PURPOSE: Brachial plexus birth injuries (BPBI) can be life-changing for both the patient and family. While most resolve spontaneously, surgical intervention is indicated for those who fail to improve. This can be performed via nerve grafting or nerve transfer. The spinal accessory nerve is a commonly used extraplexal donor in brachial plexus surgery. In cases of BPBI, it can be transferred to the musculocutaneous nerve to restore elbow flexion. While there are short-term reports in BPBI, few, if any, long-term outcome data are available.
METHODS: A retrospective review of patients who underwe

BACKGROUND: Nerve transfers are increasingly used to restore upper limb function in infants and toddlers with brachial plexus birth injuries (BPBI). However, standardized, pediatric-specific rehabilitation protocols remain lacking. Adult-focused approaches are often developmentally inappropriate, and the absence of tailored rehabilitation may contribute to high failure rates, particularly for complex transfers.
PURPOSE: To present a structured, milestone-based rehabilitation protocol adapted from the Donor Activation Focused Rehabilitation Approach (DAFRA) for infants and toddlers undergoing

BACKGROUND: The spinal accessory nerve (SAN) is particularly vulnerable to iatrogenic injury during head and neck procedures, especially in the posterior cervical triangle where it courses superficially. Despite its clinical relevance, reliable surface-based strategies for SAN localization remain underdeveloped, particularly in the context of aesthetic and limited-access surgeries.
METHODS: A cadaveric study was conducted on 186 hemi-necks from 111 Thiel-embalmed cadavers to identify consistent anatomical relationships between the SAN and four readily palpable skeletal landmarks: the mastoid

OBJECTIVE: To systematically evaluate the efficacy and safety of video-assisted lateral neck dissection (VALND) in the treatment of lateral lymph node metastasis (LLNM) of papillary thyroid carcinoma (PTC).
METHODS: A retrospective cohort study was performed on PTC patients with LLNM who underwent VALND in our center from June 2013 to December 2017. The safety of VALND was evaluated by the rate of postoperative complications, while its efficacy was evaluated by the number of lateral necks dissected (LND) and the rate of recurrence.
RESULTS: A total of 191 patients with pathologically confirm

The extra-cranial spinal accessory nerve (SAN) is an important motor nerve, responsible for innervating the trapezius and sternocleidomastoid muscle (SCM). At a particular point in the neck SAN is vulnerable to iatrogenic injuries, particularly during lymph neck node dissection. We present the radiological and clinical features of a 29 year old female sickle cell patient who was referred for an ultrasound assessment of a suspected injured SAN following an excisional lymph node biopsy. A sural graft repair was subsequently preformed of both SAN and the greater auricular nerve (GAN). This case r

INTRODUCTION: The spinal accessory nerve is the external terminal division of the 11th cranial nerve which passes postero-inferiorly in the neck, innervating the sternocleidomastoid and trapezius muscles. The spinal accessory nerve is often in the field of view of neck ultrasound imaging, however, can be overlooked and underappreciated during sonographic neck imaging and ultrasound-guided lymph node biopsies.
METHOD: A scoping review of the literature was conducted to assess current knowledge regarding sonographic imaging of the spinal accessory nerve. The authors' sonographic experience and

PURPOSE: Outcomes after brachial plexus birth injury (BPBI) vary widely, highlighting the need for rehabilitation strategies that reliably enhance functional recovery. We hypothesized that a coordinated, interdisciplinary rehabilitation network involving surgeons and occupational therapists improves functional outcomes after spinal accessory to suprascapular nerve (SAN-SSN) transfer. We compared outcomes from two academic centers with similar surgical indications and techniques but distinct rehabilitation models.
METHODS: We reviewed 25 infants who underwent SAN-SSN transfer from 2022 to 2024

BACKGROUND: In the treatment of obstetric brachial plexus palsy (OBPP), neurotization, or nerve transfer, has become a crucial microsurgical procedure, especially when nerve roots are avulsed or non-functional. To reinnervate a denervated target and restore motor function, a functional donor neuron must be redirected. The objective of the study is to assess the outcomes of various neurotization approaches in individuals with brachial plexus injuries sustained during pregnancy.
METHODS: Thirty patients with OBPP who received microsurgical treatment using various brachial plexus neurotization p

Upper trunk (C5-C6) injuries of the brachial plexus impair shoulder abduction and elbow flexion. For C5-C6 ruptures with preserved proximal stumps, nerve grafting (NG) remains the conventional approach. Nerve transfers (NT) represent the sole viable option in C5-C6 avulsions. The current global standard for NT involves triple transfer procedures: spinal accessory nerve (SAN) to suprascapular nerve(SSN), Oberlin procedure, and Leechavengvongs procedure. Recent clinical trends indicate a broadening of NT indications to include root rupture scenarios, driven by the rationale that distal NT signif

BACKGROUND: Balloon-based ablation (BBA) is a useful treatment strategy for atrial fibrillation (AF) despite the current trend of decreasing BBA volume due to pulsed-field ablation. However, during BBA for AF of the right-sided (RS) pulmonary veins (PVs) (RS-PVs), constant phrenic nerve (PN) pacing from the superior vena cava (SVC) is required to prevent right PN injury. The PN courses anterior to the scalenus anterior muscle and posterior to the sternocleidomastoid muscle in the neck via a branch of the C3-5 roots. This study determined the efficacy and safety of right PN pacing from the neck

Femoral nerve block is an essential regional anesthetic technique for surgery and analgesia of the anterior thigh, hip, and knee. Its effectiveness depends on understanding the femoral nerve's origin, branching pattern, fascial relationships, and common anatomical variations. This review summarizes current anatomical, cadaveric, and imaging studies to clarify the course of the femoral nerve and highlight variations such as high divisions, accessory branches, and vascular overlap that may influence block success. Approaches including landmark, nerve stimulator, and ultrasound-guided techniques

Robotic-assisted surgery has gained prominence in orthopedic practice due to its precision, enhanced visualization, and minimally invasive nature. While robotic applications for orthopedic surgery are well established in arthroplasties, their use in soft-tissue surgery remains limited. This study presents the indications, technique, and clinical outcomes of robotic-assisted latissimus dorsi tendon transfers for shoulder reconstruction. The robotic approach allows accurate identification of the axillary and radial nerves, minimal soft-tissue trauma, reduced bleeding, and excellent cosmetic outc

PURPOSE: Posterior spinal accessory nerve (SAN) to suprascapular nerve (SSN) transfer is a well-established procedure for restoration of shoulder function in upper brachial plexus injuries. Nerve localization traditionally relies on anatomical surface landmarks, which may be influenced by surgeon experience and patient-specific anatomy. Ultrasound provides direct visualization of neural and vascular landmarks before incision. We compared ultrasound-guided preoperative localization with conventional anatomical landmark-based localization during posterior SAN-to-SSN nerve transfer.

Anatomical variations in the brachial plexus can significantly affect upper-extremity surgical procedures. This report highlights a rare case where both pectoralis muscles receive accessory innervation. The pectoralis major exhibited three additional branches stemming directly from the anterior divisions of the superior and middle trunks, effectively bypassing the lateral cord. The pectoralis minor likewise demonstrated variant innervation, receiving two additional branches. One was an unexpected confluence in which a branch destined for the pectoralis major coursed through and innervated the

INTRODUCTION: Restoration of shoulder function is a key objective in brachial plexus surgery, as impaired abduction and external rotation substantially limit upper limb use and quality of life. Distal nerve transfers offer shorter regeneration distances and more predictable outcomes than nerve grafting.
RESEARCH QUESTION: This study evaluated the effectiveness of distal nerve transfers in restoring shoulder function, reducing pain, and improving quality of life in patients with peripheral nerve injuries.
MATERIAL AND METHODS: A prospective cohort of 36 patients with traumatic brachial plexus

BACKGROUND: In complex cervical nerve surgeries, ensuring the integrity of the phrenic nerve (PN) adjacent to the anterior scalene muscle is critical. However, we have encountered a clinical paradox: Postoperative diaphragmatic dysfunction occurred even when the main PN trunk was preserved intraoperatively. This suggests the potential involvement of PN variations (PNVs), such as the accessory PN, which merges with the main trunk, and separate branches that maintain an independent course.
METHODS: Using contralateral C7 nerve transfer (CC7) as a surgical model, this study aimed to characterize
