skull fracture caused by head injuries and usually presents as a single lesion. The surgical strategy becomes more complex when multiple AEDHs are present. In this report, we present the usefulness of CT angiography for detecting active bleeding lesions in multiple AEDHs. A 33-year-old man was transferred to our hospital with a head injury. Two isolated right-sided AEDHs were observed on CT: a thick temporoparietal lesion causing a midline shift and another thin lesion in the middle cranial fossa. Contrast-enhanced CT revealed extravasat

Functional and three-dimensional radiographic outcomes after open reduction and internal fixation of condylar head fractures using magnesium alloy cannulated screws - a retrospective long-term follow-up.

OBJECTIVES: Condylar head fractures are common in facial trauma and can result in significantly impaired mandibular function. Open Reduction and internal fixation overall produce better functional outcomes than closed treatment. Multiple methods of fixation have been proposed, including the use of biodegradable magnesium-based cannulated bone screws. This study aims to investigate the long-term efficacy of this fixation method using three-dimensional radiographic imaging and clinical outcomes. MATERIALS AND METHODS: This retrospective cohort study of a single centre included patients who unde

Factors Associated With Persistent Symptoms Following Mild Traumatic Brain Injury.

IMPORTANCE: Traumatic brain injury (TBI) is a common presentation in which it is clinically difficult at presentation to identify patients at risk for persistent symptoms, defined as lasting longer than 30 days. Few studies report the time period from injury to presentation; thus, it may be useful to determine early findings that identify patients who are likely to have persistent 30-day symptoms. OBJECTIVE: To identify parameters associated with persistent 30-day symptoms in adult emergency department (ED) patients presenting shortly after a mild TBI (mTBI). DESIGN, SETTING, AND PARTICIPANT

Do National Hockey League Injury Rates Increase During the Years Players Participate in the Winter Olympics?

BACKGROUND: Every 4 years, the Winter Olympics occurs in the middle of the National Hockey League (NHL) season. The NHL allowed its players to participate in the Olympics during the 2002, 2006, 2010, and 2014 editions. The added game load, travel, and intense exertion over a short period of time may increase players' risk of sustaining injuries. PURPOSE: To compare the injury rates between a normal NHL season and an Olympics-interrupted season and to compare the rates of different types of injuries, games missed due to injury, and the incidence of multiple injuries in NHL players. STUDY DESI

A case of open skull fracture, subdural hematoma, and brain contusion complicated by traumatic brain injury-associated coagulopathy caused by a lawn trimmer; A case report.

Penetrating head injury is less common than blunt head injury in Japan but causes devastating complications and requires prompt surgical intervention. However, not all penetrating head injuries are apparent on arrival. We herein present a case of a 74-year-old male who lost his balance on a steep hillside and fell on an active lawn trimmer with rotating circular blades, which hit his face. On arrival, the patient was alert and conscious, but was unable to recall the accident. His vital signs were stable and motor functions were normal. The patient had a large laceration ranging from the right

Complex Dorsal Dislocation of the Metacarpophalangeal Joint of the Fifth Finger Treated by Open Reduction With Volar Approach: A Case Report.

Complex dorsal dislocations of the metacarpophalangeal (MCP) joint are rare, with involvement of the fifth finger being exceedingly uncommon. These injuries typically result from hyperextension forces that entrap the volar plate, precluding successful closed reduction and necessitating surgical intervention. In this report, we present the case of a 20-year-old male patient who sustained multiple injuries, including craniofacial trauma and an open dorsal dislocation of the left fifth MCP joint, following a road traffic accident. Initial attempts at closed reduction under local anesthesia were u

Multiple Lumbar Compression Fractures Following a New-Onset Seizure: A Case Report.

Vertebral compression fractures (VCFs) occur when axial forces exceed the vertebra's strength, such as those occurring with low-impact injuries in osteoporotic patients. In healthy individuals, VCFs require significant trauma. Though rare, seizures can cause spinal compression due to muscle contractions. These fractures often affect the mid-thoracic spine and are usually asymptomatic due to the postictal state. This case discusses an uncommon incidence of multiple lumbar fractures following a seizure, otherwise known as seizure-induced spinal fractures. A 50-year-old male with a past medical h

Nonoperative management of terrible triad injuries: clinical outcomes of 39 cases.

BACKGROUND: Terrible triad injury (TTI) poses therapeutic challenges in joint stability and functional recovery. While surgery has traditionally been preferred, recent interest in nonoperative management for select cases has emerged, with studies suggesting positive outcomes for carefully chosen patients. METHODS: This retrospective study, conducted at a tertiary medical center, included patients treated conservatively for TTI between 2014 and 2022. Patients younger than 18 years with open or pathological fractures or multiple injuries were excluded. Conservative treatment was chosen for elbo

Closed Reduction and Spica Cast Immobilization in Patients Aged 18 Months and Older With Developmental Dysplasia of the Hip.

OBJECTIVES: For children aged 18 months and above with developmental dysplasia of the hip (DDH), treatment typically involves open reduction and hip reconstruction surgery. However, there is limited literature evaluating the efficacy of closed reduction in this patient population. The purpose of this study was to investigate the clinical efficacy of closed reduction and spica cast immobilization treatment for children aged 18 months and above with DDH, as well as the incidence of avascular necrosis of the femoral head (ANFH) and their associated risk factors. METHODS: We retrospectively revie

Innovative Use of Joshi's External Stabilization System for Tibial Fractures Complicated by Skin and Soft-Tissue Issues: A Case Series.

INTRODUCTION: Femoral head fractures occur almost exclusively as a result of a traumatic hip dislocation. Due to the intrinsic anatomical stability of the hip, most of these injuries result from high-energy trauma. Treatment is typically an emergency surgery that includes the reduction of the dislocated hip under anesthesia to fix the fracture of the head of the femur and reduce dislocation. Treatment outcomes tend to be inconsistent, largely because of the fracture's frequent association with pain, joint stiffness, and loss of function. Complications that are most commonly seen after femoral

Sequential Cranial and Overlooked Cervical Spine Injuries Due to Head Trauma: A Billiard-Like Mechanism.

A 55-year-old male construction worker presented with multiple fractures after falling downstairs and striking his forehead on a concrete wall, an incident preceded by consuming approximately 50 g of alcohol. Initial evaluation revealed a contaminated 7 cm forehead laceration and a depressed frontal bone fracture, with no neurological deficits or intracranial bleeding on computed tomography (CT), leading to discharge with follow-up instructions despite mild intoxication. The next day, neck pain prompted further imaging, uncovering a complex injury pattern: frontal bone fracture, occipital base

Fireworks and Injury in the United States: A Review of Epidemiology, Clinical Burden, and Policy Implications.

Fireworks are a hallmark of American celebrations, yet they generate a growing and under-recognized burden of injury. The aim of this narrative review is to integrate current United States evidence on firework-related harm, reveal clinical and economic patterns, and evaluate how policy shapes risk. Recent studies and national surveillance reports show a steady rise in emergency visits linked to consumer fireworks, with males comprising most patients. The hands and upper extremities sustain the largest share of harm, followed by the eyes and the head, face, and neck region. Although less freque
