Facial Nerve Palsy Recovery After Vestibular Schwannoma Surgery: Temporal Patterns and Early Predictors of Functional Outcome.

Background and Objectives: Facial nerve palsy (FNP) remains a clinically relevant complication after vestibular schwannoma (VS) resection. Recovery trajectories vary according to the severity of neural injury and early postoperative facial function. This study aimed to characterize the temporal pattern of facial nerve recovery during the first postoperative year and to explore factors associated with favorable long-term function. Materials and Methods: We retrospectively analyzed 51 adult patients who developed FNP after VS surgery between 2020 and 2023. Facial nerve function was assessed usin

Impact of the brainstem in normal and impaired human movement control.

The corticospinal tract represents a highly developed neural system that dominates motor control in humans. This allows the performance of skilled finger movements, such as playing the piano. In contrast, according to preclinical studies, automatically performed complex movements, such as locomotion or cooperative forelimb movements, appear to be controlled mainly by brainstem motor centres. Only in recent years, with the advancement of new methodologies, has evidence emerged that limb coordination in humans is also based on neural limb coupling mediated by brainstem motor centres. This neural

Pre-operative blood-based inflammatory indices and long-term facial nerve outcomes following vestibular schwannoma resection.

OBJECTIVE: Inflammation contributes to tumor pathogenesis and growth in vestibular schwannoma (VS) patients, but associations with facial nerve outcome following surgical resection remain unclear. We aimed to evaluate whether blood-based inflammatory indices are associated with postoperative facial nerve outcomes following VS resection. PATIENTS AND METHODS: This retrospective cohort study included adult VS patients who consecutively underwent microsurgical resection at our institution between July 2016 and July 2024. Patients with neurofibromatosis, previous radiotherapy or resection, baselin

Hydrogen nanobubble water reduces cochlear inflammation and oxidative stress in noise-induced hearing loss via the gut-inner ear axis.

Environmental noise is a widespread stressor that leads to auditory dysfunction and systemic inflammatory disturbances. Noise-induced hearing loss (NIHL) remains a major environmental health concern, yet effective preventive strategies remain limited. Increasing evidence suggests that environmental stress may disrupt gut microbial balance and promote inflammation that leads to distant organs, including cochlea. Here, we demonstrate that hydrogen nanobubble water (HNW), an improved and stabilized oral formulation of molecular hydrogen, reduces cochlear injury induced by noise exposure. Compared

Cochlear aging after synaptopathic noise: age-noise interactions in hair cell loss and axonal degeneration.

Despite many studies of cochlear synaptic loss and/or recovery after acoustic injury, none has followed degeneration of auditory-nerve fiber (ANF) peripheral axons - a critical knowledge gap in the context of regenerative strategies. We exposed CBA/CaJ mice to a synaptopathic 8-16 kHz noise stimulus producing an immediate 50% loss of inner hair cell synapses, with minimal post-exposure recovery. Noise-exposed and unexposed animals were allowed to survive for 1 month, 6 months, 1 year or 2 years before tissue harvest. Using immunostaining and machine-learning pipelines, we quantified hair cells

Avoidance and Management of Complications in Retrosigmoid Approach to Vestibular Schwannomas.

An experience with two rare complications during surgery of vestibular schwannomas (VSs) is presented, and measures to avoid and manage the complications are discussed.Case A: Spinal cord ischemia in semi-sitting position: A 47-year-old with a giant vestibular schwannoma (VS) underwent surgery through a retrosigmoid approach in the semi-sitting position. The intraoperative phase was uneventful, except for an episode of moderate hypotension. Postoperatively, the patient woke up with quadriparesis. MRI on the cervical spine revealed restricted diffusion from C4 to C7 suggestive of cord ischemia.

Cross-modal cortical circuit for sound sensitivity in neuropathic pain.

Hyperacusis, exaggerated sensitivity to sound, frequently accompanies chronic pain in humans, suggesting interactions between different sensory systems in the brain. However, the neural mechanisms underlying this comorbidity remain largely unexplored. In this study, behavioral tests measuring sound-evoked pupil dilation and reaction times to lick water following auditory stimuli showed hyperacusis-like behaviors in neuropathic pain model mice. Through viral tracing, fiber photometry, and multi-electrode recordings, we identified glutamatergic projections from primary somatosensory cortex (S1HL

Ultrasonic Vocalization Acoustics After Recurrent Laryngeal Nerve Injury and Recovery.

OBJECTIVES: The lack of well-defined functional acoustic correlates of vocal fold paralysis in animal models creates challenges for translating basic science studies of recurrent laryngeal nerve (RLN) injury to patients with vocal fold paralysis. Capitalizing on natural communicative rodent ultrasonic vocalizations (USVs), this study aimed to (1) investigate the impact of RLN injury on USV acoustics in both acute and chronic stages of RLN injury and (2) to identify USV parameters with the greatest ecological validity specific to RLN injury. METHODS: USVs were recorded over 2 weeks (acute) in

[Imaging Use in Vestibular Schwannoma Surgery:Strategies for Visualizing Anatomy and Function].

Functional preservation is increasingly emphasized in vestibular schwannoma surgery. Achieving this requires preoperative imaging, intraoperative cranial nerve monitoring, and adequate dissection of the layers. High-resolution magnetic resonance imaging, including diffusion tensor imaging and contrast-enhanced fast imaging employing steady-state acquisition, allows preoperative visualization of the facial and cochlear nerve courses. Additionally, three-dimensional (3D) reconstructed images aid in simulating nerve trajectories relative to tumors within the surgical field. The facial and cochlea

Protective effect of an oriented PCL electrospun membrane loaded with red ginseng polysaccharides and magnetic nanoparticles against nerve injury of mice.

Clinical neurological diseases often result in irreversible axonal damage and neuronal death, posing significant challenges for nerve regeneration. Red ginseng polysaccharide (RGP) possesses antioxidative, anti-inflammatory, and immunomodulatory properties that support neuronal repair. The synergistic effects of magnetic field (MF) and magnetic materials show great potential in enhancing nerve growth. Herein, an oriented polycaprolactone (PCL) electrospun membrane (PCL-RGP-Fe3O4) carrying with RGP and Fe3O4 magnetic nanoparticles (MNPs) was synthesized through electrospinning technology. This

Acoustic Motor Cortex Stimulation Enhances the Descending Analgesic Pathway to Alleviate Chronic Pain in Mice.

Chronic pain poses considerable health risks, necessitating the development of effective treatments. Physical modulation of the motor cortex has demonstrated promise for pain relief; however, existing methods require invasive electrode implantation or have limited spatial resolution. Therefore, we developed a non-invasive, high-precision acoustic motor cortex stimulation (aMCS) system to alleviate chronic pain and explore its mechanisms. We developed a wearable aMCS system and employed the spared nerve injury (SNI) method to establish a mouse model of chronic pain. The model mice underwent aMC

[Hemostasis].

Hemostasis is a critical skill in cerebellopontine angle (CPA) tumor surgery given its deep anatomical location, narrow surgical corridor, and proximity to vital neurovascular structures. Inadequate bleeding control can obscure the operative field, increase the risk of cranial nerve injury, and lead to life-threatening complications, such as brainstem infarction or cerebellar swelling. This article outlines the key principles of hemostasis at each step of CPA tumor resection, from the preoperative setting and craniotomy to tumor debulking and dissection. Based on our surgical experience, we pr

Bioconvergence of sound-guided and supramolecular assembly strategies to create peptide-protein composite hydrogels with predictable shape-to-function features.

Purely protein-based hydrogels are widely used in tissue engineering for their biomimicry and biocompatibility, yet remain challenging to tailor with precision and predictability at biological and mechanical levels. To overcome this, synthetic self-assembling peptide amphiphiles (PAs) offer opportunities for supramolecular customization, both as single-phase materials and co-assembled with proteins to create hybrid nanocomposites with emerging functionalities. Similarly, contactless, sound-guided bioassembly techniques using liquid-phase hydrogel precursors are emerging as strategic tools for

Facial Nerve Conduction Block during Vestibular Schwannoma Surgery: A Rare and Unclarified Transient Neural Dysfunction.

Preservation of facial nerve function while achieving sufficient tumor removal is a key objective in vestibular schwannoma surgery, and continuous facial nerve monitoring provides real-time, quantitative, and functional assessment of facial nerve during resection. In our experiences of more than 1,900 vestibular schwannoma surgeries, we have occasionally observed an abrupt and marked decrease in monitoring amplitude without any high-risk manipulation such as direct nerve dissection. This rare phenomenon, illustrated by 2 recent cases in this study, was transient or unrelated to postoperative f

External Auditory Canal Transillumination-Guided Middle Fossa Approaches: An Anatomical Feasibility Study.

BACKGROUND AND OBJECTIVES: The middle fossa approaches are tremendously versatile for treating small vestibular schwannomas, selected petroclival meningiomas, midbasilar trunk aneurysms, and lesions of the petrous bone. Our aim was to localize the internal acoustic canal and safely drill the petrous apex with these approaches. This study demonstrates a new method to locate the internal acoustic canal during surgery in the middle fossa. METHODS: The microsurgical anatomy of the middle fossa floor was studied in 11 formalin-fixed and silicone-injected cadaveric heads. Extradural dissection of t

Chlorogenic Acid Improves Salicylic Acid-Induced Cochlear Injury by Reducing Oxidative Stress and Inflammatory Response Through TLR4/NF-κB Signalling Pathway.

BACKGROUND: Sodium salicylate (SS) causes hearing damage and tinnitus in humans and animals. Chlorogenic acid (CGA) has strong antioxidant and anti-apoptotic effects, but whether it can protect the cochlea is unknown. METHODS: SS was used to induce cochlear injury in rats, followed by 150, 300 and 600 mg/kg CGA treatment. The changes of tinnitus behaviour in each group were detected by auditory brainstem response (ABR), behavioural test, beam balance, Preyer reflex and auditory startle test. The levels of oxidative stress and inflammation were detected by kit. The pathological damage of cochl

Low-Intensity Ultrasound Facilitation of Intranasal Drug Delivery to Olfactory Bulb and Trigeminal Nerves.

OBJECTIVE: Nasal-to-brain (NtoB) delivery is a noninvasive approach that uses the nasal cavity as a pathway to transport therapeutic agents directly to the brain. This approach bypasses systemic circulation and avoids the blood-brain barrier (BBB). Transcranial ultrasound, coupled with microbubbles (MB), is a technique used to oscillate and generate acoustic cavitation to open the capillary tight junctions of BBB temporarily. Its efficacy in facilitating NtoB delivery has been demonstrated in vivo. However, while opening the BBB, sonication with MB poses the risk of cerebral microhemorrhage or
