Endoscopic Decompression of Supraorbital Fissure Syndrome After Zygomaticomaxillary Fracture.

The Superior Orbital Fissure Syndrome (SOFS) is a rare complication after craniofacial trauma. According to Hirschfeld, who first described SOFS in 1858, and Lakke, who further defined the complete SOFS, this syndrome results from damage to structures passing through the superior orbital fissure. The structures passing via the superior orbital fissure include the lacrimal nerve, nasociliary nerve, oculomotor nerve, abducens nerve, superior orbital nerve, trochlear nerve, and superior ophthalmic vein. The symptoms of supraorbital fissure syndrome are ophthalmoplegia, ptosis, extraocular muscle

Non-anatomical MPFL reconstruction and failure to address relevant pathologies are common reasons for treatment injuries following treatment for patellofemoral instability.

PURPOSE: Management of patellofemoral instability (PFI) tailored to the individual patoanatomy can be complex with potential pitfalls. However, reports on complications and suboptimal outcomes remain limited. The aim of this study was to analyse problems related to PFI treatment in Denmark. This knowledge can be used to optimize future treatment of PFI. METHODS: Compensation claims from 2004 to 2025 related to PFI were identified in the Danish Patient Compensation Association. Medical records and imaging were independently reviewed by two observers. Radiographs and magnetic resonance imaging

Secondary Brown's syndrome following fourth nerve palsy - case series and review of literature.

Introduction: Restriction of eye movements following fourth nerve palsy has been attributed to orbital injury in the past. In this report, we challenge this view. Methods: In a retrospective case series, we analyzed seven consecutive patients with a Brown's syndrome that evolved after a fourth nerve palsy. Results: All patients presented with fourth nerve palsies following trauma or meningioma surgery. After a delay of months, the fourth nerve palsy improved but the patients developed a limitation in adduction and upgaze in the same eye. None of the cases involved an injury to the orbit and th

Analysis of Pontine cavernous malformation resection based on 3D microanatomical study.

Pontine cavernous malformations (PCMs), the most common subset of brainstem cavernous malformations (BSCMs), carry a high surgical risk. This study analyzes surgical strategies based on anatomical and clinical considerations. Ten Klingler-prepared human brainstem specimens underwent microdissection to map critical pontine nerve fiber tracts. Ten additional specimens simulated surgical approaches to delineate safe exposure ranges, followed by validation in clinical cases. PCMs surgical regions were categorized as ventral, ventrolateral, and dorsolateral. The trigeminal nerve subdivides the vent

Orbital Apex Syndrome Resulting From Trauma: Recovery Outcomes Following Surgical Decompression of the Orbital Wall.

While orbital apex syndrome is considered a rare condition, it has significant neuro-ophthalmic implications for diagnosis and management. It is characterized by the involvement of the following cranial nerves: the optic nerve, the oculomotor nerve, the trochlear nerve, the abducens nerve, and the first division of the trigeminal nerve, manifesting as clinical symptoms upon their impairment. Although various factors can be causal, trauma can also lead to the development of orbital apex syndrome. Regardless of the cause, treatment often involves inducing decompression of the orbital space using

Bifurcation of the trochlear nerve: cadaveric case report and literature review.

BACKGROUND: The trochlear nerve [the fourth cranial nerve (IV CN)], one of the extraocular motor cranial nerves, exits the midbrain at its dorsal side, runs inside the lateral wall of the cavernous sinus (CS), and innervates the superior oblique muscle. In the wall of the CS, the IV CN travels in the vicinity of the oculomotor and ophthalmic nerves. These nerves border the infratrochlear and supratrochlear triangles (ST), which are part of a parasellar group of triangles of the CS, and provide safe entry zones to the inside of the CS. CASE REPORT: During standard anatomical dissection, the un

Trochlear Nerve Palsy: A Systematic Review of Etiologies and Diagnostic Insights.

Background/Objectives: Trochlear nerve palsy (TNP) is a clinically significant neuro-ophthalmic disorder with a broad and heterogeneous etiologic spectrum. Due to the trochlear nerve (TN)'s long intracranial course and its proximity to key neurosurgical corridors, it is particularly susceptible to injury. This systematic review aimed to synthesize contemporary evidence on TNP etiologies and highlight diagnostic considerations. Methods: Following PRISMA 2020 guidelines (PROSPERO registration: CRD420251150614), we systematically searched PubMed through July 2025 for studies reporting TNP etiolog

Reducing complications using minimally invasive retrosigmoid approach for microvascular decompression in patients with trigeminal neuralgia.

BACKGROUND: The most effective treatment for a typical trigeminal neuralgia is microvascular decompression of the trigeminal nerve. An approximately 3-5 cm in diameter retrosigmoid craniotomy with an approximately 12-14 cm long skin incision is considered the standard approach. Modern neurosurgery strives for minimally invasive surgical approaches, which in turn reduces both intraoperative and postoperative complications. The use of minimally invasive approaches in skull base neurosurgery and, in particular, in microvascular decompression of the trigeminal nerve can reduce the risk of complica

[Microsurgical Procedures of Microvascular Decompression for Trigeminal Neuralgia].

Microvascular decompression for trigeminal neuralgia was successfully performed following the secure surgical steps. The most crucial step involves creating a safe operative field in the dural opening. The petrotentorial junction should be identified without cerebellar retraction before proceeding to the deeper areas. Dissecting the petrosal vein and opening the horizontal fissure contributed to the expansion of the operative field. Bleeding often occurs from the dorsal cerebellar bridging vein and junction of the petrosal vein into the superior petrosal sinus. Transposition of the most common

Selective plication and grading of plication for correction of torsion in superior oblique palsy.

Acquired palsy of the trochlear nerve has been associated with closed-head injuries. We report the case of a 33-year-old male patient, who presented with complaints of torsional diplopia in the straight gaze which increased in the down gaze following head trauma. After a detailed ophthalmic evaluation, the patient was diagnosed with bilateral superior oblique (SO) palsy causing torsional diplopia. To counteract excyclotorsion, we have plicated the anterior one-third of the fibers of the SO in both eyes. Postoperatively, the patient was diplopia free and orthophoric, which maintained up to 3-mo

Surgical Anatomy of the Retrosigmoid Approach With Transtentorial Extension: Protecting the 4th Cranial Nerve.

BACKGROUND AND OBJECTIVES: The retrosigmoid approach with transtentorial extension (RTA) allows us to address posterior cranial fossa pathologies that extend through the tentorium into the supratentorial space. Incision of the tentorium cerebelli is challenging, especially for the risk of injury of the cranial nerve (CN) IV. We describe a tentorial incision technique and relevant anatomic landmarks. METHODS: The RTA was performed stepwise on 5 formalin-fixed (10 sides), latex-injected cadaver heads. The porus trigeminus's midpoint, the lateral border of the suprameatal tubercle (SMT)'s base,

A Modified Anterior Petrosectomy Approach for Resection of Petroclival Meningioma; with Management of Complications.

Due to deep location and for being adjacent to neurovascular structures, petroclival meningiomas (PCMs) are generally considered to be associated with a high rate of recurrence and cranial nerve deficits.1 This video presents a 49-year-old female patient reporting right trigeminal neuralgia for more than 1 year. The incidence of this symptom with PCMs is about 5%.2 According to the classification system proposed by Kawase et al.3 and Ichimura et al.,4 this is a tentorium type PCM. A modified anterior petrosectomy approach was adopted based on the tumor size and its origin. The case presentatio

Pattern of Ocular Injuries Following Road Traffic Accidents Seen at a Tertiary Eye Hospital.

INTRODUCTION: Traffic trauma can lead to ocular damage. Open globe injuries usually have a poor prognosis, which can be ameliorated by prompt diagnosis and appropriate treatment. Nonetheless, few studies have focused on the visual outcomes of patients following traffic accidents. In this study, we aimed to examine the characteristics and prognosis of ocular complications in patients following traffic accidents at a specialized tertiary eye hospital. METHODS: We classified 44 patients from traffic accidents (88 eyes) into groups with equal or better (better group) and worse (worse group) corre

Bilateral Congenital Trochlear-Oculomotor Synkinesis in a 5-Month-Old Infant.

Oculomotor synkinesis is an unusual phenomenon that encompasses various presentations with few associations reported in the literature. It can occur with or without a previous setting of oculomotor nerve injury. The authors present the first reported case of bilateral congenital trochlear-oculomotor synkinesis in a 5-month-old infant. [J Pediatr Ophthalmol Strabismus. 2024;61(5):e50-e53.].

Application of Topographical Anatomy of the Trochlear Nerve in Transtentorial Approaches: An Anatomic Study.

BACKGROUND AND OBJECTIVES: Transection of the free edge of tentorium (FET) might be necessary when approaching cavernous sinus lesions, clip placement in certain posterior communicating artery aneurysms, or during transpetrosal and retrosigmoid transtentorial approaches. No anatomic study has investigated the relationship of the trochlear and oculomotor nerve and FET in an attempt to identify a safe zone for such maneuver. METHODS: Ten embalmed specimens (20 sides) were studied and the following morphometric measurements were taken using digital microcalipers: trochlear (TP) to oculomotor por

Cranial nerve palsies following neuraxial blocks.

Spinal anesthesia is one of the most frequently performed regional anesthesia techniques for a variety of surgeries world-wide. Cranial nerve palsy is a rarely reported complication of central neuraxial block. The etiology varies; however, it is most often associated with nerve compression or traction due to intracranial hypotension. In October 2023, we searched PubMed and Google Scholar databases for English-language articles published between 1952 and 2023. The following search terms were used in the search strategy: olfactory, optic, oculomotor, trochlear, trigeminal, abducens, facial, vest

Minimally invasive extraocular cranial nerve electromyography.

OBJECTIVE: A reluctance to monitor extraocular cranial nerve (EOCN) function has restricted skull base surgery worldwide. Spontaneous and triggered electromyography (EMG) monitoring can be recorded intraoperatively to identify and assess potential cranial nerve injury. Determining the conductive function of EOCNs requires the collection of clear, reliable, and repeatable compound muscle action potentials (CMAPs) secondary to stimulation. EOCN EMG needle electrodes can, although infrequently, cause ocular morbidity including hematoma, edema, and scleral laceration. The aim of this study was to
