Fracture of the anterior nasal spine is so rare that clinicians do not specifically look for it if other fractures of the middle third of the face have been ruled out. However, as suggested by this case, the possibility of fracture of the nasal spine should not be overlooked.

A new technique for exposure of the orbit for decompression of the orbit is described. It may be used for other purposes such as orbital fracture repair, orbital exploration for tumors in the inferior orbit, or reconstruction of the orbit by a variety of methods. The technique provides ready, broad exposure. The approach also avoids not uncommon complications of the eyelid, approaches to the orbit such as skin shrinkage and ectropion of the lower lid. It also results in minimal cosmetic deformity.

An invitation to report for a follow-up was accepted by 74 patients (51 females and 23 males) who had undergone various types of osteotomy (50 mandibular and 30 maxillary osteotomies; 6 patients had undergone both mandibular and maxillary osteotomy). Their age at the time of operation ranged from 15 to 44 years. The follow-up period ranged from 3 months to 7 years. Maxillary osteotomy was never followed by a relapse, but 12 of the 50 patients treated by mandibular osteotomy showed some relapse: hyperplasia of the mandibular condylar process caused some relapse in 2 cases, and in 10 other cases

The term communicating exophthalmos is suggested for a condition where manual retropulsion of one eye results in proptosis of the other eye. This sign was observed in a patient with bilateral pulsating exophthalmos. The pulsating exophthalmos was ascribed to orbital roof defects in combination with post-traumatic hydrocephalus. The late presence of contour defects, as well as one eye being lower than the other, were directly related to a lack of interosseous wire fixation in this series of mid-facial fractures. There have been described a variety of methods of freezing bone in the maxillo-facial region with the object of eradicating neoplastic or other abnormal cellular elements, without the need for radical excision. Thermography allows analysis of penetration so that a suitable method can be chosen for a particular clinical situation. Sequestration and pathological fracture can occur as complications but may be avoided by attention to technique.

The authors first review the problems associated with inter-maxillary blockage and of classical methods of immobilisation used in all types of facial osteotomy. Their experience of osteosynthesis in traumatology has led them to use whenever possible osteosynthesis alone with screw fixed microplates. The other methods are used only as secondary assistance in the case of necessity. Osteosynthesis without blockage prevents recurrence and does not compromise the result as far as satisfactory dental articulation is concerned.

Autogenous costochondral grafts have found application in 26 variegated cases of congenital dysplasia, ankylosis, neoplastic disease, osteoarthritis, and post-traumatic dysfunction. Fifty-two patients with zygomatic fractures were examined clinically and roentgenologically 1 to 4 years after treatment.
