Paediatric temporal bone fractures: a single centre experience.

I read with great interest the article titled "Paediatric Temporal Bone Fractures: A Single Centre Experience" published in your esteemed journal. The study provides valuable insights into the clinical presentation, management and outcomes of temporal bone fractures in the paediatric population - a subject that remains understudied compared with adult cases. While the authors' efforts in compiling retrospective data from a single centre are commendable, the study largely reiterates established knowledge without offering significant new contributions to the literature.

Injuries following accidental low height-falls sustained by children under 1 year of age - A systematic review.

BACKGROUND: Low-height falls are a recurrent cause of accidental trauma in young children. They also represent the most commonly reported history provided by perpetrators of abuse. The study of traumatic injuries resulting from low-height falls is essential to provide the forensic expert with the necessary data to better distinguish accidental falls from cases of inflicted violence. Previously published clinical studies are sometimes contradictory, and systematic reviews on this topic remain scarce. OBJECTIVES: To describe the traumatic injuries resulting from accidental falls of less than 15

Predictors of Interfacility Transfer in Head and Neck Trauma: A National Trauma Data Bank Study.

INTRODUCTION: Traumatic head and neck (H/N) injuries frequently present to level III/IV trauma centers, yet many require care available only at higher-level facilities. This study aims to characterize transfer patterns in H/N trauma patients initially treated at level III/IV centers. METHODS: The National Trauma Data Bank was queried for adults with H/N trauma initially presenting to level III/IV trauma centers between 2007 and 2015. Univariate analysis compared cohort characteristics, while multivariate logistic regression identified independent predictors of transfer. RESULTS: In total, 18

Facial Injury Severity Scale and Frontal-Segment Involvement as Risk Stratification Markers in Panfacial Fractures: A Single-Center Retrospective Study.

Panfacial fractures are high-energy injuries. We evaluated whether the Facial Injury Severity Scale (FISS) and frontal-segment involvement are associated with early surgical complications, length of stay, intensive care unit (ICU) admission, and craniocerebral injury. Thirty-four panfacial fracture patients treated between January 2016 and May 2026 were analyzed retrospectively at a single center. Fractures were classified with the Erdmann-Follmar four-segment model (≥3 of 4 segments); FISS used the Duke modified system. Because several tables were sparse, associations were estimated by Firth

Airway management using submental intubation in a patient with multiregional facial fractures from a blast injury: a case report.

Airway management in complex maxillofacial trauma is challenging because intraoperative occlusion assessment requires an unobstructed oral cavity, typically favoring nasotracheal intubation (NTI). However, NTI is absolutely contraindicated in patients with a confirmed or suspected basilar skull fracture and is additionally contraindicated when concurrent nasal bone reduction is required. We report the case of a 36-year-old man with multiregional facial fractures caused by a workplace fire-extinguisher blast, in whom an expansile left ethmoid sinus hematoma raised concern for cribriform plate i

Effect of gonial angle variation on stress distribution and fracture susceptibility in the mandibular angle: a finite element analysis.

AIM: The mandibular angle is affected by morphological and functional factors. This study aimed to evaluate the stress distribution and susceptibility to fracture in the angle region under two different traumatic loading conditions in mandibular models with different gonial angles using finite element analysis. MATERIALS AND METHODS: A mandible model with three different gonial angles (115°, 125°, and 135°) was created based on a standard reference representing human anatomy. In the linear static analysis, a traumatic force of 2000 N was applied to a 1 cm diameter circular contact area in two

Addressing unilateral lambdoid craniosynostosis using endoscopic suturectomy and barrel-stave osteotomies: surgical technique and literature review.

PURPOSE: Unilateral lambdoid craniosynostosis (ULC) is a rare cranial deformity requiring intervention to correct asymmetry of the posterior skull. Conventional open remodeling and distraction osteogenesis offer robust expansion but carry risks of higher blood loss, hardware complications, or reoperation. We report a novel, minimally invasive technique combining endoscopic lambdoid suturectomy with barrel-stave osteotomies, supplemented by a literature review. CASE DESCRIPTION: A 2-month-old healthy male presented with classic left ULC features, including occipitoparietal flattening, ipsilate

Do 3D-printed craniofacial fracture models improve learning outcomes in maxillofacial trauma? A multicenter cluster-randomized trial.

BACKGROUND: Teaching maxillofacial traumatology is challenging, as it requires not only theoretical knowledge but also advanced visuospatial skills. Conventional educational resources such as CT scans and diagrams offer limited support for understanding complex three-dimensional fracture patterns. Three-dimensional (3D) printing has emerged as a promising educational tool, yet its pedagogical impact in maxillofacial surgery remains insufficiently investigated. METHODS: This prospective, multicenter, cluster-randomized controlled trial was conducted at three French medical faculties (Nice, Tou

A Bump in the Night: A Case Report of Pediatric Epidural Hematoma Secondary to Sinus Pericranii.

A sinus pericranii (SP) is a rare transcranial vascular malformation caused by diploic emissary veins forming an irregular connection between a dural sinus and extracranial veins. It is generally considered a benign condition with few complications1. We report the case of a 5-year-old male patient who sustained a skull fracture from head trauma with development of an epidural hematoma that was traced to an underlying ruptured SP as the source. Non-contrast computer tomography (CT) confirmed epidural hematoma. Craniotomy for evacuation revealed an emissary venous connection along the skull frac

The role of bloodstain pattern analysis (BPA) in homicide reconstruction: A case of patricide involving two Brothers.

INTRODUCTION: The term parricide refers to the killing of a close relative, though it commonly indicates the murder of one's father (patricide) or mother (matricide). This paper presents a case of patricide in Genoa involving two brothers, X and Ywhere Bloodstain Pattern Analysis (BPA) was key to reconstructing the crime dynamics. CASE DESCRIPTION: The victim was found lying supine in the entrance of home. Near the body were a bloodstained rolling pin and screwdriver. Autopsy revealed 23 head and facial injuries, multiple skull fractures, and diffuse subarachnoid haemorrhage. Injury character

Fracture Prevalence Among Those With Hypermobility Spectrum Disorder/Hypermobile Ehlers-Danlos Syndrome.

Previous series have questioned bone fragility in those with hypermobility spectrum disorders (HSD) or hypermobile Ehlers-Danlos syndrome (hEDS). These conclusions have been extrapolated to infants and children often in the setting of suspected child abuse. Few studies address this in non-ambulatory children, leaving medical professionals and the courts alike to debate the reported observations. One hundred patients with HSD/hEDS were recruited from a large hypermobility clinic. Each was required to have a guardian/parent present to attest to the fracture history for the entire age of the indi

Bi-allelic EPHA2 mutations cause dysosteosclerosis by inhibiting osteoclastogenesis and enhancing osteoblastogenesis.

Erythropoietin-producing hepatocellular (EPH) receptors, the largest receptor tyrosine kinase family, have been shown to play a crucial role in normal physiological processes and pathologies. EPHA2, encoded by EPHA2 gene, a member of the EPH receptor family, has been implicated in cataracts and various bone tumors, including osteosarcoma, chondrosarcoma, and bone metastases of breast cancer. However, no association between the EPHA2 gene and genetic skeletal disorders has been reported to date. Here, we identified a Chinese family with a diagnosis of dysosteosclerosis, presenting with signific

Mandibular fracture with subsequent sequestrum formation and tooth-root abscess in a 3-year-old mare.

A 3-year-old quarter horse cross mare was presented to the Atlantic Veterinary College (Charlottetown, Prince Edward Island) for evaluation of a persistent draining tract on the ventral aspect of the left mandible, associated with a previous fracture. Radiography and ultrasonography confirmed the presence of a chronic sequestrum separated from the parent bone. Sequestrectomy and surgical debridement of the mandible were carried out, and the horse was discharged. Three months later, the mare was presented again to the Atlantic Veterinary College, as the draining tract had not resolved. A CT sca

Key points in orbital reconstruction.

Orbital fractures represent the most challenging fractures of the facial skeleton to treat. Decision making in the management of patients with orbital fractures is often demanding, requiring experience and profound knowledge of the anatomical and functional aspects of this anatomical region. The complex anatomy and functionality of the orbital area, along with the limited visibility of the surgical field and the extreme surgical precision required, make the reconstruction of this region extremely demanding, thus necessitating an experienced surgeon with high surgical technical skills. The rest

Epidemiology and burden of facial fractures in China, 1990-2021: findings from the Global burden of disease study 2021.

OBJECTIVE: Facial fractures can be disabling injuries, leading to long-term functional, aesthetic, and psychological impairments. Comprehensive assessment of their disease burden in China is essential for informed health policy and prevention strategies. METHODS: Data regarding facial fractures in China from 1990 to 2021 were extracted from the Global Burden of Disease study (GBD) 2021 database. Incidence, prevalence, and years lived with disability (YLDs) were estimated using DisMod-MR 2.1, a Bayesian meta-regression modeling tool. Estimates were presented as numbers and rates, with comparis
