Albendazole-ivermectin co-formulation for the treatment of Trichuris trichiura and other soil-transmitted helminths: a randomised phase 2/3 trial.

BACKGROUND: Treatments for soil-transmitted helminthiases face challenges, especially in addressing Trichuris trichiura. Combination regimens, particularly of ivermectin and albendazole, are promising. We aimed to assess the safety, efficacy, and palatability of a combination tablet for the treatment of T trichiura, hookworm, and Strongyloides stercoralis infections among school-aged children in Ethiopia, Kenya, and Mozambique. METHODS: We conducted an adaptive phase 2/3, randomised, parallel-group, active-controlled, superiority trial in 15 schools in Ethiopia, Kenya, and Mozambique. Eligibl

Vaccines in a time of global boiling and megacities.

Neglected tropical diseases are accelerating because of climate change and urbanization to create new clusters of vast urban areas beset by poverty and environmental degradation. These hot and contaminated megacities could enable the rise of parasitic and other tropical infections. A new generation of antiparasitic vaccines will be needed.

Factors contributing to helminth prevalence after repeated mass administration of medicines in Anambra State, Nigeria.

Over the past decade, Mass Administration of Medicines (MAM) has been a key strategy for controlling schistosomiasis and soil-transmitted helminthiasis (STHs) in Anambra State, Nigeria. This longitudinal study, conducted from 2017 to 2019, evaluated the impact of interventions for controlling schistosomiasis (SCH) and STHs in recipient communities. A total of 1,046 pupils aged 5 to 16 years were enrolled, with Kato-Katz and urine filtration methods used for faecal and urine sample analysis. A structured questionnaire was administered to 243 people to assess the contextual factors. At baseline,

Diagnostic sensitivity of formalin-fixed faecal microscopy for the detection of soil-transmitted helminths.

BACKGROUND: Faecal microscopy is the mainstay of soil-transmitted helminth diagnosis and commonly completed on formalin-fixed samples when resources are insufficient to analyse fresh samples. This study assessed the diagnostic sensitivity of microscopic techniques using formalin-fixed samples. METHODS: Formalin-fixed faecal samples from 574 individuals were tested by the formalin-ethyl acetate concentration technique (FECT), Malachite smear, McMaster and McMaster2 methods. Agreement between tests was assessed by Kappa. Bayesian latent class models and a composite reference standard estimated

The impact of multiple infections and community knowledge on engagement with a historical deworming programme: hookworm and Ascaris in Jamaica, 1913-1936.

INTRODUCTION: Community engagement with public health efforts often depends on existing knowledge of a health issue. METHODS: Here, qualitative analysis of archival material from the Jamaica Hookworm Commission (1919-1936) and quantitative analysis of prevalence data are used to assess knowledge of and ecological interactions between different helminths during a historical hookworm eradication campaign. RESULTS: Archival sources demonstrate that Jamaicans were familiar with Ascaris lumbricoides. Surveys revealed a high prevalence of hookworm (62% of individuals infected), Ascaris (30%) and T

Dopamine signaling drives skin invasion by human-infective nematodes.

Skin-penetrating nematodes are one of the most prevalent causes of disease worldwide - nearly 15% of the global population is infected with at least one species of skin-penetrating nematode1,2. The World Health Organization has targeted these parasites for elimination by 20303, but the lack of preventative measures is a major obstacle to this goal. The infective larvae of skin-penetrating nematodes enter hosts through skin4, and blocking skin penetration is an as-yet unexplored approach for preventing infection. However, in order to prevent worm ingress via the skin, an understanding of the be

Optimal control strategies supported by system dynamics modelling: a study on hookworm disease in China.

BACKGROUND: Hookworm disease remains a global health issue. In China, it persists with a 0.67% infection rate and uneven distribution in 2021. Optimized control strategies are needed. This study aims to optimize intervention strategies for hookworm disease in China. METHODS: Structural analysis and parameter estimation were conducted using system dynamics theory. Key variables were identified via the Delphi method, leading to the creation of a causal loop diagram (CLD) and stock flow chart (SFC). Based on the SFC, parameter estimation and quantitative relationships were established and the mo

Genetic characterization of zoonotic hookworms infecting wild felids in northern India.

BACKGROUND: Hookworms are the most common soil-transmitted helminths that inhabit the small intestine of various domesticated and wild animals. Despite their conservation status, there is a paucity of research on hookworm infections in wild felids. This study aimed to investigate the prevalence of hookworm infections in wild felids in northern India and to genetically characterize the hookworms. Faecal samples (n = 96) from wild felids (lion, tiger, leopard, panther, jungle cat, and civet cat) were examined for helminthic infections. Samples positive for hookworms were subsequently subjected t

Recombinant antigen-based lateral flow tests for the detection of Strongyloides stercoralis infection.

BACKGROUND: Strongyloidiasis is a serious public health issue affecting millions of people worldwide particularly in tropical and subtropical regions. The laboratory diagnosis of strongyloidiasis is often serologically based, typically by enzyme linked immunosorbent assays (ELISA). However, the use of these assays at the point of care requires significantly different approaches for serologic measurements. We sought to determine the diagnostic performance of 2 prototype lateral flow tests alongside the Strongy Detect ELISAs (IgG and IgG4) that uses a cocktail of 2 Ss-specific recombinant antige

Gastrointestinal parasites of zoonotic importance in Cercopithecus mona of Tafi Atome monkey sanctuary in the Afadjato South District of Ghana.

Tafi Atome monkey sanctuary, a conserved grove for many troops of mona monkeys is a major tourist site in Ghana. The human communities near the sanctuary create opportunities for pathogen exchange given the encounters between the wildlife and human population on daily basis. This study assessed the gastro-intestinal parasitic infection in the monkeys and the human populations and the potential zoonotic importance. Stool samples of school-aged children and faecal samples of mona monkeys were collected, processed and examined microscopically for parasite infection. Two different sets of structur

Zoonotic gastrointestinal parasites of shelter dogs in Israel.

Dogs are potentially infected with a diverse spectrum of endoparasites, some which may affect their health or be of zoonotic concern. In Israel, no information exists on the prevalence and spatial distribution of endoparasitic infection among dogs. An investigation of the prevalence of endoparasite infection in Israel shelter dogs was performed by fecal flotation and molecular methods, with particular interest in parasites of zoonotic concern. Polymerase-chain reaction (PCR) was performed targeting the small subunit ribosomal RNA (rrnS) of Taenia spp.,-cox1 mitochondrial DNA of Strongyloides s

Cost-Effectiveness of Treatment for Canine Parasites in Remote Indigenous Communities.

Zoonotic canine parasites and the vector-borne diseases they may carry can cause high morbidity and mortality in dogs and people. Many remote Indigenous communities in northern Australia have numerous free-roaming dogs, tropical climates favouring parasite development, and limited access to veterinary care, which can promote high prevalence of zoonotic parasites. To successfully combat parasites, treatment programs are needed, which are effective in reducing parasite burden and prevalence as well as being cost-effective and feasible. We compared canine parasite treatments in a Torres Strait Is

Strongyloides stercoralis, but Not Hookworm, First-Stage Larvae Require Stool-Soluble Components for Growth and Development.

In regions with prolonged rainy seasons where hookworm and Strongyloides stercoralis (S. stercoralis) are co-endemic, the prevalence of hookworm infection frequently exceeds that of strongyloidiasis, despite both parasites sharing a similar route of infection via skin penetration by third-stage larvae (L3). Climate factors may contribute because rainwater can dilute stool, reducing bacterial abundance and stool-soluble component concentrations that may affect first-stage larvae (L1) development. In this study, we aimed to compare the stool-soluble component requirements of S. stercoralis and h

The Impact of Rhinovirus, Syncytial Respiratory Virus and Helminth Infection on the Risk of New-Onset Asthma and Other Allergic Conditions-A Systematic Review for the EAACI Guidelines on Environmental Science for Allergic Diseases and Asthma.

This systematic review evaluated the association between lower respiratory tract infections (LRTI) in infancy with respiratory syncytial virus (RSV), rhinovirus (RV) or infestation with helminths and the risk of developing asthma and allergic diseases. The risk of bias was assessed with ROBINS-E, and the certainty of evidence (CoE) with GRADE. Meta-analysis applied a random-effects model. RSV LRTI is likely associated with an increased risk of developing asthma by age 7 (OR 3.02, 95% CI 2.23-4.09; I2 = 98%; moderate CoE). The impact on wheezing, atopic dermatitis (AD), and allergic rhinitis is

Deep learning-based automated detection and multiclass classification of soil-transmitted helminths and Schistosoma mansoni eggs in fecal smear images.

In this work, we developed an automated system for the detection and classification of soil-transmitted helminths (STH) and Schistosoma (S.) mansoni eggs in microscopic images of fecal smears. We assembled an STH and S. mansoni dataset comprising over 3,000 field-of-view (FOV) images containing parasite eggs, extracted from more than 300 fecal smear prepared using the Kato-Katz technique. These images were acquired using Schistoscope-a cost-effective automated digital microscope. After annotating the STH and S. mansoni eggs, we employed a transfer learning approach to train an EfficientDet dee

Evidence of canine intestinal parasites and associated fecal bacteria among urban and rural dog parks in central Appalachia U.S.

Public dog parks provide opportunities for exercise and socialization, benefiting both dogs and their owners. However, dog parks can serve as reservoirs for gastrointestinal (GI) parasitic helminths, posing risks to canine and public health. This study investigated the presence of three common GI parasites, hookworm (Ancylostoma spp.), roundworm (Toxocara canis), and whipworm (Trichuris vulpis), in abandoned fecal samples from nine dog parks in Central Appalachia, comparing rural and urban environments. We also examined the relationship between parasite presence and fecal bacterial community c

Serpiginous Travels: A Case of Cutaneous Larva Migrans.

Cutaneous larva migrans (CLM) is a term used to describe the migrating linear skin lesions caused by the burrowing of parasitic nematode larvae within the epidermis. It may cause significant discomfort and distress to patients despite being a relatively self-limiting condition. We present a case of a 34-year-old female with no significant past medical history who arrived at our institution with a pruritic rash on the left foot. Previous treatment with antivirals and azoles was ineffective. Interestingly, her travel history revealed a recent trip to Barbados which broadened the differential for

Widespread Soil-Transmitted Parasitic Contamination in Raw Vegetables at Fresh Markets in the Inner Zone of Bangkok: Findings from a Cross-Sectional Study.

Bangkok, a globally popular travel destination, hosts numerous fresh markets and street food vendors catering to locals and visitors. Although raw vegetables are integral to Thai cuisine, their consumption may pose risks of soil-transmitted parasitic infections. This cross-sectional study assessed the prevalence of parasitic contamination in raw vegetables from Bangkok's inner zones, focusing on five districts: Huai Khwang, Khlong Toei, Pathum Wan, Chatuchak, and Phra Nakhon. A total of 200 samples across eight vegetable types were randomly collected from these areas and prepared for analysis
