The annual incidence rate of facial injuries from vehicle crashes, 278 per 100,000 residents, was determined from a population-based study involving all Dane County, Wisconsin, hospitals with emergency departments. Applying this figure to the U.S. population yields an estimated 625,000 hospital-treated facial injuries from vehicles occurring in the United States each year. Successful reduction of severely telescoped nasofrontal ethmoidal fractures is difficult to achieve. In an effort to improve the results, a technique involving open reduction and external fixation was performed on three patients. These cases, along with ten other nasofrontal ethmoidal fractures treated between 1971 and 1980, are presented, with special emphasis in methods of reduction, stabilization, and results. The method of open reduction using an osteoplastic frontal sinus flap with external fixation gave the best overall results.

A retrospective study of 379 consecutive patients who underwent operative repair of facial fractures at the West Virginia University Medical Center was carried out. This report reviews the experience with naso-ethmoid-orbital fractures at a major medical center. Thirty-three patients were treated between 1 October 1976 and 1 January 1979, who met our definition of naso-ethmoid-orbital fractures. The majority sustained trauma in a motor vehicle accident and 60% of the patients had associated nonfacial injuries. In a case of attempted suicide a frontotransversal gunshot through both orbits led to symmetrical destruction of the bulbs and orbits. Examination revealed a rupture of both bulbs, fractures of the orbital roofs and floors, and destruction of the os ethmoidale. In the frontal cortex we found focal contusions. The physical effect of the gunshot on one hand, and the special anatomical relations of the orbit on the other, explain the serious consequences of orbital gunshot damages.

Three patients with gunshot wounds to facial and vascular structures were described. They demonstrate the importance of angiography in the acute care phases of treatment. In one case, angiography lead to eventual surgical management of an arteriovenous fistula. The other two cases described arterial injury that required careful surgical follow-up to detect delayed complications.

Over a period of one year 20 549 new patients attended an accident and emergency department. Of these patients, 15 555 were victims of accidents, including 875 who had sustained facial injuries.
