Midfacial trauma and facial growth: a longitudinal case study of monozygotic twins.

The purpose of this article is to present a long-term follow-up of the growth of the face and the nasomaxillary complex in a pair of identical twins, one of whom had suffered severe midfacial trauma at age 2 years. Growth of the face and the nasomaxillary complex was longitudinally assessed and compared by means of facial photographs and cephalometric x-rays. Twin A suffered Le Fort II and III fractures with an associated frontal bone injury in early childhood and subsequently developed significant midface hypoplasia; twin B had no trauma and normal facial growth and development. Facial profil

Manipulation of fractured nose using mallet and champagne cork.

We describe an alternative method of manipulating fractured nasal bones using a surgical mallet and a champagne cork. This method enables accurate fracture reduction with minimal skin trauma, by affording the surgeon a high level of control. This method may be applied successfully to late-presenting fractures.

Teriparatide and osseous regeneration in the oral cavity.

BACKGROUND: Intermittent administration of teriparatide, a drug composed of the first 34 amino acids of parathyroid hormone, has anabolic effects on bone. Although teriparatide has been evaluated for the treatment of osteoporosis and for the healing of fractures, clinical trials evaluating it for the treatment of osseous conditions of the oral cavity in humans are lacking. METHODS: A total of 40 patients with severe, chronic periodontitis underwent periodontal surgery and received daily injections of teriparatide (20 μg) or placebo, along with oral calcium (1000 mg) and vitamin D (800 IU) sup

Comparative study of the prognosis of an extracorporeal reduction and a closed treatment in mandibular condyle head and/or neck fractures.

PURPOSE: The objective of this study was a comparison of the prognosis between an extracorporeal reduction technique and closed treatment of a mandibular condyle fracture. The relationship between condylar resorption and several clinical variables was also studied. PATIENTS AND METHODS: Seventy-one patients who had a mandibular condyle fracture took part in this study. Thirty-five patients (female: 7, male: 28, age: 30.46 ± 14.27 years) were treated by extracorporeal reduction, and 36 patients (male: 24, female: 12, age: 24.28 ± 9.99 years) were treated using a closed treatment. Paget's disease of bone: analysis of 134 cases from an island in Southern Brazil: another cluster of Paget's disease of bone in South America.

Paget's disease of bone (PDB) exhibits a marked geographic variation. In Brazil, the prevalence of PDB is unknown and only a few clinical data are available. The aim is to determine clinical, laboratory, imaging and response to treatment data in a large PDB case series in the city of Florianopolis, Brazil. We have performed a retrospective study based on charts reviews of all patients with PDB followed at the University Hospital of the Federal University of Santa Catarina and at five different private rheumatology outpatient offices in Florianopolis, between 1995 and 2009. Recurrent chronic suppurative osteomyelitis of the mandible.

Osteomyelitis of the maxillofacial skeleton is a rare condition in recent times. The combination of antibiotic therapy and surgical debridement is effective in the treatment of chronic suppurative osteomyelitis (CSO). This case report describes the successful surgical treatment of CSO of the mandible in an 18-year-old adult. Treatment included a pre-surgical course of antibiotics followed by sequestrectomy, resection of the coronoid process and removal of the pathologically fractured condylar process of the left side of the mandible. On post-operative clinical review at 1 week, the extra oral

Monoclonal gammopathy of undetermined significance and risk of skeletal fractures: a population-based study.

Patients with multiple myeloma (MM) have an increased risk of fractures. On the basis of small numbers, patients with monoclonal gammopathy of undetermined significance (MGUS) have been reported to have an increased fracture risk. Using population-based data from Sweden, we assessed the risks of fractures in 5326 MGUS patients diagnosed from 1958 to 2006, compared with 20 161 matched controls. MGUS patients had an increased risk of any fracture at 5 (hazard ratio [HR] = 1.74; 95% confidence interval [CI], 1.58-1.92) and 10 (HR = 1.61; 95% CI, 1.49-1.74) years. The risk was significantly higher

Cranial nerve injury after minor head trauma.

OBJECT: There are no specific studies about cranial nerve (CN) injury following mild head trauma (Glasgow Coma Scale Score 14-15) in the literature. The aim of this analysis was to document the incidence of CN injury after mild head trauma and to correlate the initial CT findings with the final outcome 1 year after injury. METHODS: The authors studied 49 consecutive patients affected by minor head trauma and CN lesions between January 2000 and January 2006. Detailed clinical and neurological examinations as well as CT studies using brain and bone windows were performed in all patients. Based

Immediate, single stage, truly anatomic zirconia implant in lower molar replacement: a case report with 2.5 years follow-up.

This report demonstrates the clinical use of a modified, truly anatomic, root-analogue zirconia implant for immediate replacement of a two-rooted, left first mandibular molar. A 50-year-old female patient with chronic apical periodontitis of the left mandibulary first molar was referred and the tooth was extracted. The mesial root had to be removed surgically due to a root fracture. A truly anatomical, root identical, roughened zirconia implant modified by macro-retentions was manufactured and placed into the extraction socket by tapping 7 days later. After 4 months a composite crown was cemen

Fatal facial-intracranial impalement injury in an accidental fall from a height: an autopsy case report with a review of the literature.

Impalement injury to the face and head is of clinical and forensic pathological significance because of its diagnostic pitfalls. This injury often penetrates the orbital, nasal or oral cavity into the skull, but impaling other sites of facial bone is rare. The present case was a rare type of facial-intracranial impalement injury with a direct cerebral arterial laceration. The victim was a 44-year-old man who fell from a height of about 2.5m onto a broken chair and died 6 days later despite surgical treatment. The clinical diagnosis was a 'small abrasion' of the face and 'traumatic intracerebra

Use of 4 immediately loaded zygomatic fixtures for retreatment of atrophic edentulous maxilla after complications of maxillary reconstruction.

This article reports the 20-month clinical outcome of the use of 4 zygomatic implants with immediate occlusal loading and reverse planning for the retreatment of atrophic edentulous maxilla after failed rehabilitation with autogenous bone graft reconstruction and maxillary implants. The intraoral clinical examination revealed mispositioned and loosened implants underneath a maxillary complete denture. The panoramic radiograph showed 6 maxillary implants. One implant was displaced into the right maxillary sinus, and the implant anchored in the region of tooth 21 was fractured. The other implant

Vertical ridge augmentation using titanium mesh.

With tooth loss, there is increased bone loss of the alveolus. In some cases alveolar bone loss can be severe. Severe bone loss may cause difficulty for patients wearing a conventional prosthesis or being restored with dental implants. Severe alveolar bone loss can result in malnutrition, poor self-esteem, multiple dental visits for failed prosthesis, and jaw fracture. In many cases, patients with loss of alveolar bone height or width may require reconstructive procedures. Vertical ridge augmentation remains a challenge in the reconstruction of the atrophic maxilla and mandible. The main probl

Single-stage implantation in the atrophic alveolar ridge of the mandible with the Norian skeletal repair system.

Dental implants have played a part in rehabilitation of the jaws for more than 40 years, but in some cases they alone are inadequate because of extreme alveolar resorption. Correction may necessitate a two-stage procedure with additional interventions. We have made a preliminary study of the use of the Norian skeletal repair system (SRS), a carbonated calcium phosphate bone cement used to augment the alveolar ridge as a single-stage procedure, with the placement of implants. Ten edentulous patients with insufficient vertical bone in the interforaminal area were treated. After a horizontal oste

Recurrent bacterial meningitis caused by an occult basilar skull fracture.

BACKGROUND: We present a rare case of recurrent bacterial meningitis caused by an occult basilar skull fracture. METHODS: A 9-year-old boy presented with acute headache, fever and vomiting. He had recurrent meningitis episode one month earlier and a head trauma 6 months ago. Laboratory findings and lumbar puncture suggested an intracranial bacterial infection. Computerized tomography, magnetic resonance imaging and nasal endoscopy failed to find the presence of rhinorrhea. Spiral computed tomography was performed and a three-dimensional reconstruction of the bony cranium was done. RESULTS: A

Factors associated with incomplete insertion of electrodes in cochlear implant surgery: a histopathologic study.

OBJECTIVES: Atraumatic and complete insertion of the electrode array is a stated objective of cochlear implant surgery. However, it is known that obstructions within the cochlea such as new bone formation, cochlear otosclerosis, temporal bone fracture, and cochlear anomalies may limit the depth of insertion of the electrode array. In addition, even among patients without obvious clinical or radiographic indicators of obstruction, incomplete insertion may occur. The current study is a histopathologic evaluation of possible sources of resistance to insertion of the electrode array using the temp

Evaluation of stress distribution in resorbable screw fixation system: three-dimensional finite element analysis of mandibular setback surgery with bilateral sagittal split ramus osteotomy.

The purpose of this study was to evaluate the stress distribution of resorbable screw (RS) and cortical/cancellous bone in the mandibular setback surgery with bilateral sagittal split ramus osteotomy (BSSRO) according to fixation geometry and number of RSs using three-dimensional finite element analysis. Three-dimensional virtual models of the mandible and bicortical RS (INION CPS System; diameter, 2.5 mm; length, 12 mm [Inion Ltd, Tampere, Finland]) were constructed by Mimics (Materialise, Ann Arbor, MI) using three-dimensional computed tomography DICOM data with 0.5-mm-thickness cut. Lumbar peritoneal shunt.

A lumbar peritoneal (LP) shunt is a technique of cerebrospinal fluid (CSF) diversion from the lumbar thecal sac to the peritoneal cavity. It is indicated under a large number of conditions such as communicating hydrocephalus, idiopathic intracranial hypertension, normal pressure hydrocephalus, spinal and cranial CSF leaks, pseudomeningoceles, slit ventricle syndrome, growing skull fractures which are difficult to treat by conventional methods (when dural defect extends deep in the cranial base or across venous sinuses and in recurrent cases after conventional surgery), raised intracranial pres
