Full-field characterization of wishboning strain in the colobine mandibular symphysis.

The masticatory loading regime of lateral transverse bending (wishboning) is hypothesized to be instrumental in the evolution of symphyseal form among primates. The biomechanics of wishboning have largely been inferred by assuming that the mandible behaves as a curved beam under this load; however, the characterization of stress and strain in the anthropoid symphysis has been interpretively challenging. This is due, in part, to both limitations of sampling strain in an in vivo context and the incongruence of beam theory assumptions on the one hand, and the anatomical complexity of mandibular m

Mandibular condylar fractures and acute atlanto-axial subluxation Part 2 A physiopathological factor for the cervical spine sprain.

AIM: The aim of this study was to analyze the physiopathology of the acute cervical injure in the event of mandibular condylar fractures. METHODS: As in the Part 1, 25 non-consecutive cases of condylar mandibular fractures (16 males and 9 females, mean age: 22.96/range 14-36 years) observed and treated in the Maxillofacial Surgery Department of the University of L'Aquila, have been studied. Types of fractures examined included: unilateral: 19 cases (solitary: 12; associated with other mandibular fractures: 7, homolateral: 2); bilateral: 6 cases (equivalent: 2, not equivalent: 4). An unusual lateral luxation of an upper incisor owing to long-term boxing without protection.

This case report describes a lateral luxation of an upper incisor in a 19-year-old non-professional boxer. The tooth displacement gradually worsened over several months because the boxer received repeated blows to the head without using a mouthguard. Being a chronic dental trauma, rather than an acute lateral luxation, this case had several unique features: the labial plate of the alveolar bone was penetrated, the root apex was free in the vestibulum, the tooth was mobile (second degree) and radiographs revealed bone resorption. This case emphasized the need for a mouthguard to be used even wi

A new technique during septoplasty to prevent saddle nose.

During septoplasty, especially in patients with severe deviation of the nasal septum, there is the risk of septal instability, which may result in a saddle nose deformity. Therefore, prevention of this unwanted outcome is very important and removes surgeon's anxieties. This article describes a simple technique during septoplasty to prevent the development of this serious complication. In this surgical technique, the dislocated nasal septum is lifted and stabilised with a temporary traction suture. This traction suture holds the mobile septal cartilage in the proper position to restore and supp

Aesthetic reconstruction of a crooked nose via extracorporeal septoplasty.

BACKGROUND: The aim of this study was to evaluate the effectiveness, indications, and contraindications of the extracorporeal approach in treating a severely deviated nose. METHODS: Sixty-four patients operated on for aesthetic correction of a severely deviated nose were studied. Forty-six of them were first operations and 18 were revision cases. The surgical procedure of choice was the closed technique. Septal cartilage was prepared and then dislocated, followed by external alignment and reimplantation. Hump reduction with lateral osteotomies and, occasionally, medial dislocation of nasal bo

Morphological and histological findings after typical surgical manipulations on growing septal cartilage in rabbits.

One remaining problem of nose surgery is surgical correction of a prepubertal distortion of the septum. Unfortunately, the growing cartilage reacts differently upon a surgical intervention; in most cases this might cause iatrogenic disturbance of the development. Our clinical and histological studies with polydioxanone (PDS) foil in septal reconstruction showed that the PDS foil successfully prevented postoperative sequelae like dislocation of the reimplanted cartilage fragments and recurrent deviation caused by overlapping of the cartilage borders. Furthermore, it seemed to stimulate cartilag

Condylar injury: magnetic resonance imaging findings of temporomandibular joint soft-tissue changes.

Recent studies have suggested magnetic resonance imaging (MRI) as useful in the evaluation of soft-tissue changes that occur in the temporomandibular joint (TMJ) after acute condylar trauma. The aim of this study was to investigate whether MRI findings of disc displacement, capsular tear and haemarthrosis are linked to the degree of condylar injury. Nineteen patients were assigned a diagnosis of uni- or bilateral condylar fracture (n=17), or condylar contusion with a uni- or bilateral diagnosis of TMJ sprain/strain (n=2). Condylar injuries were classified as grade I (absence of condylar fractu

Management of the long nose: review of techniques for nasal tip supporting structures.

The long nose with a "plunging" tip is a deformity that involves an inferiorly rotated nasal tip, leading to an increase in the length of the nose. The anatomic basis of the long nose with a plunging tip may be divided into two types. Type 1 presents a normal alar-cartilage complex inferiorly displaced by a long nasal septum and long upper lateral cartilages. Type 2 is caused by a dislocation of the alar cartilages downward from the aponeurotic attachments to the septal angle. During the study period, the authors identified 60 patients with long noses and plunging tips. For 22 patients with ty

Finite-element modeling of the anthropoid mandible: the effects of altered boundary conditions.

Finite-element modeling provides a full-field method for describing the stress environment of the skull. The utility of finite-element models, however, remains uncertain given our ignorance of whether such models validly portray states of stress and strain. For example, the effects of boundary conditions that are chosen to represent the mechanical environment in vivo are largely unknown. We conducted an in vitro strain gauge experiment on a fresh, fully dentate adult mandible of Macaca fascicularis to model a simplified loading regime by finite-element analysis for purposes of model validation

Ontogeny, function, and scaling of the mandibular symphysis in papionin primates.

In vivo study of mastication in adult cercopithecine primates demonstrates a link between mandibular symphyseal form and resistance to "wishboning," or lateral transverse bending. Mechanical consideration of wishboning at the symphysis indicates exponentially higher stresses along the lingual surface with increasing symphyseal curvature. Lengthening the anteroposterior width of the symphysis acts to resist these higher loads. Interspecific adult cercopithecine allometries show that both symphyseal curvature and symphyseal width exhibit positive allometry relative to body mass. The experimental

Repetitive strain injury. 2. Diagnostic and treatment tips on six common problems. The Goff Group.

Repetitive strain injury is caused by recurrent overuse, resulting in microtrauma to tissues. Local pain and tenderness, weakness, inflammation, and limited function are common findings. Some of the strain injuries seen most often are carpal tunnel syndrome, trigger finger, shoulder impingement syndrome, tennis elbow, thoracic outlet syndrome, and myofascial pain disorders. Often, treatment can be started at the initial visit, after systemic disorders have been ruled out. A vital step is elimination of aggravating factors, such as improper posture, inadequate attention to ergonomic factors at

Weakness in mouse masticatory muscles by repetitive contractions with forced lengthening.

The etiology of myofascial tenderness and pain of masticatory muscles in humans is difficult to understand. Parafunctional oral habits such as tooth grinding or vigorous chewing are thought to be factors. The objective of this study was to determine if masticatory muscles are susceptible to weakness and injury induced by repetitive, dynamic, forced-lengthening contractions. Results would support the hypothesis that contraction-induced injuries could occur in hyperactive masticatory muscles of humans in response to parafunctional oral habits. Mice were anesthetized and randomly assigned to thre

The effect of the mandatory use of face masks on facial lacerations and head and neck injuries in ice hockey. A prospective study.

A 4-year prospective review of lost-time injuries and facial lacerations was performed for a National Collegiate Athletic Association Division I, intercollegiate ice hockey team. The total injury exposure time consisted of 798.5 practice hours and 163 games. There were 16 facial lacerations, with an incidence of 14.9 per 1000 player-game hours and 0.1 per 1000 player-practice hours; both incidences were found to be less than in previous comparable studies where the use of face masks was not mandatory. In addition, there were eight lost-time head and neck injuries that accounted for 6.3% of all

Functional and aesthetic nasal reconstruction in unilateral CLP-deformity.

The correction of nasal deformities in cleft patients presents a challenge in the secondary treatment of this malformation. The complexity of the nasal deformity in cleft patients partly depends on the congenital anomaly itself and partly on the surgical procedures which were previously carried out. The nasal deformities in unilateral CLP patients are typical. The nasal floor and the nostril sill are mainly constructed during primary lip repair. While the ala on the cleft side is repositioned, the septum deformity and the anterior spine dislocation are not touched. If large circular incisions

Outcome following epineurotomy in carpal tunnel syndrome: a prospective, randomized clinical trial.

To assess the efficacy of an adjunctive epineurotomy in carpal tunnel syndrome, 36 wrists in 33 patients were prospectively randomized into epineurotomy and non-epineurotomy treatment groups. The operating surgeons and evaluating therapist were double-blinded. Patients were evaluated preoperatively and at either 6 or 12 months postoperatively, with 15 patients examined at both postoperative time points. Evaluation consisted of five sensory and four motor tests: static and moving two-point discrimination, Semmes-Weinstein monofilament testing, vibratory (30 and 256 cps) sensibility, and dynamom

[A personal rhinoplasty concept for patients with cleft lip, jaw, palate].

In the treatment of patients with cheilognathouranoschisis, rhinoplasty is a great challenge, since the cleft lip and palate will also produce nasal deformation requiring surgical correction. Unilateral cleft lip and palate is usually associated with a pronounced septal deformity resulting in "crooked nose" as well as typical asymmetry of the apex of the nose and of the nasal vestibule. The anterior part of the septum is dislocated in the direction of the unaffected side and the lower border of the septum is at the same time subluxated to the opposite side. The dorsal part of the septum presen

[Surgical approach to the nasal valve stenosis].

Nasal valve, originally defined by Mink (1903), has changed its concept and is now regarded as slit-like opening between the caudal end of the upper lateral cartilage and the septum. Contrary to Caucasians whose smallest area of the nose lies at the nasal valve, the narrowest part of Japanese nose situates at the area between the anterior portion of the inferior turbinate and the septum. Thus, we have fewer chance to experience patients with stenotic nasal valve. Six patients complaining nasal obstruction resulted from nasal valve stenosis, mainly due to dislocated or collapsed upper lateral c
