Anatomical restrictions in the transsphenoidal, transclival approach to the upper clival region: a cadaveric, anatomic study.

OBJECTIVE: Tumours in the clival region are difficult to remove surgically. Before the 1970s, clival tumours had very high mortality and morbidity rates. METHODS: An anatomic dissection was performed on 24 spheno-occipital bone blocks obtained from 28 adult cadavers. The internal carotid artery, paraclival carotid tubercle, sixth cranial nerve and dorsum sellae in the upper clival region were analyzed qualitatively and quantitatively. For the histological evaluation, 4 samples were decalcified and sagittal sections were cut. From the eight blocks obtained, 32 incisions were made in the axial

A novel simple real-time electrooculographic monitoring system during transsphenoidal surgeries to prevent postoperative extraocular motor nerve dysfunction.

In transsphenoidal surgery (TSS) for pituitary tumors, the use of endoscopes allows approach to the lateral sides in and around the cavernous sinus. However, this approach is often associated with a risk of cranial nerve dysfunction causing impaired extraocular movement. We employed a novel, simple, and real-time monitoring system using electrooculography during TSS to avoid postoperative extraocular motor nerve dysfunction. A conventional electroencephalograph, which is available in every hospital, was used to detect effects induced by intraoperative manipulation on the cranial nerves related

Orbitoethmoidal impacted injury by kitchen knife causing abducens nerve palsy.

INTRODUCTION: Impacted knife injuries in the maxillofacial region are rare and infrequently reported. In cases of injury involving orbit or eye, these reports are even rarer. DISCUSSION: Damage to the orbital contents may result in a rupture of the globe, extraocular muscle injury, lacrimal gland damage, and others. Orbital foreign bodies are not only difficult to detect, and clinical features vary according to its size, characteristics, shape, penetrating method, and site. In this report, a case of abducens nerve palsy after orbitoethmoidal knife injury is presented.

Three-dimensional in vivo modeling of vestibular schwannomas and surrounding cranial nerves with diffusion imaging tractography.

BACKGROUND: Preservation of cranial nerves (CNs) is of paramount concern in the treatment of vestibular schwannomas, particularly in large tumors with thinned and distorted CN fibers. However, imaging of the CN fibers surrounding vestibular schwannomas has been limited with 2-dimensional imaging alone. OBJECTIVE: To assess whether tractography of the CN combined with anatomic magnetic resonance imaging of the tumor can provide superior 3-dimensional (3D) visualization of tumor/CN complexes. METHODS: Magnetic resonance imaging at 3 T, including diffusion tensor imaging and anatomic images, we

Penetrating craniofacial trauma from a vice clamp: case report and discussion.

Penetrating craniofacial trauma, although uncommon, has a high potential for death or serious morbidity from injury to vital neurovascular structures. An in situ facial foreign body, particularly if large, presents significant challenges beyond safe and timely removal. Airway management, stabilization of the object, management of increased intracranial pressure, and identification of injuries to local structures are all issues that may require addressing. We present a case of penetrating facial trauma from a vice clamp, with an in situ foreign body, that illustrates several of these challenges

Ophthalmoplegia associated with transorbital penetrating brainstem injury by broken fishing pole.

We report our findings in a case of ophthalmoplegia caused by a transorbital penetrating brainstem injury. An 8-year-old boy was accidentally injured by a broken fishing fiberglass pole which penetrated through the right orbit and entered the brainstem. Magnetic resonance imaging showed a linear wound that entered and passed through the pons obliquely and reached the fourth cerebral ventricle and cerebellar vermis. He had a left-sided hemiplegia and left facial nerve palsy and was diagnosed with "one-and-a-half syndrome". His hemiplegia and left facial nerve palsy resolved in 2 weeks leaving o

Isolated bilateral abducens nerve palsy without radiographic etiology and unique mechanism of injury.

Isolated bilateral abducens palsy is a rare event, especially in the setting of closed head injury. Cases that lack radiographic or pathologic findings to explain the etiology of the palsy are limited to case reports. Injury mechanisms have been postulated, but a consensus does not exist. The authors describe a case of traumatic isolated bilateral abducens palsy lacking radiographic and pathologic findings. A previously unreported potential pathophysiologic injury mechanism is theorized based upon anatomical structure and mode of injury.

Dorello canal revisited: an observation that potentially explains the frequency of abducens nerve injury after head injury.

OBJECTIVE: The abducens nerve is frequently injured after head trauma and some investigators have attributed this to its long intracranial course. The present study aimed to elucidate an additional mechanism to explain this phenomenon. METHODS: Twelve fresh adult cadavers underwent dissection of Dorello canal using standard microsurgical techniques. In addition, traction was applied to the nerve at its entrance into this canal before and after transection of Gruber ligament to observe for movement. RESULTS: In all specimens, a secondary tunnel (i.e., tube within a tube) was found within Dore

Spatial and temporal variation of rod photoreceptor reflectance in the human retina.

Objectives Numerous surgical approaches have been developed to access the petroclival region. The Kawase approach, through the middle fossa, is a well-described option for addressing cranial base lesions of the petroclival region. Our aim was to gather data about the variation of cranial nerve locations in diverse petroclival pathologies and clarify the most common pathologic variations confirmed during the anterior petrosal approach. Method A retrospective analysis was made of both videos and operative and histologic records of 40 petroclival tumors from January 2009 to September 2013 in whic

Abducens nerve palsy associated with a clival epidural hematoma.

A clival epidural hematoma is a rare lesion that usually develops after a hyperflexion or hyperextension injury of the neck, often in a child. A 5-year-old girl presented after a motor vehicle accident with multiple cranial neuropathies, including bilateral abducens nerve pareses and right facial, glossopharyngeal, and hypoglossal cranial nerve palsies. Neuroimaging identified a clival epidural hematoma. The child was observed and the hematoma resolved. The abducens nerve palsies resolved during the ensuing 14 months.

Isolated bilateral abducens nerves palsy due to traumatic delicate arteriovenous fistula.

BACKGROUND: Ocular motor dysfunction is common in patients with head trauma. Also, traumatic lateral gaze palsy is usually associated with brain stem lesion, peripheral nerve injury with or without basilar skull fracture and lateral rectus muscle injury or entrapment. However, isolated bilateral abducens nerve palsy is extremely rare. CASE STUDY: This study describes a case of isolated bilateral abducens nerve palsy in a 35-year-old male with head trauma without cervical and skull fractures. He was diagnosed with mild diffuse axonal injury. During his assessment, he developed mild limb weakne

Severe visual loss after ear surgery.

Sigmoid sinus thrombosis was once one of the most common intracranial complications of otitis media, but this has become less common since the advent of antibiotics. The frequency of sigmoid sinus hypoplasia reported in the literature is about 17%. Considering the possibility of postoperative intracranial hypertension, sigmoid sinus and jugular bulb surgeries were contraindicated in this case.We present a case with sigmoid sinus thrombosis of the sinus due to compression flowing surgical injury. This is a unique case, which presents with contralateral sigmoid sinus hypoplasia. Eventually, the

Clival epidural hematoma in traumatic sixth cranial nerve palsies combined with cervical injuries.

Eight patients sustained a combination of clival epidural hematoma, traumatic sixth cranial nerve palsy (6 NP), and occipitocervical injury. This combination of features has been sparsely described. Whether the hematoma, which represents tectorial membrane injury, is merely a marker for 6 NP and occipitocervical injury or is causative is unresolved, but this imaging finding should alert examiners who note traumatic 6 NP to the need for detailed cervical imaging, as surgical stabilization of this region may be critical to prevent future spinal cord dysfunction.
