The State of the Art in Septoplasty: A Review of the Latest Achievements.

The deviated nasal septum is the most common cause of nasal airway obstruction, frequently diagnosed by otolaryngologists in clinical practice. Reports displayed that this condition affects 70%-80% of the population. The etiology of this upper airway problem is mainly related to intrinsic deformity or quadrangular cartilage dislocation from its bony borders, which can finally confer different disorders, such as breathing problems, sinus infections, snoring, headaches, exacerbating sleep apnea, and sleep disturbances. One of the most common methods for correcting a deviated nasal septum is sept

Deviated Nasal Septum a Risk Factor for the Occurrence of Chronic Rhinosinusitis.

The aim of this review article is to determine whether a deviated nasal septum (DNS) is a potential risk factor for the occurrence of chronic rhinosinusitis (CRS). Nasal septal deformities include spur, deviated nasal septum, thickening, and dislocation. Deformities of the nose tip and columella are examples of external deformities, together with the deviated part of either cartilage or both the bony and cartilage part of the dorsum of the nose. Various symptoms of chronic rhinosinusitis include nasal obstruction, nasal or post-nasal drainage, facial pain and pressure, and smell disturbances.

The Deviated Nasal Tip: A Personal Technique for Correction Using Monolateral Crural Overlay and Monolateral Dome Truncation.

This study aimed to present a novel approach to correct nasal tip deviation with monolateral crural overlay or monolateral dome truncation, presenting as an isolated deformity or in complex nose deviations. Nasal tip deviation can be congenital or posttraumatic, due to a dislocated septum or cartilaginous septal or lower lateral cartilage malformations. Although some treatment strategies have been introduced, appropriate treatment remains a challenge because of the complexity and variability of such deformities. It had been assumed that in most nasal tip deviations, a lower lateral cartilage w

A new persistent suture technique for correction of caudal septal dislocation.

INTRODUCTION: Caudal septal dislocation is both a functional and an aesthetic problem. The caudal septum that causes nasal obstruction displaces the right or left of columella. Despite the various maneuvers currently available for treatment, this issue is still controversial. We describe a new technique that uses a nonabsorbable suture material to keep the septum in the midline in mild and moderate caudal septal dislocations and can be used in addition to other current techniques. SURGICAL TECHNIQUE: A full-length (lengthways) pocket is opened between the medial cruras, and the dislocated cau

Repositioning of the caudal septal dislocations with notching and suturing the cartilage to the nasal spine.

We aim to define a cartilage reshaping and repositioning technique for caudal septal dislocations, with the logic of adapting the smoother tissues to the stiffer structures. The study was designed retrospectively with the patient charts and photographic analysis. Thirty-six patients who had septal surgery in a septoplasty or an open approach septorhinoplasty operation and meet the inclusion criteria were evaluated for the study. Postoperative success of the technique was based on preoperative and postoperative photographic analysis by two independent otorhinolaryngologists. The estimators clas

Alar web in cleft lip nose deformity: study in adult unilateral clefts.

BACKGROUND: The correction of alar webbing in unilateral cleft lip nose deformity is challenging because of progressive distortions in the alar web region during the period of growth. Alar webbing is a persistent universal deformity in both the primary and secondary cleft lip noses. The purpose of this article is to study the alar web deformity in adult patients with unilateral cleft lip noses. METHODS: Twenty-five patients aged 13 years and older presenting with unilateral cleft nasal deformity were included. Preoperative and postoperative measurements of the nose, along with detailed intrao

The efficacy of bioabsorbable mesh as an internal splint in primary septoplasty.

Nasal bone fractures are often accompanied by septal fractures or deformity. Posttraumatic nasal deformity is usually caused by septal fractures. Submucosal resection and septoplasty are commonly used surgical techniques for the correction of septal deviation. However, septal perforation or saddle nose deformity is a known complication of submucosal resection. Hence, we chose to perform septoplasty, which is a less invasive procedure, as the primary treatment for nasal bone fractures accompanied by septal fractures. During septoplasty, we used a bioabsorbable mesh as an internal splint. We use

Structure rhinoplasty of the long nose.

Rhinoplasty modifies the aesthetic appearance and functional properties of the nose with operative manipulation of the skin, underlying cartilage, bone, and linings. A long nose is an aesthetically undesired feature disturbing the harmony of the face. The underlying pathology of the long nose may be due to either a long septum that invades the lip or dislocation of the alar cartilages downward from the aponeurotic attachments to the septal angle. The increase in the nasal height due to a dorsal nasal hump may give an illusion of a long nose. An absent or shallow frontonasal angle also gives th

A new technique during septoplasty to prevent saddle nose.

During septoplasty, especially in patients with severe deviation of the nasal septum, there is the risk of septal instability, which may result in a saddle nose deformity. Therefore, prevention of this unwanted outcome is very important and removes surgeon's anxieties. This article describes a simple technique during septoplasty to prevent the development of this serious complication. In this surgical technique, the dislocated nasal septum is lifted and stabilised with a temporary traction suture. This traction suture holds the mobile septal cartilage in the proper position to restore and supp

Morphological and histological findings after typical surgical manipulations on growing septal cartilage in rabbits.

One remaining problem of nose surgery is surgical correction of a prepubertal distortion of the septum. Unfortunately, the growing cartilage reacts differently upon a surgical intervention; in most cases this might cause iatrogenic disturbance of the development. Our clinical and histological studies with polydioxanone (PDS) foil in septal reconstruction showed that the PDS foil successfully prevented postoperative sequelae like dislocation of the reimplanted cartilage fragments and recurrent deviation caused by overlapping of the cartilage borders. Furthermore, it seemed to stimulate cartilag

Management of the long nose: review of techniques for nasal tip supporting structures.

The long nose with a "plunging" tip is a deformity that involves an inferiorly rotated nasal tip, leading to an increase in the length of the nose. The anatomic basis of the long nose with a plunging tip may be divided into two types. Type 1 presents a normal alar-cartilage complex inferiorly displaced by a long nasal septum and long upper lateral cartilages. Type 2 is caused by a dislocation of the alar cartilages downward from the aponeurotic attachments to the septal angle. During the study period, the authors identified 60 patients with long noses and plunging tips. For 22 patients with ty

Functional and aesthetic nasal reconstruction in unilateral CLP-deformity.

The correction of nasal deformities in cleft patients presents a challenge in the secondary treatment of this malformation. The complexity of the nasal deformity in cleft patients partly depends on the congenital anomaly itself and partly on the surgical procedures which were previously carried out. The nasal deformities in unilateral CLP patients are typical. The nasal floor and the nostril sill are mainly constructed during primary lip repair. While the ala on the cleft side is repositioned, the septum deformity and the anterior spine dislocation are not touched. If large circular incisions

[A personal rhinoplasty concept for patients with cleft lip, jaw, palate].

In the treatment of patients with cheilognathouranoschisis, rhinoplasty is a great challenge, since the cleft lip and palate will also produce nasal deformation requiring surgical correction. Unilateral cleft lip and palate is usually associated with a pronounced septal deformity resulting in "crooked nose" as well as typical asymmetry of the apex of the nose and of the nasal vestibule. The anterior part of the septum is dislocated in the direction of the unaffected side and the lower border of the septum is at the same time subluxated to the opposite side. The dorsal part of the septum presen

[Surgical approach to the nasal valve stenosis].

Nasal valve, originally defined by Mink (1903), has changed its concept and is now regarded as slit-like opening between the caudal end of the upper lateral cartilage and the septum. Contrary to Caucasians whose smallest area of the nose lies at the nasal valve, the narrowest part of Japanese nose situates at the area between the anterior portion of the inferior turbinate and the septum. Thus, we have fewer chance to experience patients with stenotic nasal valve. Six patients complaining nasal obstruction resulted from nasal valve stenosis, mainly due to dislocated or collapsed upper lateral c

Wound healing of autologous implants in the nasal septal cartilage.

The cartilaginous nasal septum of the child is often involved in trauma or congenital deformation. In perspective of its role in postnatal growth of the midface, the wound healing of septal cartilage in reaction to various surgical procedures was studied in young rabbits. It was concluded that (1) elevation of mucoperichondrium does not induce significant histologic reactions in the septal cartilage and (2) the wound reaction of cut edges includes only a +/- 0.8-mm-deep zone and lasts no longer than 2 weeks, resulting in a stump, encapsulated by fibrous tissue. Reconnection of separated parts
