Subcutaneous and Orbital Emphysema With Pneumocephalus Secondary to Compressed-Air Gun Injury to the Face.

Compressed-air guns-devices that remove particles such as dust or wood/metal shavings using compressed air-are now widely used in industry. Although rare, the high-speed airflow they generate can cause injuries. A healthy 26-year-old man presented to our emergency department with swelling around his right eye and headache. His history revealed that the injury occurred during routine use of a compressed-air gun while cleaning work clothes. The blast of compressed air to his face resulted in subcutaneous emphysema, orbital emphysema, and pneumocephalus. No obvious entry site for the air could be

Assessing the Gap: Surgical Infection Society Guidelines Versus Real-World Antibiotic Agent Use in Facial Fractures.

INTRODUCTION: Facial fractures account for over 400,000 emergency department visits annually in the United States. They are managed operatively, non-operatively, or via observation with diet and activity modification. Regardless of management, antibiotic agents are commonly prescribed. The Surgical Infection Society (SIS) published the 2020 guidelines limiting antibiotic agent use to the peri-operative period. METHODOLOGY: We performed a retrospective chart review to evaluate antibiotic agent prescribing practices for patients with isolated facial fractures at an academic level-one trauma cen

Repeated Mild Head Injury Establishes a Senescent Cranial Bone Marrow Niche that Impairs Brain Metabolism.

Traumatic brain injury (TBI) of any severity is associated with long-term systemic inflammation and increased risk of peripheral comorbidities, yet the mechanisms driving immune dysregulation and accelerated aging after repeated mild head impacts remain poorly defined. Here, we investigated the acute and chronic effects of repeated mild TBI (rmTBI) on distal and proximal bone marrow compartments in the femur and calvaria, respectively. Using a modified weight-drop mouse model delivering rotational and acceleration-deceleration forces (3 hits/week for up to 16 weeks), rmTBI produced no mortalit

Artificial intelligence for fracture detection on computed tomography: a comprehensive systematic review and meta-analysis of diagnostic test accuracy in non-commercial and commercial solutions.

Rising patient volumes, the increasing use of computed tomography (CT) imaging in emergency departments and the resulting prolonged waiting times highlight the urgent need for efficient and accurate diagnostic tools, especially given that the number of experienced healthcare professionals is not increasing at the same pace. Artificial intelligence (AI) has emerged as a promising tool to support fracture detection on CT scans, with the potential to streamline diagnostic workflows in emergency care. However, concerns exist regarding dataset bias, limited external testing, and methodological vari

Quadriplegia Following Depressed Skull Fracture Over Superior Sagittal Sinus: A Rare Presentation of a Common Event.

Depressed skull fractures of the superior sagittal sinus (SSS) deserve special attention because they may lead to sinus obstruction or stenosis, resulting in venous hypertension. Here, we highlight an extremely unusual clinical presentation of a depressed skull fracture over the SSS obstructing the sinus, along with successful surgical management. A 35-year-old male patient presented with quadriplegia and intact sensations throughout the body following an alleged history of a rock falling on his head. On examination, computed tomography and magnetic resonance imaging of the brain revealed a de

Traumatic Middle Meningeal Artery Pseudoaneurysm Managed With Preoperative Glue Embolization Followed by Decompressive Craniectomy: A Case Report.

Traumatic pseudoaneurysm of the middle meningeal artery (MMA) is a rare but potentially life-threatening condition, typically resulting from a laceration of the arterial wall due to adjacent skull fractures. We report an unusual case of a traumatic MMA pseudoaneurysm presenting with a large intracerebral hemorrhage (ICH) and significant subfalcine herniation that was successfully treated with preoperative glue embolization followed by decompressive craniectomy. This case emphasizes the importance of early vascular evaluation in traumatic ICH and highlights the value of endovascular embolizatio

Bioengineering approaches to dynamic impact analysis for cranial fracture interpretation in arcaheology.

Cranial fractures are widely documented in archaeological contexts, yet the application of fracture mechanics to differentiate traumatic events remains limited. This study analyses a dataset of 234 human cadavers subjected to 329 experimentally controlled blunt-impact tests, examining mechanical variables and fracture patterns that could be relevant to archaeological interpretation. The results show substantial methodological variability across the analysed studies. Analysis of these studies indicates that impact energy is the most reliable parameter for assessing fracture severity, suggesting

Prevalence and Morphological Classification of Persistent Metopic Suture in Adult Autopsy Cases: A Forensic Anatomical Study from Western Türkiye.

Background: Persistent metopic suture represents a normal anatomical variant that may persist into adulthood and can be misinterpreted as a frontal skull fracture, particularly in trauma-related forensic cases. Despite its clinical and medico-legal relevance, data derived from autopsy-based evaluations remain limited, with most prevalence studies relying on dry skull collections or radiological series. This study aimed to determine the prevalence and morphological characteristics of persistent metopic suture in adult autopsy cases and to evaluate its distribution according to age, sex, and cau

Fetal parietal bone fracture in late 3rd trimester secondary to traffic accident with improper seatbelt use: a case report.

Approximately half of the women in developed countries drive motor vehicles and the consequences of road traffic-related injuries involving pregnant women can be severe. This case report describes a rare injury of a parietal bone fracture in a fetus resulting from improper seatbelt use during a motor vehicle accident. A 36-year-old woman at 35 weeks of gestation was involved in a traffic accident while seated in the front seat wearing a seatbelt, but in an improper way for pregnant women. Transabdominal ultrasonography revealed a displaced fracture of the fetus's right parietal bone with an ov

Post-mortem CT scan reveals sequential self-inflicted gunshot wounds to the head: Initial non-lethal followed by fatal injury.

INTRODUCTION: Post-mortem imaging plays a crucial role in forensic investigations. While self-inflicted gunshot wounds to the head are common, cases involving multiple shots, especially with a clear sequential pattern, are extremely rare. CASE PRESENTATION: We report the case of an 82-year-old man found deceased by his wife, seated in a chair with a 9mm firearm on his lap. Two ejected shell casings were found nearby. No signs of struggle or forced entry were noted. The decedent had a known history of metastatic prostate cancer and had previously expressed suicidal ideation. TECHNIQUE AND IMA

Growing skull fracture presenting with late-onset seizures: a case report.

Growing skull fractures (GSF) are a rare but serious complication of pediatric head trauma. The condition primarily affects children due to the weak, thin skull, and tightly adherent dura, which is more prone to tearing. GSF can present in various ways, including seizures. We report a case of a 1-year-old girl who presented with seizures and a right-sided scalp swelling. The swelling was found to be due to a GSF, resulting from a previous head injury when the child was 2 months old. The child was managed with surgery, during which the dura was repaired along with osseous reconstruction. Neglec

Are Le fort fracture lines relevant in modern trauma?

PURPOSE: Midface fractures comprise 23-97% of all facial fractures. Due to the high-speed road traffic injuries (RTI), the fracture pattern has drastically changed from the classic Le Fort pattern to a combination of these lines with varieties. The purpose of this study is to observe the Le Fort fracture lines seen nowadays. MATERIALS AND METHODS: A retrospective observational study was conducted to evaluate the Le Fort fracture pattern lines. The fracture lines were traced using computed tomography of face to visualise the Le Fort fracture line course as described by Rene Le Fort. RESULTS:

Pediatric growing orbital roof fracture with ptosis: a case report.

The majority of pediatric skull fractures are minimally displaced and can usually be managed conservatively. A rare but serious complication is the development of a growing skull fracture (GSF), that may lead to significant sequelae including orbital meningoencephalocele or neurological impairments. We report a 6-year-old boy who initially presented with right-sided traumatic ptosis and a minimally displaced right orbital roof "blow-up" fracture following minor head trauma. This injury subsequently progressed into a growing orbital roof fracture, complicated by the development of an orbital me

Protocol for establishing a mouse model of bilateral maxillary first molar extraction to study alveolar bone healing.

Alveolar bone healing following tooth extraction is a complex biological process. Here, we present a protocol for establishing a reproducible mouse model of bilateral maxillary first molar extraction to study alveolar bone healing. We describe steps for surgery, postoperative care, and tissue collection for downstream genomic, histological, and micro-computed tomography (CT) analyses. We provide practical strategies to minimize root fracture and soft tissue injury, ensuring procedural consistency and high-quality outcomes.

Comparative evaluation of MRI-based bone-targeted sequences and computed tomography for preoperative assessment of midfacial trauma.

This prospective study aimed to compare computed tomography (CT) and magnetic resonance imaging (MRI) in the preoperative assessment of acute midfacial trauma. Twenty patients received posttraumatic CT and MRI scans using a 3T scanner with a dedicated 15-channel dentomaxillofacial coil. Five MRI protocols were evaluated: UTE, DESS, Dark Bone, StarVIBE, and STIR. Three observers qualitatively assessed fracture detection, image quality, fracture line visibility, cortical delineation, and bone-to-soft-tissue contrast using a five-point scale. Descriptive statistics and inter-observer reliability
