BACKGROUND: Minimally invasive approaches to severe panfacial fractures are being used increasingly to reduce surgical trauma and the related complications. In this study, it was aimed to determine the ideal surgical approaches in severe panfacial fractures.
METHODS: Sixteen patients with severe panfacial fractures were included in this study. Minimally invasive approaches were used for Le Fort III fracture in six patients, for Le Fort II fracture in four patients, and for bilateral maxillary and orbital floor fractures in six patients. We used subciliary, intraoral vestibular, lateral eyebro

Mandibular condylar neck fractures and subcondylar fractures represent, respectively, 19-29% and 62-70% of all mandibular fractures; treatment involves some problems, common to both, concerning the choice of an adequate approach. Herewith, personal experience is reported related to the surgical treatment of some cases of mandibular condylar neck and subcondylar fractures by transparotid approaches with partial parotidectomy, removing the salivary tissue overlying the condylar neck and/or the subcondylar region. Over the last 5 years, we observed 22 fractures of the condylar neck and 10 fractur

This study investigates the postoperative complication rate in American military members treated for fractures of the facial skeleton with either immediate fixation in the Operation Iraqi Freedom combat theater or delayed fixation after transport out of the combat theater. Based on an army head and neck surgeon's case log, retrospective chart review was performed on 21 American active-duty patients evaluated for facial fractures in Balad, Iraq, between April 16, 2006, and October 30, 2006. Follow-up standardized patient interviews and review of electronic medical records were conducted to asse

A rare case of hyperostotic skull deformity in a patient with congenital bone defect--osteogenesis imperfecta--is described. In this case typical symptoms encountered in adults were observed: decreased body length caused by shortened extremities due to multiple pathological fractures in childhood, deformities of thorax, spine, facial bones and teeth, skull lesions with craniobasal and brainstem symptoms, bluish hue of sclera, hypoacusis etc.). In this patient non-typical abnormalities were found: visual deficit due to optic nerve atrophy caused by bilateral optic canal stenosis on the backgrou

OBJECTIVES/HYPOTHESIS: Although mandible trauma has been studied extensively, there is no standard for use of pre- and postoperative antibiotics in other facial trauma. We sought to determine whether antibiotic strategies have an effect on infection rates.
STUDY DESIGN: Retrospective chart review and cohort analysis.
METHODS: Patients seen by the otolaryngology service for traumatic facial injuries between January 1, 2003 and January 1, 2009, were included in a retrospective cohort analysis (N = 223). All patients received perioperative antibiotic coverage. Isolated mandible fractures were e

BACKGROUND CONTEXT: Despite multiple reports of survivability, dissociative occipitocervical injury (OCI) is generally accepted to be fatal in most cases. The actual number of trauma victims where OCI may have made the difference between life and death is unknown because multiple studies have shown that these injuries can be missed with current diagnostic methods. An improved understanding of the relative importance of OCI in blunt trauma mortality may help to refine protocols for the assessment and treatment of patients who arrive alive to the emergency room after severe blunt trauma. One way

About 5000 babies annually, or 1/12 of all births, are the result of vacuum-assisted delivery in Denmark. Towards the end of the 1990'ies, the US FDA published a national warning concerning increasing complications and mortality after vacuum-assisted delivery, and other countries followed. One explanation is that training is no longer given the needed focus. Recent introduction of new requirements on Danish specialist training and the fact that vacuum-assisted delivery is an important procedure in obstetrics provides the basis for this article which systematically reviews the neonate consequen

OBJECT: The aim of this study was to estimate the prevalence of brachial plexus injury (BPI) in pediatric multitrauma patients.
METHODS: The National Pediatric Trauma Registry was queried using the ICD-9 code 953.4, injury to brachial plexus, to identify cases of BPI. The patient demographics, mechanism of trauma, and associated ICD-9 diagnoses were analyzed.
RESULTS: Brachial plexus injuries were identified in 113 (0.1%) of the 103,434 injured children entered in the registry between April 1, 1985, and March 31, 2002. Sixty-nine patients (61%) were male. Injuries were most often caused by m

Data of three pediatric patients with orbitofacial trauma were reviewed. Two patients presented with recurrent orbital inflammation with partial remission with antibiotics. One patient presented with diplopia. All patients underwent full opthalmic examination and computed tomography (CT) of the brain and orbit, followed by surgical intervention. The indication for surgery was either abscess evacuation, repair of blowout fracture, or mass excision. All of the patients had intraorbital wood foreign bodies that were not evident on CT and were not suspected from the history given by the parents. A

PURPOSE: The aim of the present study was to determine whether plates with only 1 screw pair can be used for Le Fort I fracture management. Good postoperative results motivated the direct application of mandible fixation principles to the fractured midface region without additional experimental research. However, the amount and distribution of the forces in the midface region is different from those on the mandible.
MATERIALS AND METHODS: Testing was conducted on plastic anatomic models. The validity of the experimental model was tested before the fixation techniques were compared. Standard m

BACKGROUND: Several studies have reported the relationship among craniomaxillofacial fractures, cervical spine injuries, and head injuries with varying results. Life-threatening injuries to the head and cervical spine with devastating consequences cannot be missed by reconstructive surgeons during evaluation of facial trauma. The purpose of this study was to establish the prevalence of cervical spine injuries and/or head injuries with isolated and multiple craniomaxillofacial fractures at trauma centers across the United States.
METHODS: The study was a retrospective chart review of Internati

BACKGROUND: The correction of many craniofacial malformations requires the improvement and stabilization of the midface area. In this article, we present a simple surgical technique for improving and correcting the orbitozygomatic area. This technique is called zygomatic sagittal split osteotomy (ZSSO).
METHODS: Between September 2003 and June 2008, we performed ZSSO on 12 patients. The technique is described in the article. We performed a clinical evaluation of the technique using the following criteria: postsurgical complications, loss of stability, and sagittal prominence. In addition, a r

A Le Fort I osteotomy is widely used to correct dentofacial deformity because it is a safe and reliable surgical method. Although rare, various complications have been reported in relation to pterygomaxillary separation. Cranial nerve damage is one of the serious complications that can occur after Le Fort I osteotomy. In this report, a 19-year-old man with unilateral cleft lip and palate underwent surgery to correct maxillary hypoplasia, asymmetry and mandibular prognathism. After the Le Fort I maxillary osteotomy, the patient showed multiple cranial nerve damage; an impairment of outward move

A posterior fossa epidural haematoma (EDH) is uncommon and the diagnosis is difficult because the clinical symptoms are non-specific. Therefore, a computed tomography scan is important for the early diagnosis and management. Thirty-four patients with a posterior fossa EDH were admitted between 2001 and 2008. A retrospective analysis of the clinical and radiographic findings with regard to outcome and prognostic factors was carried out. The Glasgow Coma Scale (GCS) score on admission was recorded to be: one in 3-5, five in 6-8, six in 9-12 and 22 patients in 13-15. The admission GCS score was t

Recognizing typical midface fracture injuries and describing the imaging findings that are relevant to the maxillofacial surgeon are important. Particular attention should be paid to findings that potentially result in significant cosmetic or functional complications. Radiologists should evaluate facial fractures in multiple planes with coronal and sagittal reformats, which are especially helpful for horizontally oriented facial fractures, such as injuries to the orbital floor and the hard palate. 3-D images can also facilitate a broader understanding of the fracture impact on facial width, he
