Avoiding injury to the abducens nerve during expanded endonasal endoscopic surgery: anatomic and clinical case studies.

BACKGROUND: Understanding the course of the most medially located parasellar cranial nerve, the abducens, becomes critical when performing an expanded endonasal approach. OBJECTIVE: We report an anatomoclinical study of the abducens nerve and describe relevant surgical nuances to avoid its injury. METHODS: Ten anatomic specimens were dissected using endoscopes attached to an high-definition camera. A series of anatomic measurements and relationships of the abducens nerve were noted. Illustrative clinical cases are described to translate those findings into practice. RESULTS: Cisternal, inte

Cranial nerve injury after Le Fort I osteotomy.

A Le Fort I osteotomy is widely used to correct dentofacial deformity because it is a safe and reliable surgical method. Although rare, various complications have been reported in relation to pterygomaxillary separation. Cranial nerve damage is one of the serious complications that can occur after Le Fort I osteotomy. In this report, a 19-year-old man with unilateral cleft lip and palate underwent surgery to correct maxillary hypoplasia, asymmetry and mandibular prognathism. After the Le Fort I maxillary osteotomy, the patient showed multiple cranial nerve damage; an impairment of outward move

Anatomical relationships of intracavernous internal carotid artery to intracavernous neural structures.

The objective is to correlate the intracavernous internal carotid artery (ICA) with the position of the intracavernous neural structures. The cavernous sinuses of nine injected cadaveric heads were dissected bilaterally. As measured on computed tomographic angiograms from 100 adults, anatomical relationships and measurements of intracavernous ICA and neural structures were studied and correlated to the intracavernous ICA curvature. Intracavernous ICAs were classified as normal and redundant. The meningohypophyseal trunk (MHT) of normal ICAs appeared to be closely related to the abducens nerve

Isolated abducens nerve palsy following lumbar puncture: case report and review of the mechanism of action.

Isolated abducens nerve palsy following lumbar puncture is a very rare condition. In this case we discussed the probable causes of abducens nerve palsy and review the mechanism of action in anatomical relevant literature. A 53-year old hypertensive female patient with a saccular aneursym on the left middle cerebral artery (MCA) bifurcation underwent emergency operation. Before the operation lumbar puncture (LP) was performed to the patient lying on the right lateral position to facilitate cerebral relaxation intraoperatively. The left MCA bifurcation aneurysm was clipped successfully with a le

Extradural haematoma presenting as a contralateral sixth nerve palsy after cochlear implantation.

Intracranial complications after cochlear implantation are rare. The authors present the case of a 13 month old boy with a contralateral abducens nerve palsy following cochlear implantation that led to the diagnosis of an extradural haematoma on computerised tomography scanning. The abducens palsy resolved after evacuation of the haematoma and the patient made an excellent recovery. The literature is reviewed and the mechanism of injury discussed.

Development of cranial nerve palsy shortly after endosaccular embolization for asymptomatic cerebral aneurysm: report of two cases and literature review.

CLINICAL DESCRIPTION: We report two cases of asymptomatic cerebral aneurysm in which cranial nerve palsy (CNP) developed shortly after symbolization. The CNP occurred immediately in case 1, but case 2 showed the CNP 30 h after symbolization. Although both aneurysms had increased in size on follow-up angiography, case 2 who showed dome re canalization resulted in progressive CNP deterioration. CONCLUSION: These findings suggest that the CNP may result not only from mechanical compression by coils but also from inflammation induced by perpendicular thrombosis, and that the prognosis of the CNP

Usefulness of intravascular ultrasonography monitoring of coil embolization for traumatic direct carotid-cavernous fistula-case report-.

A 61-year-old woman was admitted for head injury after a traffic accident. Two months later, she developed abducens nerve palsy, chemosis, and pulsatile tinnitus. Right internal carotid angiography demonstrated a high flow direct carotid-cavernous fistula (CCF) at the C(5) portion with reflux into the superficial and deep sylvian veins, superior ophthalmic vein, superior petrosal sinus, and inferior petrosal sinus. Intravascular ultrasonography (IVUS) revealed a large fistula at the C(5) portion of the internal carotid artery (ICA). Coil embolization via transarterial and transvenous approache

Isolated abducens nerve palsy after closed head trauma: a pediatric case report.

Cranial nerve lessions often accompany head trauma. Nevertheless, isolated involvement of the sixth nerve without any cranial or cervical fracture is rare. Nerve injury could occur at the sites of the dural entry points and at the petrous apex during down- or upward movement of the brain caused by violent linear force to the head. Management is symptomatic. Most cases improve within three months and many resolve by six months, but spontaneous recovery does not always occur. We describe the case of a 13-year-old boy who developed isolated abducens nerve palsy after closed head trauma.

Acute sixth nerve palsy in a young man, beware of the 'red herring'.

BACKGROUND: Cranial nerve palsies has several etiologies including vascular insufficiency, neoplasm, trauma and inflammation. Isolated sixth nerve palsy is an extremely rare presenting feature of leukemia. AIM: We describe an unusual ocular presentation of a bilateral progressive sixth nerve palsy in a young male with a preceding head injury. CONCLUSION: Acquired sixth nerve palsies in young adults may be due to trauma but in the absence of a definitive history other systemic processes must be outruled. We describe a case of bilateral sixth nerve palsy in a patient with ALL with no obvious C

Shoulder abduction-induced reductions in reaching work area following hemiparetic stroke: neuroscientific implications.

A stroke-related loss of corticospinal and corticobulbar pathways is postulated to result in an increased use of remaining neural substrates such as bulbospinal pathways as individuals with stroke are required to generate greater volitional shoulder abduction torques. The effect of shoulder abduction on upper extremity reaching range of motion (work area) was measured in 18 individuals with stroke using the Arm Coordination Training 3-D (ACT(3D)) device. This robotic system is capable of quantifying movement kinematics when a subject attempts to reach while simultaneously generating various le

A chronically implantable device for the controlled delivery of substances, and stimulation and recording of activity in severed nerves.

We describe the use of an implantable device for peripheral nerves that allows chronic simultaneous delivery of small volumes of solution, recording of both field and multiunit potentials, and electrical stimulation. This custom-made multifunctional device was attached to the cut end of the abducens (VIth) nerve for stimulation, recording and injection purposes. Our device consists of a polyethylene chamber with two electrodes that can be used for stimulation and recording and two Teflon tubes that serve as inlet and outlet for administering chemicals to the nerve fitted inside. Since the devi
