Fulminant idiopathic intracranial hypertension presenting as sudden bilateral vision loss.

Fulminant idiopathic intracranial hypertension (IIH) is an uncommon, rapidly progressive form of IIH characterised by an acute decline in vision over days to weeks. It is a neuro-ophthalmic emergency in which immediate effective treatment is imperative to prevent irreversible optic nerve injury and permanent vision loss. We report a case of a woman who presented with a sudden onset of recurrent headaches and progressively worsening diminution of vision in both eyes. She was diagnosed with IIH. Despite intensive medical management and therapeutic lumbar puncture, her vision deteriorated signifi

Organophosphate pesticide exerts toxic effect on the optic nerve of glaucoma rats by promoting oxidative stress and inflammation.

Glaucoma, a leading cause of irreversible blindness, involves progressive retinal ganglion cell (RGC) loss. Beyond intraocular pressure (IOP), environmental risk factors like pesticide exposure are increasingly implicated. Dimethyl phosphate (DMP), a key metabolite of organophosphorus pesticides, accumulates in the body and exhibits systemic toxicity. However, its direct role and mechanism in glaucoma pathogenesis remain entirely unexplored. We investigated the impact of DMP on glaucoma progression using a rat glaucoma model. Animals were subjected to DMP exposure at varying concentrations. We

Human retinal organoids functionally bridge a transected optic nerve in a rat model.

Functional restoration of vision in adult mammals remains unattainable following complete transection of the optic nerve. In this study, we address this challenge by heterotopically transplanting human retinal organoids into the transection cavity of the optic nerve in adult rats. One month after transplantation, treated animals exhibited partial recovery of visual function, including a direct pupillary light reflex, visually evoked electrophysiological responses, and visually guided behaviors, which were sustained for at least 3 months. Neuroanatomical tracing and three-dimensional imaging re

Differential resilience against complete optic nerve transection across visual areas.

Understanding how the central visual system responds to complete optic nerve transection (ONT) is critical for evaluating the feasibility of whole-eye transplantation (WET), as envisioned by the 2024 ARPA-H THETA program. This study evaluates the dynamitic neuronal and synaptic changes within the posterior visual pathway-the superior colliculus (SC), lateral geniculate nucleus (LGN), and primary visual cortex (V1)-after optic nerve crush (ONC) and ONT in mice models. Complementary observations were obtained in non-human primates. Both injury models induced time-dependent alterations, but degen

Targeting prostaglandin catabolism via topical nanotherapy rescues vision in ischemic optic neuropathy.

Ischemic optic neuropathy is a leading cause of acute vision loss and currently lacks effective therapy. Here, we identify 15-hydroxyprostaglandin dehydrogenase (15-PGDH), the key enzyme responsible for prostaglandin degradation, as a metabolic checkpoint in ischemic optic neurodegeneration. Analysis of human ischemic optic nerve tissue and a murine model of optic nerve ischemia reveals marked upregulation of retinal 15-PGDH, suggesting dysregulated prostaglandin homeostasis after ischemic injury. To therapeutically target this pathway, we develop a nano-micellar formulation of the hydrophobic

Neuroinflammatory regulatory role of microglia in optic nerve injury: from pathological mechanisms to therapeutic targets.

Optic nerve injury, encompassing conditions such as glaucoma, optic neuritis, and traumatic optic neuropathy, is a major cause of irreversible vision loss. Traditional broad-spectrum anti-inflammatory treatments have shown limited efficacy, highlighting the need for precision-based therapeutic approaches grounded in the underlying pathological mechanisms. As the primary immune cells of the central nervous system (CNS), microglia play a crucial role in regulating neuroinflammation following optic nerve injury. This review provides a comprehensive overview of the mechanisms governing microglial

Identification of a Small-Molecule Modulator of Astrocyte Reactivity for Optic Nerve Protection.

PURPOSE: Injury of the optic nerve leads to retinal ganglion cells (RGCs) apoptosis and irreversible vision loss, in which reactive astrocytes play a central role. The aim of this study is to modulate pathological reactive astrocytes to reduce the progression of optic nerve degeneration. METHODS: Given the therapeutic potential of small molecules to modulate astrocyte reactivity, we used a drug-screening platform to identify small molecules, and evaluated their capacity to regulate astrocyte phenotypes and preserve RGCs after optic nerve crush (ONC). The primary astrocytes from neonatal C57BL

Multifunctional Catechol-Functionalized Cellulose Hydrogels for the Minimally Invasive Treatment of Acute Optic Nerve Injuries.

Oxidative stress-induced retinal ganglion cell degeneration is a major pathological feature of acute optic nerve injury, yet current posterior-segment therapies are limited by poor local retention and repeated invasive administration. Here, we developed an injectable catechol-functionalized carboxymethyl cellulose hydrogel, CMCDA, as a bioadhesive and antioxidative intravitreal platform. Through dopamine grafting and oxidative crosslinking, CMCDA exhibited shear-thinning injectability, self-healing behavior, wet-tissue adhesion, controlled biodegradability, and good biocompatibility. Comparison of volumes of brain areas in patients with bilateral early high-tension and normal-tension glaucoma in 7 Tesla MRI.

BACKGROUND: Glaucoma is an optic neuropathy characterized by progressive retinal ganglion cells degeneration and associated visual field defects. Although elevated intraocular pressure is a major risk factor, glaucoma can also develop in individuals with statistically normal intraocular pressure, implying alternative mechanisms such as neurotrophins deprivation and primary lateral geniculate nucleus injury. These mechanisms may suggest a neurodegenerative aspect of the disease. METHODS: In this study, we evaluated 15 patients with bilateral early-stage normal-tension glaucoma, 10 with bilater

Glaucoma and Autoimmunity: Immunopathogenic Mechanisms and Emerging Immunomodulatory Therapies.

Glaucoma is a chronic progressive optic neuropathy and one of the leading causes of irreversible blindness worldwide. Although elevated intraocular pressure remains the most important modifiable risk factor, increasing evidence suggests that immune dysregulation and autoimmune responses also contribute substantially to disease onset and progression. Clinical studies across different glaucoma subtypes have identified subtype-dependent immune abnormalities, including altered serum autoantibody profiles, dysregulated cytokine and chemokine expression, and changes in peripheral immune cell subsets

Oxidative Stress in Glaucoma: From Pathogenic Mechanisms to Emerging Antioxidant Therapies.

Glaucoma is the leading cause of irreversible blindness worldwide and is characterized by progressive retinal ganglion cell (RGC) loss and optic nerve degeneration. While elevated intraocular pressure (IOP) remains the primary modifiable risk factor, a certain proportion of patients continue to deteriorate despite adequate IOP control, pointing to IOP-independent mechanisms of neurodegeneration. Oxidative stress-defined as an imbalance between the production of reactive oxygen species and the capacity of endogenous antioxidant defenses-has emerged as a central, multi-tiered contributor to glau

Reno-ocular syndromes: pathophysiological mechanisms linking kidney and ocular disorders.

The frequent association between renal and ocular anomalies suggests a common pathophysiological axis between the genetic and molecular mechanisms of the kidneys and multiple ocular structures during tissue formation, differentiation, and remodeling. The search was conducted in the PubMed, SciELO, Scopus, and Web of Science databases. This review article focuses on the common molecular and genetic bases of renal and ocular involvement in both systemic diseases and rare syndromes. The interdependence between renal and ocular morphogenesis is mediated by conserved molecular mechanisms, notably t

YAP Regulates the Nrf2 Signaling Axis to Attenuate Oxidative Stress and Neuroinflammation in Retinal Ganglion Cell Degeneration.

PURPOSE: Oxidative stress is a key driver of retinal ganglion cell (RGC) degeneration after optic nerve injury. Yes-associated protein (YAP), a Hippo pathway effector, is known to reprogram stress responses, yet its role in regulating oxidative stress during RGC degeneration is unclear. METHODS: This study investigated the role of YAP in RGC injury using an in vivo optic nerve crush (ONC) model and an in vitro oxidative-stress model with primary RGCs. YAP expression was modulated pharmacologically and genetically. We assessed its effects on nuclear factor erythroid 2-related factor 2 (Nrf2) s

Targeting FBXO48 with BC1618 promotes axon regeneration and alleviates symptoms of Parkinson's disease.

INTRODUCTION: Neurons depend on the ubiquitin system to maintain proteostasis, which plays crucial roles in processes of neuronal death and axon regeneration after injuries and diseases. Developing druggable targets within the ubiquitin system that simultaneously support neuroprotection and axon regeneration has been considered an attractive therapeutic strategy. OBJECTIVES: To systematically identify druggable molecules within the ubiquitin signaling pathway and evaluate their efficacy in neuroprotection. METHODS: To this end, we screened 181 ubiquitination-related small-molecule compounds

Total RNA Sequencing Reveals Early Coding, Noncoding, and Isoform-Level Transcriptomic Changes Following Traumatic and Glaucomatous Optic Nerve Injury.

PURPOSE: Retinal ganglion cell (RGC) death following optic nerve injury is driven by transcriptional programs involved in apoptosis, neuroinflammation, and other regulated cell death pathways; however, the early transcriptomic changes initiating these processes remain poorly defined. We used rRNA-depleted total RNA sequencing to characterize early alterations in protein-coding genes, long noncoding RNAs (lncRNAs), and alternative splicing events in mouse retinas following optic nerve crush (ONC) and silicone oil-induced ocular hypertension under-detected (SOHU), prior to substantial RGC loss.

Decoding Visual Pathway Dysfunction with SERF-MEG: A Study in Patients with Optic Neuropathy.

This study aimed to characterize cortical dysfunction and frequency-specific network reorganization following optic nerve injury using spin-exchange relaxation-free magnetoencephalography (SERF-MEG), and to assess the potential of MEG-derived multiscale features as sensitive functional biomarkers for clinical evaluation. In this prospective case-control study, SERF-MEG recordings were acquired during a pattern-reversal visual stimulation paradigm. Time-domain evoked components (M100/M135), global electrophysiological indices, energy-based metrics, and alpha- and beta-band phase-based functiona

Irbesartan Promotes Retinal Ganglion Cell Survival and Modulates Mitochondrial and Inflammatory Pathways in Mouse Retinal Explant and in Optic Nerve Crush Models.

Glaucoma is a neurodegenerative eye disease characterized by progressive loss of retinal ganglion cells (RGCs), resulting in irreversible vision loss. Previous work demonstrated that irbesartan, an angiotensin II type I receptor blocker, promotes RGC survival in rat retinal explants. This study extends these findings to ex vivo mouse explants, in vivo optic nerve crush model, and transcriptomic analysis to identify associated pathways. Retinal explants from C57BL/6 mice were cultured for 4 days with irbesartan (10 μmol/L), vehicle, or angiotensin II (2 μmol/L). RGC survival was assessed by βII

Neurodevelopmental miR-135a-1-3p and miR-135b-3p promote long-distance axon regeneration after injury in the central visual pathway.

Micro-RNAs (miRNAs) play roles in the mammalian central nervous system (CNS) development and disease but the functions of specific miRNA families' members in defined neuronal types are poorly understood. Here, we characterized the miR-135 family members in the mouse CNS retinal ganglion cell (RGC) and cortical projection neurons development, and tested whether reverting expression toward embryonic levels for the identified developmentally regulated miR-135 family members may reactivate the intrinsic axon growth capacity following axonal injury in the adult CNS. We found that two-out-of-five mi

Systemic diseases to the mechanism of primary open angle glaucoma.

Primary open angle glaucoma (POAG) is a complex neurodegenerative disorder involving genetic susceptibility, environmental factors, and systemic abnormalities, whose pathogenesis has not been fully elucidated. Based on baseline intraocular pressure (IOP), POAG is classified into two subtypes: high-tension glaucoma (HTG) and normal-tension glaucoma (NTG). This review summarizes and integrates recent research advances, focusing on the roles of systemic disorders including cardiovascular, endocrine, neurodegenerative, and autoimmune diseases, as well as obstructive sleep apnea-hypopnea syndrome (
