Right-sided chyle leak encountered during supraclavicular brachial plexus exposure: illustrative case.

BACKGROUND: Chyle leaks are a rare but recognized complication of neck surgery, most commonly encountered on the left due to thoracic duct (TD) anatomy. OBSERVATIONS: The authors present an unusual case of a right-sided chyle leak encountered during brachial plexus exposure for nerve transfer for right hemidiaphragm paralysis. Intraoperatively, a moderate leak of milky fluid was identified near a lymph node bed and controlled with suture ligation. Postoperative recovery was uneventful. Right-sided occurrence suggests variant lymphatic anatomy, possibly involving an accessory TD or a large rig

Lateral Scapular Winging and Shoulder Pain Following Posterior Fossa Surgery: A Case Report.

The spinal accessory nerve (SAN) provides motor innervation to the sternocleidomastoid and trapezius muscles. Injury to the distal segment of this nerve can result in trapezius denervation, leading to lateral scapular winging (LSW), shoulder pain, and impaired arm elevation. While iatrogenic SAN injury most commonly occurs during cervical lymph node biopsies and neck dissections, it may also arise following posterior fossa neurosurgical procedures due to prolonged traction, compression, or mechanical stress during surgical positioning or, less commonly, accidental direct ligation. Recognizing

Iatrogenic Nerve Injuries Affecting Upper Limb Function: Medicolegal Implications.

PURPOSE: Nerve injuries are among the commonest iatrogenic complications and most frequently are the result of direct intraoperative damage causing serious distress and long-term morbidity. Additionally, they can add a considerable financial burden to health care systems, while they constitute a well-known source of medicolegal implications. There are few published articles addressing the medicolegal consequences of the iatrogenic nerve injuries affecting upper limb function. Our aim was to identify malpractice claims related to iatrogenic nerve injuries affecting upper limb function and to de

Anatomy and Ultrasound-Guided Injection of the Deep Branch of the Lateral Palmar Nerve in the Horse: A Cadaveric Study.

OBJECTIVE: Proximal suspensory ligament (PSL) injuries are a common cause of lameness in horses. Localization of pain in the proximal metacarpal and carpal region remains challenging. Current diagnostic analgesic techniques are not PSL-specific. The aim of this study was to investigate the nerve supply of the PSL, with particular focus on the deep branch of the lateral palmar nerve (DBLPaN), and to develop a landmark-based, ultrasound-guided injection technique targeting this nerve. STUDY DESIGN: Eighteen limbs from euthanized horses were dissected to describe the anatomy of the DBLPaN and br

Pisi-palmaris muscle: An anatomical study of a novel hypothenar muscle variant.

The presence of anomalous muscles in the hand are present in around 22-35% of individuals, leading to potential clinical implications. During a routine cadaveric dissection of a 72 year old male, a unique hypothenar muscle variant was identified: the pisi-palmaris muscle. Detailed dissection and literature review determined the origin, insertion, and relationships of the muscle. The pisi-palmaris originated from the pisiform bone, deep to the tendon of the flexor carpi ulnaris and the volar carpal ligament, and inserted onto the transverse carpal ligament, merging with fibers of the abductor a

Distal course and variability of the spinal accessory nerve: a cadaveric study with surgical implications.

PURPOSE: The spinal accessory nerve (SAN) is most vulnerable to injury along its distal extracranial course in the posterior cervical triangle. While proximal anatomy is well described, distal branching patterns and cervical plexus contributions to trapezius innervation remain poorly defined. This study aimed to characterise distal extracranial SAN branching and cervical plexus contributions and to assess whether proximal configurations predict distal trapezial innervation. METHODS: Ten sides from five adult cadavers were dissected from the emergence of the SAN at the posterior border of the

Non-iatrogenic spinal accessory nerve palsy due to lymph node compression.

Injury to the spinal accessory nerve (SAN) is most commonly of iatrogenic or traumatic origin. The incidence of SAN palsy resulting specifically from lymph node compression is difficult to ascertain, as such cases are typically reported in association with malignancies or following surgical procedures. We report the case of an adolescent who presented with clinical features consistent with SAN palsy, including spontaneous shoulder asymmetry, in the absence of prior trauma or surgical intervention to the cervical region. Surgical exploration revealed compression of the SAN by enlarged lymph nod

Clinical anatomy of the superior labial branch of infraorbital nerve.

The infraorbital nerve (ION), a branch of the maxillary division of the trigeminal nerve, provides sensory innervation to the midface via its terminal divisions. Among these, the superior labial branch (SLb) supplies the upper lip and adjacent mucosa, regions frequently involved in oral, maxillofacial, and cosmetic procedures. Despite its clinical importance, the anatomy of the SLb has received relatively limited attention compared with other ION branches. This review synthesizes current evidence on the SLb's course, branching patterns, innervation, morphometry, and variations, with emphasis o

Anterior loop of the mental nerve: a cone-beam computed tomography analysis of its prevalence and surgical implications in implant dentistry.

BACKGROUND: Anterior loops of the mental nerve are important anatomical variations in which the inferior alveolar canal extends anteriorly beyond the mental foramen before looping back. These structures pose a risk of neurosensory injury during procedures such as dental implant placement and bone grafting in the interforaminal region. Cone-beam computed tomography (CBCT) provides high-resolution, three-dimensional images that are crucial for accurately identifying and mapping anterior loops, thereby helping prevent surgical complications. This CBCT study was designed to investigate the prevale

Complex Branching Patterns of the Left Brachial Plexus: A Case Report of Five Concurrent Anatomical Variations.

BACKGROUND: Anatomical variations of the brachial plexus are common and may significantly influence surgical approaches, regional anaesthesia, and the interpretation of peripheral nerve injuries. Variations involving the musculocutaneous nerve and its relationship with the median nerve are particularly important given their clinical implications. CASE PRESENTATION: During a routine surgical anatomy dissection course on a formalin-fixed adult male cadaver, a rare variation was identified in the left upper limb. A 4 cm common trunk arising from both the medial and lateral cords of the brachial

A surgical technique for lesions involving the ultra-high-riding carotid sheath based on the anatomical principles of the prevertebral fascia: cadaveric dissection and case illustrations.

BACKGROUND: The classic extreme lateral approach (ELA) requires posterior retraction of the sternocleidomastoid muscle (SCM) to expose the ultra-high-riding carotid sheath (UHRCS), which is precisely defined as the segment of the carotid sheath (CS) extending from the site where the sigmoid sinus transitions into the jugular bulb (JB) and exits the skull base to the transverse process of the C2 vertebra. Doing so easily injures the parotid gland (PG) and carotid sheath (CS) and causes significant traction on the internal jugular vein (IJV) and accessory nerve. Thus, this study aimed to describ

[Glossopharyngeal, Vagus, Accessory, and Hypoglossal Nerves in Head and Neck Practice].

This review presents a concise, practice-focused overview of the glossopharyngeal (IX), vagus (X), accessory (XI), and hypoglossal (XII) nerves, with particular emphasis on head and neck practice. We outline clinically relevant anatomy and surgical landmarks from the skull base to the cervical field, including constant planes, vascular and osseous reference points, and danger zones encountered during neck dissection, thyroid surgery, and parapharyngeal surgery. We then summarize each nerve's major functions and characteristic deficits, linking sentinel symptoms with bedside and endoscopic find

Iatrogenic Nerve Injuries in Head and Neck Surgeries: A Systematic Review of Mechanisms, Outcomes, and Prevention Strategies.

Iatrogenic nerve injury in head and neck surgery remains a substantial yet potentially preventable source of morbidity across endocrine, otolaryngologic, and related procedures. This systematic review synthesized evidence to identify the nerves most frequently affected, delineate operative mechanisms of injury, describe the clinical course, and evaluate preventive strategies. Traction on visually intact nerves emerged as the predominant mechanism, with additional contributions from thermal injury, compression, ischemia, and entrapment by suture or clip. The recurrent laryngeal, facial, trigemi

Outcomes From Radial Fascicular Nerve Transfers Including Extensor Carpi Radialis Longus to Triceps for Elbow Extension.

BACKGROUND: Reconstruction of elbow extension is an important aim after brachial plexus injury. In extended partial brachial plexus injuries affecting elbow extension, a variety of nerve transfer techniques have been described to neurotize triceps. Spinal accessory, ulnar fascicles, axillary branches, intercostal and phrenic nerves have all been reported. These donors have mostly been transferred to triceps brachii nerves; however, more proximal transfers have also been reported. Results with these techniques have been inconsistent. In extensive partial brachial plexus palsy, radial fascicular

Morphometric characteristics and intradural connections of the spinal root of accessory nerve in human fetuses.

Intradural connections between the spinal root of accessory nerve (SRAN) and the posterior roots of spinal nerves in the cervical region are clinically significant for torticollis, occipital neuralgia and surgeries of the craniocervical region. However, information on these connections during the fetal period is limited. This study aimed to examine the morphology and morphometry of the SRAN in fetuses and its intradural connections with the posterior roots of spinal nerves in the cervical region. Twenty-nine fetuses (58 sides; 18 to 35 gestational weeks) fixed in 10% formalin, were bilaterally
