Fractures of the orbital floor are frequently accompanied by diplopia. There are other bony injuries that may produce similar symptoms; however, these are rare. We will describe a case in which a fracture of the roof of the orbit, essentially involving the anterior cranial fossa, produced entrapment with diplopia. A six-month follow-up demonstrated excellent surgical and functional results.

Neurofibromatosis has been reported to involve the eye and orbit by causing irregularities of the sphenoid and other facial bones and enlargement of the foramina. To our knowledge, abnormalities of the orbital floor, however, have not been attributed to this disease in the literature until now. Two patients with neurofibromatosis had orbital floor involvement. Multiple neurofibromas eroding the orbital floor threatened vision in one patient. The other case, seen after trauma, had a condition simulating that of a blow-out fracture. Diagnostic workup with adequate roentgenography, especially tom

The results of treatment in 67 patients with associated injuries of the face and other regions of the body are analysed. Sixteen of them died. Their poor state was conditioned not by facial injuries but by traumas of other organs and system of the organism. It is concluded that such patients should be treated not in stomatological but in special traumatological departments for polytraumas. Surgery is first to be performed in the region where the damage is most hazardous. But the first step is to prevent aspiration of blood from the oral cavity into the respiratory tract followed by the fixatio

The possibilities for radical craniofacial restructuring have increased dramatically in the past 6 years with the development of craniofacial surgery. The field developed from a background of patients with major craniofacial birth defects allowing orderly planning and expansion to correction of a multitude of other craniofacial structural problems. The procedures concentrate upon changing the skeletal structures using extensive subperiostial dissection of soft tissue, and adding bone to fill in areas of deficiency. There are three grades of complexity in craniofacial procedures. After extensiv

As a rule should the half-mouth restorations in the frame of oral rehabilitation be done with gold inlays, overlays or full crowns. It is nevertheless possible to consider such restorations with amalgam and, providing that certain principles and technics and respected, such quadrant restorations could be valuable as well on the periodontal point of view as on the occlusal one. Regarding the periodontal point of view, it is now generally accepted that de restorations of the proximal aspects of the teeth have not to extend under the gingival margin. If this creates a risk for caries recurrences,

Current advances in vehicle design include nonlacerative windshields and energy-absorbing instrument panels and interior structures. In order to prevent more severe injuries, it may be necessary to accept minor soft tissue trauma and nasal fractures, but it should be possible to prevent almost all other facial trauma at reasonable impact speeds.

Thirty-eight patients with facial pain in the trigeminal distribution have been treated by percutaneous coagulation of the trigeminal ganglion. Although use has been made of radiological criteria, it has been found from the physiological data obtained from stimulation that motor and sensory phenomena provide more information of electrode placement and that good pain relief results when these criteria are met. The advantages of the method are: (i) minimal operative risk; (ii) control of the extent of the lesion created; (iii) small sensibility deficit; (iv) minimal complication rate; (v) short

A 3-month-old infant sustained trauma to the mandibular symphysis resulting in radiographic evidence of a fracture of the right condylar head. This presented the diffculty of performing a clinical examination and total reliance on history of a trauma and subsequent swelling. Multiple radiographs of the condyles were used to establish the diagnosis and rule out a film artifact. No definitive treatment was required other than muscle exercises and parental instruction as to potential problems.

At the present time several techniques of bone grafting for restoration of mandibular defects are being employed. These methods are well established by clinical trials and animal research. Other methods are in the experimental stage. Regardless of the technique utilized it remains paramount that the surgeon plan the initial procedure with great care as each failure results in diminished blood supply to the graft bed and makes subsequent procedures less likely to succeed. The well known principles of bone grafting involve establishing good nutritional status of the patient, proper preparation o

The indications for operative treatment of traumatically damaged optic nerves are given and the results from microsurgical transethmoid approaches are communicated. This operation method has such a low risk, that it can be also used on patients with recent head injuries. The early diagnosis of traumatic optic nerve damage and the earliest possible exposure of the nerve with pressure relief of the traumatised optic nerves are important for possible success. The development of microsurgery of the orbit, the possibility of a low-risk anaesthetic, the use of modern post-operative therapy and obser

A field of sensilla extends across the ventral surface of the terminal segment of the maxillary palps of Periplaneta americana. The sensilla project from a sheet of pliable cuticle. Ultrathin sections of the cuticle in this area reveal a clear-cut parabolic microfiber pattern. Microfibers can also be seen from freeze fracture faces running parallel to the cuticular surface. These microfibers have a diameter of 80 A and may consist of chitin crystallites surrounded by a matrix coat. The number of straight parallel microfibers visible in a fracture face increases the more closely parallel to the

In summary, the following program should be useful for the physician who encounters a child with a suspected eye injury: 1. Obtain a history. What was the child doing when he hurt his eye? 2. Examine the child by first inspecting the lids. Evert the lids with a cotton tipped applicator if necessary and if the globe is intact. Look at the canalicular area and decide whether the lacrimal drainage system is disrupted. Is a lid laceration deep enough to affect the levator? Could the orbital rim or any bony structures be involved? Should an x-ray film for fracture or foreign body be obtained? Is th

The authors describe the diagnosis and operative methods for the treatment of 36 patients which were operated on in the last two years. These patients included 32 with various face fractures, of which 7 had blow-out fractures. In 4 patients the diplopia was caused by injuries to the soft tissues without bony fracture. The examinations comprised x-ray methods and also ophthalmological examinations, especially with the synoptometer. In 24 operated patients, reposition of the bony fragments or autologous bone grafts were done according to the kind of injury present. Although 17 patients were oper

Condylar fractures with or without disruption of the temporo-mandibular joint occur commonly with other facial injuries. The subsequent development of ankylosis presents surgical and anaesthetic problems in management and results in severe physical disability to the patient. Oro-nasal fistula is a rare complication of fractures involving the middle third of the facial skeleton, and two cases are reported.

After a severe trauma of the face, e.g. fractures involving the zygoma, the upper jaw or other orbital bone alteraions and deviations of the bony orbital contours and also of the orbital contents can subsist, even after primary operative correction. The patients have functional as well as cosmetic complaints. In consequence of eyeball dislocation and incarceration or fibrosis of the external eye muscles, the patients also complain of diplopia in one or more directions. Due to the bony orbit enlargement and reduction of the orbtial contents, the eye is moved inferiorly and backwards, also causi

A detailed review was made of 1025 consecutive patients with "face-bone" fractures admitted to four Montreal hospitals over the five-year period 1958-1962, inclusive. In addition a survey was carried out of the other general hospitals in Greater Montreal in order to obtain admission figures for facial-bone fractures and for total hospital cases. In the study group the common causes of face-bone fractures were found to be fights, traffic accidents, falls, and athletic pursuits. Very few of these injuries occurred in industrial settings. A distinctly vulnerable group is made up of males between
