Tissue-Engineered Fibrous Nerve Conduits Link Facial Nerve Repair with Central Network Reorganization.

Peripheral nerve injury leads to persistent motor and sensory dysfunction and is increasingly recognized to induce alterations in central sensorimotor networks. However, most current repair strategies primarily focus on peripheral regeneration, with limited understanding of its relationship to central neural changes. Here, we performed clinical resting-state functional magnetic resonance (rs-fMRI) in patients with facial nerve injury and identified the sensorimotor network alterations associated with the nerve injury. Guided by the results, we developed a tissue-engineered fibrous nerve guidan

Anatomical variation in the cervical branch of the facial nerve: a systematic review.

BACKGROUND: There is a paucity of literature on the anatomical variations of the cervical branch of the facial nerve (CBFN). Poor awareness of its anatomy and variations risks intraoperative damage, misdiagnosis of facial nerve injury and undesirable functional and aesthetic outcomes for patients. PURPOSE: To review anatomical studies on the CBFN and synthesise data on its course, innervations and common variations. METHODS: A PRISMA-compliant systematic review of anatomical studies of the cervical branch of the facial nerve was conducted. PubMed, Embase, Scopus, Cochrane Library, Web of Sci

Combined pharmacological targeting of NAD+ metabolism preserves central and peripheral axons in an ex vivo model of Wallerian degeneration.

Wallerian degeneration (WD) is a conserved axonal self-destruction program implicated in a spectrum of neurological conditions, including peripheral neuropathies, traumatic brain injury, and possibly neurodegenerative diseases. Wallerian degeneration is triggered by depletion of the NAD+-synthesizing enzyme NMNAT2, subsequent NMN accumulation, and activation of the NAD+-hydrolyzing SARM1, ultimately driving axonal fragmentation. While pharmacological inhibition of SARM1 is an emerging therapeutic strategy, current challenges due to paradoxical toxicity with orthosteric base-exchange SARM1 inhi

Nerve Injury During Thyroid Radiofrequency Ablation: Mechanisms, Prevention, and Practical Management.

Voice change is an uncommon but clinically important complication of thyroid radiofrequency ablation (RFA), because it may indicate injury to the recurrent laryngeal nerve or, less commonly, the vagus nerve. Early recognition and appropriate intervention are important, because neural dysfunction may remain reversible before the occurrence of irreversible structural nerve injury. Thermal nerve injury during thyroid RFA is not always a single instantaneous event. Rather, it may involve direct thermal exposure of the nerves, leading to acute endoneurial or perineural edema, microvascular compromi

The effect of routine intraoperative neuromonitoring on thyroidectomy complications: A propensity score-matched retrospective cohort study.

Intraoperative neuromonitoring (IONM) is commonly used during thyroid surgery to facilitate recurrent laryngeal nerve (RLN) identification and potentially reduce related postoperative complications. However, the efficacy of routine IONM in decreasing complication rates remains debated. In this study, we investigated the impact of IONM on postoperative complications after thyroidectomy. This retrospective single-center study included 214 thyroidectomy operations performed between 2021 and 2024. Patients were divided into 2 groups: Group 1 (n = 107) underwent surgery with routine IONM, and Group

Image-Guided Cervicotomy and Recurrent Laryngeal Nerve Reinnervation in Zone II Glass Neck Trauma: A Case Report.

BACKGROUND Hemodynamically stable penetrating neck trauma with retained deep foreign bodies can conceal functionally significant neural injury despite absence of major vascular findings on computed tomography angiography. In selected cases, early laryngoscopy may provide decisive functional evidence and help identify occult recurrent laryngeal nerve injury requiring timely surgical exploration. CASE REPORT A 45-year-old woman presented with penetrating neck trauma caused by multiple glass fragments, with Zone II entry wounds and a deep transcervical trajectory across zones. She was hemodynamic

Presentation and management of tislelizumab induced toxic optic neuropathy.

Immune checkpoint inhibitors (ICIs) have revolutionised the management of advanced malignancies, but can cause immune-related adverse events (irAEs), including vision-threatening complications such as optic neuropathy. Ocular irAEs have been described with several programmed death-1/programmed death-ligand 1 and cytotoxic T lymphocyte-associated antigen-4 inhibitors, but ocular toxicities of tislelizumab have so far been limited to Vogt-Koyanagi-Harada (VKH)-like uveitis, panuveitis and retinal vasculitis. We describe a woman with metastatic lung adenocarcinoma who was treated with second-line

Long-Term Outcomes of Spinal Accessory Nerve to Musculocutaneous Nerve Transfer in Brachial Plexus Birth Injuries.

PURPOSE: Brachial plexus birth injuries (BPBI) can be life-changing for both the patient and family. While most resolve spontaneously, surgical intervention is indicated for those who fail to improve. This can be performed via nerve grafting or nerve transfer. The spinal accessory nerve is a commonly used extraplexal donor in brachial plexus surgery. In cases of BPBI, it can be transferred to the musculocutaneous nerve to restore elbow flexion. While there are short-term reports in BPBI, few, if any, long-term outcome data are available. METHODS: A retrospective review of patients who underwe

A Study of Time to Recovery Following Loss of Neuromonitoring Signal of the Recurrent Laryngeal Nerve in Thyroid Surgery.

BACKGROUND: Injuries of the recurrent laryngeal nerve (RLN) during thyroidectomy although infrequent can lead to major morbidity. Permanent RLN injury is uncommon; however, temporary neurapraxia and loss of signal (LOS) during intraoperative neuromonitoring (IONM) are seen more frequently. This study aimed to identify factors associated with type I (segmental) and II (global) LOS of the RLN during thyroid surgery and to analyze time to recovery of vocal cord function. METHODS: This observational retrospective cohort study included 3806 patients (2924 female, 76.8% and 882 male, 23.2%) who und

Dual-Modality Bioactive Repair of Facial Nerve Injury Using PEG-Mediated Axonal Fusion and Extracellular Vesicles-Enriched Human GMSC Secretome.

Facial nerve injuries may cause severe functional deficits with limited recovery after standard neurorrhaphy. Polyethylene glycol (PEG)-mediated axonal fusion enables immediate structural reconnection and prevents Wallerian degeneration, while gingiva-derived mesenchymal stem cell (GMSC) secretome provides a potent, acellular trophic stimulus that supports neuronal survival and remyelination. We report a dual-modality bioactive repair interface integrating PEG fusion with human GMSC secretome to accelerate neural repair. Fluorescently labeled secretome components were rapidly internalized by n

Transcervical vs. Transcervical-Combined Surgical Approaches for Primary Parapharyngeal Space Tumors: A Systematic Review of Surgical and Functional Outcomes.

Primary parapharyngeal space (PPS) tumors are rare neoplasms comprising 0.5% of head and neck tumors. Their complex anatomical location and proximity to critical neurovascular structures pose significant surgical challenges. While transcervical and transcervical-combined approaches represent the primary surgical techniques, no comprehensive systematic comparison exists. This systematic review compared surgical success, complications, functional outcomes, and recurrence rates between these approaches. Following PROSPERO registration (CRD420251037201), we searched PubMed, Cochrane, Web of Scienc
