Surgical Implications of Anatomic Relationships Among Foramen Ovale, Foramen Spinosum, and VI Cranial Nerve in the Anterior Transpetrosal Approach.

BACKGROUND AND OBJECTIVE: The anterior transpetrosal approach (ATA) is a valuable option for accessing the petroclival region, although it can be associated with considerable morbidity. The objective of the study was to provide a detailed description of the anatomic relationship between key neurovascular structures encountered in the ATA, to facilitate crucial steps of the approach. METHODS: ATA has been performed on 34 sides in cadaveric heads. The extent of drilling was divided into standard ATA and extended ATA with removal of the petrous apex below the mandibular nerve (V3) and Gasserian

Cranial Nerve Injury in Endoscopic Endonasal Approach to Skull Base Surgery: A Systematic Review.

The endoscopic endonasal approach (EEA) to the skull base has revolutionized the management of ventral skull base lesions, offering superior visualization and reduced morbidity. Yet, iatrogenic cranial nerve (CN) injuries, causing temporary or permanent deficits, remain an underexplored complication that significantly affects quality of life. This systematic review describes the number of reported cases of CN injuries in EEA and the evidence on prevention and management strategies. Following PRISMA-SR guidelines, we searched Medline, Embase, Web of Science, and the Cochrane Library, and screen

A Duplicated Anterior Inferior Cerebellar Artery Traversing a Bifurcated Abducens Nerve.

The neurovascular relationship between the anterior inferior cerebellar artery (AICA) and the abducens nerve (CN VI) exhibits significant morphological variability, with important implications for skull base surgery and neurology. During routine cadaveric dissection of a 68-year-old male donor, an uncommon anatomical configuration was identified. The left side demonstrated a duplicated AICA originating from the basilar artery. While the first branch (AICA-1) coursed ventrally to CN VI, the second branch (AICA-2) passed directly through a bifurcation of the nerve. The contralateral side showed

Endoscopic Decompression of Supraorbital Fissure Syndrome After Zygomaticomaxillary Fracture.

The Superior Orbital Fissure Syndrome (SOFS) is a rare complication after craniofacial trauma. According to Hirschfeld, who first described SOFS in 1858, and Lakke, who further defined the complete SOFS, this syndrome results from damage to structures passing through the superior orbital fissure. The structures passing via the superior orbital fissure include the lacrimal nerve, nasociliary nerve, oculomotor nerve, abducens nerve, superior orbital nerve, trochlear nerve, and superior ophthalmic vein. The symptoms of supraorbital fissure syndrome are ophthalmoplegia, ptosis, extraocular muscle

A Case of Drowning with Acute Kidney Injury and Raised Intracranial Tension.

BACKGROUND: Drowning is the third leading cause of unintentional injury and death worldwide, accounting for 7% of all injury-related deaths. People who are at higher risk include those who have free accessibility to water and younger children lacking supervision. The common complications of drowning are mainly due to hypoxia. It mainly affects the respiratory, cardiac, and central nervous system. Rarely, drowning may cause acute kidney injury (AKI) secondary to rhabdomyolysis. Here, we present a case of near drowning with AKI, rhabdomyolysis, and elevated intracranial tension (ICT) with a fals

Syndrome of Inappropriate Antidiuresis and Symptomatic Hyponatremia After Onyx Embolization of a Carotid-Cavernous Fistula: A Case Report.

BACKGROUND Carotid-cavernous fistulas (CCFs) are abnormal arteriovenous communications between the carotid artery and cavernous sinus. They may be direct or indirect and commonly present with both ocular and neurovascular symptoms. Endovascular embolization is the main treatment, with high rates of fistula obliteration and clinical improvement. Although generally safe, complications can occur, including cranial nerve dysfunction and vascular injury. Endocrine complications following carotid-cavernous fistula embolization are exceedingly rare. To our knowledge, only 1 case of syndrome of inappr

Isolated traumatic abducens nerve avulsion in the cisternal segment: mechanism, diagnosis and neurosurgical treatment.

The abducens nerve’s long intracranial course makes it highly vulnerable to injury during traumatic brain injury. While traditionally attributed to skull base fractures, high-resolution MRI has identified avulsion in cistern as a significant, yet underrecognized, mechanism, which can occur even without adjacent fractures. The study retrospectively analyzed 31 patients with traumatic abducens nerve palsy and radiologically suspected or surgically confirmed cisternal segment avulsion. Injuries were classified via MRI as complete or partial/suspected avulsion. Patients received different treatmen

The Sinus Snare: A Case Series of Isolated Sphenoid Sinusitis Presenting as Orbital Apex Syndrome in Poorly Controlled Diabetes.

Isolated sphenoid sinusitis is an uncommon entity, accounting for a small proportion of paranasal sinus disease. Its deep anatomical location and proximity to the optic nerve, cavernous sinus, and orbital apex often result in delayed diagnosis, particularly when typical nasal symptoms are absent. In poorly controlled diabetics, fungal infection of the sphenoid sinus can progress rapidly, presenting initially with ophthalmic deficits rather than sinonasal complaints. This series describes three female patients between 50 and 53 years of age, all with long-standing type 2 diabetes mellitus with

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Resection of Meningiomas Invading the Cavernous Sinus: Treatment Strategy and Clinical Outcomes.

Background/Objectives: Resection of tumors invading the cavernous sinus (CS) carries a risk of injury to the cranial nerves and internal carotid artery. Therefore, radical surgery involving lesions around the CS remains challenging, especially for lesions invading the CS, optic sheath, and oculomotor cave. Here, we describe a surgical strategy for meningiomas invading these structures and report on the clinical outcomes. Methods: Surgical resection was indicated in patients with neurological symptoms or rapid tumor growth for the restoration of cranial nerve function. We investigated 13 patien

Diplopia Rhinoceros.

Bilateral cranial nerve VI palsy is a rare neuro-ophthalmological presentation often associated with systemic or intracranial pathologies. In diabetic patients, cranial neuropathies can occur due to immune suppression, ischemia, or infectious complications. This case highlights the interplay of poorly controlled type 2 diabetes mellitus (T2DM), sinusitis, and skull base inflammation leading to bilateral lateral rectus palsy. A 56-year-old postmenopausal woman presented with a 10-day history of headache, an 8-day history of right-sided facial pain, and progressive binocular diplopia over 5 days

Traumatic Abducens Nerve Palsy Due to Avulsion in its Cisternal Segment: Illustrative Cases.

BACKGROUND Traumatic abducens nerve palsy can be caused by fractures around the Dorello's canal, direct injury at the dura entry point or within the Dorello's canal, or ischemia due to posterior meningeal artery damage. Recent imaging studies have also identified avulsion of the abducens nerve at its cisternal segment as a significant cause. CASE REPORT We present 2 cases of traumatic abducens nerve palsy due to avulsion at the cisternal segment. Preoperative thin-slice T2 magnetic resonance imaging indicated unclear visualization of the nerve at the cisternal segment in both cases. Surgical e

Isolated abducens nerve involvement as a revealing sign of temporal bone fracture: A case report.

Isolated abducens nerve palsy is an uncommon yet clinically significant manifestation of temporal bone fracture, often following blunt head trauma. We report the case of a 52-year-old woman who sustained a right-sided head injury after a domestic fall under the influence of alcohol. She presented with diplopia, vertigo, and right-sided conductive hearing loss. Clinical examination revealed impaired abduction of the right eye, anisocoria with mydriasis, right retroauricular ecchymosis, and facial hypoesthesia in the maxillary (V2) and mandibular (V3) territories. Otoscopy showed hemotympanum, a

When the tumor encases or displaces the abducens nerve: anatomically based strategies to prevent its injury in the retrosigmoid route.

OBJECTIVE: Lesions of the cerebellopontine angle (CPA) and petroclival region represent a challenging surgical target due to the complex anatomy of the involved neurovascular structures. In this scenario, cranial nerve (CN) VI is particularly exposed to potential injuries due to its deep-seated location and absence of a bony foramen that serves as a reference of its most distal cisternal point, especially when it is encased or displaced by large lesions. This study aimed to provide reliable operative guidance for preventing injuries to CN VI during the retrosigmoid approach to address CPA and
