Fatal Traumatic Brain Injury With Traumatic Axonal Injury, From a Nonpenetrating Sponge Round: A Forensic Case Report.

Although morbidity and fatalities related to nonpenetrating kinetic impact projectiles (KIPs), including rubber bullets and sponge rounds, have been reported, fatal traumatic brain injury resulting from these projectiles remains uncommon. A 38-year-old incarcerated male sustained a nonpenetrating sponge round impact to the frontotemporal region of the head during an altercation. The impact resulted in severe traumatic brain injury with traumatic axonal injury, ultimately resulting in death. Autopsy findings included subdural and subarachnoid hemorrhage, coup and contrecoup cortical contusions,

Reassessing Combat Helmet Protection Against Blunt and Blast Threats: A Structured Scoping Review.

INTRODUCTION: Traumatic brain injury (TBI) remains a leading cause of morbidity among military personnel, despite improved personal protective equipment. Although modern combat helmets effectively prevent penetration and skull fractures, their ability to mitigate behind-helmet blunt trauma (BHBT) and primary blast-induced TBI (bTBI) is less clear. This review evaluates combat helmet protection considering biomechanical, medical, and operational evidence. MATERIALS AND METHODS: A structured scoping literature review was conducted according to the SANRA framework. Searches were performed in Pub

Recent Advances in the Evaluation and Management of Craniofacial Missile Injuries.

Advances in missile war injuries to the craniofacial region have become a hot topic because of the recent Ukrainian-Russian and Gaza wars. It is a miserable situation affecting world peace by initiating hunger, starvation and death, and loss of energy, including electricity. In addition, the population experienced higher prices of oil and gas and a high cost of living, with damage to northern Nord storm of 1 and 2 was supplying gas to Europe. During its 23 years since US invasion with UK of Iraq, the country became unstable still subjected to confluence of conventual war, guerrilla and terrori

Characterizing bone injuring in avalanche fatalities in the French Alps: preliminary insights from post-mortem CT scans.

BACKGROUND: Avalanches present a significant risk in mountainous environments, frequently causing severe trauma and death. While asphyxia is often the primary cause of mortality (65-100 %), the incidence of traumatic injuries, particularly bone fractures, may be increasing due to evolving recreational practices and climate change. This study aims to characterize the distribution and types of fractures in avalanche fatalities. METHODS: We conducted a retrospective study at Grenoble Alpes University Hospital from April 2023 to April 2025. It included deceased adult avalanche victims with post-m

When a Pulmonary Embolism Precedes the Fall: Anticoagulation Challenges in Traumatic Intracranial Hemorrhage.

Pulmonary embolism (PE) is typically managed with urgent systemic anticoagulation or reperfusion therapy. However, management becomes complex when PE occurs concurrently with traumatic intracranial hemorrhage (ICH), where anticoagulation may worsen bleeding. A 74-year-old male patient presented after syncope and traumatic head injury. He was hypoxic and tachycardic on arrival with concern for right ventricular strain. Given persistent clinical concern, CT angiography of the chest demonstrated a massive saddle PE. CT of the head revealed traumatic subarachnoid hemorrhage and a small subdural he

Early versus delayed fixation of maxillofacial fractures in polytrauma: a systematic review.

BACKGROUND: Maxillofacial fractures are common in polytrauma, and these patients often require intensive care unit (ICU) admission until delayed definitive surgical management. This review aims to evaluate the scientific evidence on timing of definitive internal fracture fixation of maxillofacial fractures in polytrauma. We hypothesise that delayed definitive surgery is associated with better outcomes. METHODS: This systematic review was conducted in accordance with the PRISMA protocol. MEDLINE, EMBASE, Scopus, CINAHL and PubMed were searched from inception to July 2025 for articles in English

Epidemiology, Clinical Profiling, Management, and Functional Outcome in Moderate-to-Severe Traumatic Brain Injury in Children: A Single-center Experience.

BACKGROUND: Pediatric traumatic brain injury (TBI) is a leading cause of morbidity and mortality, with children being more vulnerable due to thinner skulls, immature autoregulation, and lower seizure thresholds. In India, they account for up to 30% of TBI cases. The outcomes remain variable, especially in the low-resource settings, highlighting the need to understand local epidemiology and management strategies. MATERIALS AND METHODS: A prospective study of 70 children aged 1-14 years admitted with moderate-to-severe TBI at a tertiary neurosurgical center between August 2023 and February 2025

Establishment and External Validation of a Nomogram for Predicting In-Hospital Mortality in Patients With Maxillary Fractures Combined With Basilar Skull Fractures.

BACKGROUND: Maxillary fractures rarely cause death alone; when combined with basilar skull fractures, they indicate high-energy craniofacial trauma and are often accompanied by severe intracranial, airway, and systemic injuries. PURPOSE: The purpose of this study was to develop and validate a nomogram for predicting in-hospital mortality in patients with maxillary fractures combined with basilar skull fractures. STUDY DESIGN, SETTING, AND SAMPLE: A retrospective cohort study utilized a training set from the Medical Information Mart for Intensive Care IV (MIMIC-IV) database (US, 2008-2022) an

Early middle meningeal artery embolization for small acute epidural hematomas: a clinical study from a single center.

OBJECTIVE: Small acute epidural hematomas (EDHs) are generally managed conservatively; however, in certain instances, they may necessitate surgical evacuation owing to delayed hematoma expansion. This study aims to evaluate whether early embolization of the middle meningeal artery (MMA) can prevent hematoma progression in patients with small acute epidural hematomas accompanied by skull fractures involving meningovascular structures. We hypothesize that early MMA embolization could be a prophylactic strategy to prevent hematoma progression. METHODS: A retrospective study was conducted on 14 p

Score to scan: Is there a link between Glasgow Coma Scale score and CT neuroimaging findings in trauma?

BACKGROUND: Many South African peripheral medical centres lack direct access to CT scans or neurosurgery. The Glasgow Coma Scale (GCS), used with or without other findings, remains widely utilised in traumatic brain injury (TBI) assessments with lack of standardisation between centres. There is limited data from South Africa (SA) correlating GCS scores to CT imaging in TBI. OBJECTIVES: This study aimed to assess CT findings at various GCS levels to determine whether GCS was a reliable indicator for imaging and referral. METHOD: A retrospective review of 385 patients categorised with mild, mo
