A decline in the overall number and severity of facial fractures has been observed at Madigan Army Medical Center, Tacoma, Wash. Hospital records of patients (N = 262) who were treated for fractures of the zygoma, maxilla, and mandible between 1983 and 1987 were reviewed. The three leading causes of facial fractures were assault (35%), vehicular accidents (26%), and sports (12%). The preferred treatment modality consisted of open reduction and internal fixation with plates. Societal influences have been responsible for this declining trend in the number and severity of facial fractures. This p

Over the years, depression of the malar eminence has been one of the most persistent complications of zygoma fractures, regardless of the fixation technique used. The use of a sublabial incision for direct visualization and alignment of the fracture lines at the infraorbital rim and lateral maxillary buttress is described. When coupled with single miniplate fixation of the zygoma across the frontozygomatic suture, the sublabial approach results in improved reduction of the fractures with rigid stabilization. Our experience with this approach, as well as with two-point wire fixation and with mi

In 1980 a survey was conducted of the methods employed by OMF surgeons in the United Kingdom on the treatment of facial injuries. In 1986 a similar questionnaire was circulated in the UK with the addition of questions about clinical management. Towards the end of 1986 at a meeting in Amsterdam, arrangements were made for similar questionnaires to be circulated in The Netherlands, the USA and later in India and Hong Kong. The results recorded were as follows: UK: 164; The Netherlands: 94; USA: 1999; India: 20; Hong Kong: 5. This survey has revealed differences in the treatment of maxillofacial

Facial fractures are common in the multiply-traumatized patient, and the results of previous studies that have examined the relationship between facial fractures, cervical spine injuries, and head injuries have produced conflicting results. A retrospective review of 1,050 blunt-trauma patients during a 40-month period at a regional trauma unit revealed 168 patients with facial fractures. The average injury severity score (ISS) of these patients with facial fractures was 31 compared with an average ISS of 25.6 for the entire group. There were 266 facial fractures in these 168 patients: mandible

The fornix or transconjunctival approach to the orbital floor and inferior orbit provides excellent exposure for the surgeon managing orbital fractures. The technique is ideally suited for patients with isolated blowout fractures; however, it can be combined with other surgical incisions to provide wide exposure of the inferior orbit, zygoma, and maxilla for the repair of more complex midfacial fractures. The surgical technique provides excellent access to the inferior orbit for biopsy or excision of orbital tumors and excellent opportunity for surgically augmenting orbits with posttraumatic e

Over a 6 year period 20 patients presented for secondary corrections of the periorbital region after trauma. Corrections after isolated fracture of the zygoma were necessary in 6 cases, mainly comprising re-osteotomies and contour corrections. A tertiary correction was needed only once (17%). Secondary treatment was indicated in 6 patients after fracture of the midface, necessitating mainly re-osteotomies of the zygoma, corrections of the nose and canthopexies. Tertiary corrections were performed in 3 cases (50%). In 8 patients the results of treatment of fronto-orbito-nasal fractures required

Middle face fractures are classified as follows: fractures of the nose, maxillary fractures (vertical and transverse, Le Fort I-III), lateral middle face fractures, isolated orbital floor fractures (blow out fractures), isolated zygoma fractures and combined multiple fractures. Rhinosurgical indications are discussed with emphasis on osteosynthesis using wire and miniplates.

The purpose of this article is to present a concept which facilitates the plain film identification of mid-face fractures. The concept evolves from a logical classification of mid-face fractures based upon the region(s) of involvement (orbit, zygoma, maxilla), the extent of the fractures (unilateral or bilateral) and the characteristic distribution of the fracture lines. Recognition of these fractures depends upon an awareness of the fact that the radiographic signs of mid-face fractures differ in certain respects from those involving other skeletal parts. In addition to skeletal disruption, t

Fracture dislocations of the middle third of the face usually involve a complex combination of Le Fort's three great weak lines of the facial skeleton. The fracture lines transverse the vertical buttresses of the middle third of the face at near right angles and strong impact forces can cause displacement and malalignment of the buttresses. The vertical dimension of the midfacial skeleton may be shortened or lengthened and the maxilla may be rotated. Direct realignment and stabilization of the zygomaticomaxillary buttresses with rigid fragment-to-fragment fixation gives precise reconstruction

To obtain objective data on the consequences of delayed treatment of facial fractures we reviewed the records of 220 patients who had suffered facial fractures concomitantly with extrafacial trauma severe enough in its own right to warrant hospital admission. Fractures studied were those of the mandible, maxilla, zygoma, and frontal sinus. Seventy-three patients fit inclusion criteria. Most injuries occurred in motor vehicle accidents. Associated extrafacial injuries were common and frequently multiple, the more serious of which generally took priority over the facial injuries in the triage sy

Traditional plain film evaluation of facial fractures includes a lateral view of the face. This projection is often not exploited to its full potential because the many overlapping shadows are perceived to detract from its usefulness. To assess the value of this view, the authors reviewed the later al facial films of 50 patients with a variety of fractures including 25 orbital blow-out fractures, 27 zygomaticomaxillary complex fractures, and 17 maxillary (including Le Fort) fractures. Three observations were encountered: orbital floor displacement in 60% of orbital fractures; malar strut displ

The incidence, aetiology and trends in fractures of the facial bones occurring in the years 1965, 1970, 1975 and 1980 in the Grampian Region of Scotland are reported. A 2.7 fold increase in incidence occurred during this period. Males are seen to be most at risk and an increase in assault and sports-related trauma is seen. Offshore oil exploration developed during this period and its effects are discussed. Attention is drawn to the differences in the aetiology of right and left malar fractures.

The author discusses the various problems that can arise in the static or dynamic state in the eyeball, the eyelids, and the lacrymal canals--due to alterations of the bony orbit. Reconstruction techniques in the orbitomaxillomalar, orbitonasal, and frontoorbital regions are dealt with separately, with emphasis on the need for a global and primary solution to all the alterations. Particular attention is payed to some cases of fibrous dysplasia in the frontoorbital region that were treated with reimplantation of the same dysplasic bone after it was remodeled. This is a personal technique that i

A survey of 1,447 cases of fractures of the facial skeleton from Kaduna, Nigeria, showed that 85 cases (5.9%) occurred in children younger than 13 years. The lowest incidence recorded was in the group of patients younger than age 3 but there was a gradual increase up to the age of 13 years, with the highest incidence occurring in the 12- to 13-year-old age group. The male-to-female ratio was 2.1 to 1. The site distribution in descending order of frequency was body of the mandible, mandibular condylar process, angle of the mandible, malar and nasal complexes, maxilla, and ascending ramus of the

A case of odontogenic fibromyxoma of over 5 years' duration which was evident roentgenographically (but not diagnosed) at the time of treatment for a fractured zygoma of the ipsilateral side is presented. The rate of growth did not appear to be significantly altered by trauma. The tumor was successfully treated by enucleation and curettage with removal of the associated teeth. No evidence of recurrence was noted 6 months postoperatively.

The transmaxillary K-wire is a simple, fast, safe, and effective technique for the fixation of unstable tractured malar bones. Combined with other techniques such as interdental fixation it simplifies and provides the fixation of the Le Fort II fracture or osteotomy and certain osteotomies used for facial advancement. The technique of insertion is described and illustrated.

Fifty years after the publication of Harold Gillies' work, an assessment of zygomatic injuries is made, in the light of a half century of medical advances. Recent experience suggests that the fractured orbital floor is the commonest and most significant component of all zygomatic injuries. Exploration of 582 floors in the last 5 years revealed that over 20% had true blowout fractures, without damage to the orbital rim. A further 5% had torn periosteum with only minimal bony damage. Only two cases (0.3%) had fractures of the rim without a blown-out floor. The significance of floor damage is con
