A comprehensive review of trauma transfers to a rural pediatric trauma center.

PURPOSE: The purpose of this study is to delineate transfer patterns to a rural Level 2 pediatric trauma center by identifying common diagnoses associated with potentially avoidable transfers (PAT) to a higher level of care with the end goal of optimizing resource utilization in the center's catchment area. METHODS: Using trauma registry data, every pediatric trauma patient (< 15 years old) transferred to our Level 2 trauma center from January 2017 to December 2023, was included. "Potentially avoidable transfer" was characterized as a patient who, upon arriving at the trauma center, did not u

Spatiotemporal Sonodynamic-Nanozymic Therapy for Infected Jawbone Defects.

Infectious jawbone defects, arising from open fractures, surgical contamination, or hematogenous pathogen spread, represent severe orthopedic complications characterized by concurrent bone loss and chronic infection. Precise control of pathogen colonization, effective modulation of local immune microenvironment, and activation of osteogenesis are spatiotemporal phases that pose significant clinical challenges. Here, we construct a multifunctional platform of copper-zinc bimetallic metal-organic nanosheets encapsulated in a polyvinylpyrrolidone-polyvinyl alcohol hydrogel (CuZn-MON@gel). Upon ul

Botulinum toxin injections into masticatory muscles: A brief review on applications in dental occlusion management for orthognathic surgery and facial fractures.

BACKGROUND: Orthognathic and maxillofacial trauma surgeries are often challenged by postoperative instability resulting from persistent masticatory muscle forces acting on the repositioned bone segments. Botulinum toxin type A (BTX-A) has been proposed as an adjunctive intervention to transiently reduce these forces and enhance surgical stability. OBJECTIVES: The aim of this narrative review was to evaluate the adjunctive role of BTX-A in orthognathic and maxillofacial trauma surgery, with particular emphasis on its potential to reduce postoperative masticatory muscle forces, thereby enhancin

Toward a semiotic-Bayesian epistemology of skeletal trauma interpretation in forensic anthropology.

This article introduces a semiotic-Bayesian epistemological framework for the analysis and interpretation of skeletal trauma in forensic anthropology. The model integrates Peircean semiotics with probabilistic inference to provide a structured, reproducible approach to evaluating forensic evidence under conditions of uncertainty. Bone injuries are conceptualized as signs that maintain a causal relationship with the events that produced them; their interpretation begins with abductive reasoning, through which the forensic anthropologist formulates plausible hypotheses about the origin of the ob

Risk stratification for significant acute traumatic intracranial hemorrhage in older adults after a ground-level fall: A prospective multicentre cohort study.

BACKGROUND: Falls among older adults are now the leading cause of traumatic brain injury worldwide. We aimed to identify historical and clinical characteristics including the visible head impact location indicative of significant acute traumatic intracranial hemorrhage in older patients presenting to emergency department with mild traumatic brain injury subsequent to a ground-level fall. METHODS AND FINDINGS: We conducted a multicentre prospective cohort study across five university-affiliated emergency departments over a 2-year period (1 July 2023 to 30 June 2025) in Europe. We included pati

Age-stratified outcomes after small-volume isolated blunt traumatic brain injury: A national ACS-TQIP analysis.

BACKGROUND: Small-volume traumatic brain injuries (TBIs) are often considered low risk clinically, but older adults may face disproportionate adverse outcomes. Evidence outlining age-specific risks related to limited subdural hematomas (SDH), epidural hematomas (EDH), and cerebral contusions remains limited. METHODS: We performed a retrospective cohort study using data from the American College of Surgeons Trauma Quality Programs Participant Use File (ACS-TQIP-PUF), 2017-2022. Adults aged 40 years and older with isolated blunt TBI were included, excluding skull fractures and multisystem traum

Standardized Minimum Number of Individuals (MNI) Enumeration and Veterinary Forensic Assessment in a Large-Scale Animal Starvation Scene: A Technical Note.

Mass companion animal mortality scenes pose major veterinary forensic and animal welfare challenges, particularly when remains are commingled, variably decomposed, or skeletonized. In March 2023, the remains of 1256 dogs and cats were recovered from a private residence in Yangpyeong-gun, Gyeonggi-do, Republic of Korea, after prolonged deprivation of food and water. Because whole-carcass enumeration was not feasible across the entire site, a standardized skull-based minimum number of individuals (MNI) approach was used to derive the scene-wide count by combining direct counts of intact, non-dup

Vertex epidural haematoma secondary to sagittal sinus involvement: a surgical challenge.

Epidural haematomas (EDHs) involving the superior sagittal sinus (SSS) are rare and carry significant surgical risks. We report a man in his 50s who sustained an occipital head injury secondary to a tonic-clonic seizure. Initial imaging revealed a large occipital EDH displacing the SSS, associated with a bifocal parietal fracture. Despite neurological stability and no radiological evidence of active bleeding, conservative management was initially chosen due to high surgical risk and thrombocytopenia. The patient later developed recurrent seizures with rapid neurological deterioration, promptin

Effective management of traumatic venous sinus injuries in a resource-limited health system: a retrospective cohort study.

OBJECTIVE: Traumatic venous sinus injury (VSI) is a rare but serious complication with high rates of morbidity and mortality. In resource-limited health systems, restricted access to advanced imaging and high trauma volumes further complicate care. The objectives of this study were to quantify the effectiveness of a resource-constrained imaging protocol, evaluate its impact on outcomes, and identify predictors of VSI. METHODS: The authors conducted a retrospective cohort study of adults with skull fractures overlying dural venous sinuses at Tygerberg Academic Hospital, a resource-limited publ

Superiority of Cone Beam Computed Tomography Over Intraoral Radiography for Detection of Periapical Lucency in Complicated Fractures of the Maxillary Fourth Premolar Teeth in Dogs.

Apical periodontitis (AP) is commonly encountered in dogs. Cone beam computed tomography (CBCT) has emerged as the preferred imaging modality for identification and monitoring of AP in the human endodontic community. This study compared intraoral radiographs (IOR) and CBCT for identifying periapical lucency (PAL) (as a proxy for AP) of the maxillary fourth premolar teeth in dogs. All dogs had IOR and CBCT performed under general anesthesia. Imaging was evaluated retrospectively and compared. PAL was identified more frequently with CBCT, and results were statistically significant. No other stat

Comprehensive interdisciplinary management of multiple combination injuries in permanent maxillary anterior teeth in an adolescent patient.

Traumatic dental injuries are prevalent among children and adolescents, especially those between 6 and 12 years of age with varying degrees of severity. The complexities of their diagnosis, management and overall prognosis are compounded by their occurrence in combination. International Association of Dental Traumatology guidelines provide a systematic roadmap for the management of isolated dental trauma but the management of a combination of injuries is still left to clinicians' discretion. This case report illustrates the uncommon occurrence of multiple luxation injuries, including avulsion,

Three-dimensional symmetry of the midfacial skull: A cross-sectional CT morphometric analysis comparing mirroring and best-fit alignment.

INTRODUCTION: This study assessed midfacial skeletal symmetry in healthy individuals using advanced image segmentation and three-dimensional analysis. It aimed to quantify left-right symmetry and evaluate potential influences such as age and sex. Two methods were compared: mirroring with best-fit alignment (BFA) and rigid median sagittal plane mirroring (MSP). It was hypothesised that BFA provides a more accurate assessment of symmetry, and demographic variables do not significantly affect asymmetry. METHODS: A retrospective cross-sectional study, conducted in accordance with STROBE guideline

A Hybrid Paradigm to Develop Head Injury Criteria for Behind Helmet Blunt Trauma.

INTRODUCTION: The widely used head injury criterion (HIC) has improved human safety and vehicle design in automotive environments. Because of its application to civilians, tests that led to HIC were conducted without helmets. Studies focusing on military environments are sparse. Behind helmet blunt trauma (BHBT)-specific head injury criteria do not exist to the best knoweldge of the authors. The objective of this study is to present a focused literature review of military studies and a novel paradigm for developing enhanced BHBT medical injury criteria (EMIC) for assessing helmet performance a
