A combined otoneurosurgical team has operated upon 68 patients with acoustic nerve neurinomas over a period of 13 months (Sept. 1982-Oct. 1983). Exploration and operation in this homogeneous series were carried out under the same conditions, and a CT scan was performed in each patient before and after surgery. The size of the neuroma was less than 2 cm in diameter in 18 cases, between 2 and 3 cm in 28 cases, between 3 and 4 cm in 10 and 4 cm or more in 12. Recent evidence suggests that an excitant amino acid may be a neurotransmitter at acoustic nerve synapses in cochlear nucleus (CN). Release of excitant amino acids is reportedly reduced by baclofen, a lipophilic GABA-mimetic used to treat the spasticity of multiple sclerosis and spinal injury. Microiontophoresis of (-)baclofen suppressed spontaneous and tone-evoked activity in CN neurons. GABA inhibited the responses of most neurons responsive to (-)baclofen. However, iontophoresis of these two substances onto the same CN neuron resulted in dramatic differences in time course to maximum effect

An understanding of vestibular function and a careful history will quickly separate patients with disease of the vestibular system from others. Further history and physical examination, often coupled with audiometric and vestibular function tests, will aid in localizing the lesion to the central or peripheral portions of the system and in determining the affected side. Medical management, which is often nonspecific and involves considerable psychological support, usually suffices.

Liquor cerebrospinalis, obtained from patients with various diseases, inhibited Na, K-ATPase from rat and bovine synaptic membranes and did not influence on the activity of Mg-ATPase. The inhibition was independent on the type of disease. The inhibitory effect of liquor cerebrospinalis disappeared after dyalisis of it against Krebs-Ringer solution. Liquor cerebrospinalis appears to contain an endogenous inhibitor of synaptic Na,K-ATPase, which controls the enzymatic activity in vivo.

Stapedectomy has become recognized as the procedure of choice in the surgical treatment of clinical otosclerosis. Based on results obtained in stapes surgery performed in 1977, profiles were established for hearing improvement, vertigo, tinnitus, chorda tympani injury and temporary threshold shift of high tones.
