Uncommon complication of facial fractures.

Vascular complications after head and neck surgical procedures are rare but can life-threatening. A pseudoaneurysm may appear weeks or months after the initial injury and may be difficult to diagnose. The authors report the case of a 43-year-old man involved in a motor vehicle accident presenting with multiple facial fractures, which after surgical treatment presented episodes of bleeding. After an angiography was performed, a pseudoaneurysm of the facial artery was detected. The patient later underwent endovascular embolization and had a satisfactory resolution.

Antley-Bixler-syndrome--staged management of craniofacial malformations from birth to adolescence--a case report.

In 1975 Antley and Bixler described an unusual syndromal disorder consisting of complex craniosynostosis with midfacial hypoplasia, dysplasia of ears and nose, radiohumeral synostosis, congenital fractures of the femur and upper airway impairment in a newborn. Additional urogenital and cardiac malformations can be associated however diagnosis is based on a characteristic craniofacial deformity in association with humeroradial synostosis. Complex disturbance of craniofacial growth due to premature synostoses of the cranial base and vault results in a characteristic phenotype. Steroidogenesis du

Early onset mandibuloacral dysplasia due to compound heterozygous mutations in ZMPSTE24.

Mandibuloacral dysplasia (MAD) is an autosomal recessive disorder characterized by hypoplasia of the mandible and clavicles, acro-osteolysis, and lipodystrophy due to mutations in LMNA or ZMPSTE24. Only six MAD patients are reported so far with ZMPSTE24 mutations and limited phenotypic data are available for them. Here, we report on two brothers (4 years and 9-month old) with early onset MAD due to ZMPSTE24 mutations in whom thin skin was noted as early as 5 months of age. Both had micrognathia, mottled hyperpigmentation, and enlarged fontanelles but little evidence of lipodystrophy. There was

Orthognathic surgery in pycnodysostosis: a case report.

Pycnodysostosis is an extremely rare genetic osteosclerosis caused by cathepsin K deficiency. It is a human autosomal recessive genetic disorder characterized mainly by osteosclerosis of the skeleton due to decreased bone turnover. It is characterized by short stature, brachycephaly, short and stubby fingers, open cranial sutures and fontanelle, and diffuse osteosclerosis. Multiple fractures of the long bones and osteomyelitis of the jaw are frequent complications. The authors describe an 18-year-old girl with a clinical and radiological diagnosis of pycnodysostosis and the ortho-surgical trea

Depressed skull fracture by a three-pin head holder: a case illustration.

UNLABELLED: BACKGROUND AND CASE REPORT: Many surgical procedures require a rigid immobilization of the patients' head, which is usually achieved by using a pin-type head holder. We briefly illustrate the case of a 4-year-old girl who sustained a depressed skull fracture by penetration of a pin of the head holder. The fracture was noted at the end of the surgery performed for treatment of a cerebellar astrocytoma and was managed conservatively. DISCUSSION: Several factors seem to be involved in the production of this complication as are faulty application of the pins, excessive pin pressure, s

Independent external validation of nomograms for predicting risk of low-trauma fracture and hip fracture.

BACKGROUND: A set of nomograms based on the Dubbo Osteoporosis Epidemiology Study predicts the five- and ten-year absolute risk of fracture using age, bone mineral density and history of falls and low-trauma fracture. We assessed the discrimination and calibration of these nomograms among participants in the Canadian Multicentre Osteoporosis Study. METHODS: We included participants aged 55-95 years for whom bone mineral density measurement data and at least one year of follow-up data were available. Self-reported incident fractures were identified by yearly postal questionnaire or interview (

[Injuries to the upper cervical spine. Part 2: osseous injuries].

This article for continuing education describes osseous injuries of the upper cervical spine and their treatment. Fractures of the occipital condyles are very rare and mostly result from high-speed traumas. These are usually managed conservatively. Atlas fractures can also usually be treated with immobilization of the cervical spine; in cases of unstable or dislocated injuries, various surgical procedures are employed. Three types of axis fractures can be distinguished: odontoid fractures, traumatic spondylolistheses, which also affect the isthmic region, and atypical fractures in the corpus r

Mechanism in favorable prognosis of pediatric condylar fractures managed by closed procedures: an experimental study in growing rats.

The purpose of this study was to investigate the underlying mechanism in favorable prognosis following pediatric condylar fractures managed by closed procedures. Seventy-five 1-month-old male Wistar rats were used in this experiment. Unilateral medially rotated condyle fracture in growing rats was adopted as the condyle fracture model to investigate the mechanism in favorable healing of pediatric condylar fractures. The entire fracture healing process was investigated. The rotated subcondylar fractures in young rats healed by means of callus formation, with simultaneous and prompt repositionin

Functional outcomes following surgical treatment of bilateral mandibular condylar fractures.

Debate continues regarding unilateral or bilateral treatment for mandibular condylar fractures. This retrospective study evaluates the functional outcomes of bilateral condylar process fractures after surgical intervention. From May 1994 to December 2004, 51 adult patients with bilateral mandibular condylar process fractures were studied. There were 33 cases of bilateral condylar fractures (type I); 12 cases of condylar-subcondylar fractures (type II); and six cases of bilateral subcondylar fractures (type III). All patients underwent open reduction and internal fixation. Four patients had chi

Unique paleopathology in a pre-Columbian mummy remnant from Southern Peru--severe cervical rotation trauma with subluxation of the axis as cause of death.

We describe the multidisciplinary findings in a pre-Columbian mummy head from Southern Peru (Cahuachi, Nazca civilisation, radiocarbon dating between 120 and 750 AD) of a mature male individual (40-60 years) with the first two vertebrae attached in pathological position. Accordingly, the atlanto-axial transition (C1/C2) was significantly rotated and dislocated at 38° angle associated with a bulging brownish mass that considerably reduced the spinal canal by circa 60%. Using surface microscopy, endoscopy, high-resolution multi-slice computer tomography, paleohistology and immunohistochemistry,

A clinical investigation on disc displacement in sagittal fracture of the mandibular condyle and its association with TMJ ankylosis development.

This retrospective study evaluated the potential influence of the sagittal fracture pattern and articular disc displacement on the development of temporomandibular joint (TMJ) ankylosis. 33 sagittal fractures of mandibular condyles (SFMCs) in 19 patients were treated conservatively and were divided into non-ankylosis and ankylosis groups based on their prognosis. Using computed tomography (CT) images, the SFMCs were classified into types I, II and III, and the displacement of the articular disc was investigated using magnetic resonance imaging (MRI). There were 19 (58%) SFMCs in the non-ankylo

Bilateral anterosuperior dislocation of intact mandibular condyles in the temporal fossa.

Dislocation of the temporomandibular joint occurs when the mandibular condyle is displaced anteriorly beyond the articular eminence and represents 3% of all dislocated joints reported in the body. Superolateral dislocation of the condyle into the temporal fossa is well documented in the literature, but anterosuperior dislocation without an associated fracture is rarely described. This report documents the case of a middle-aged woman with bilateral anterosuperior dislocation of the intact mandibular condyle after a fall on the face. This report reviews the literature on dislocations, suggests a

Novel technique of reduction of a chronic atlantoaxial rotatory fixation using a temporary transverse transatlantal rod.

BACKGROUND CONTEXT: Chronic atlantoaxial rotatory fixation (AARF) is uncommon as acute AARF is easily reduced either spontaneously or by conservative methods. Various anterior and posterior surgical approaches for a chronic AARF have been reported because of the difficulty encountered in obtaining reduction. PURPOSE: To describe a novel technique of reduction of a chronic AARF using a temporary transverse transatlantal rod. STUDY DESIGN: Technical report. METHODS: A 13-year-old girl presented with an 8-month-old chronic AARF with typical torticollis and "cock-robin" posture of the head with

Deformation of nasal septum during nasal trauma.

BACKGROUND: Injury to the nasal septum is commonly found in most nasal fractures. The nasal septum deforms and crumples, leading to nasal deviation and internal nasal obstruction. AIM: This study aims to identify the main areas of high stress concentration when a dynamic anteroposterior load is applied to the nasal tip, simulating nasal trauma. We wish to determine if the nasal septum acts as a crumpled zone, and deforms significantly during nasal trauma. MATERIALS AND METHODS: An idealized and a patient-specified finite element model have been generated for the present study. Several models
