Predictive factors for distal suprascapular nerve injury in brachial plexus injury: A retrospective cohort study.

BACKGROUND AND PURPOSES: The spinal accessory nerve (SAN) to suprasapular nerve (SSN) transfer is performed to improve shoulder function after brachial plexus injury (BPI) involving C5-6 nerve roots. Unrecognized SSN injury may contribute to reconstruction failure. This study aimed to assess the incidence of distal SSN injury and the associated factors in adults with BPI undergoing SAN to SSN transfer. METHODS: This retrospective cohort study included adults with BPI with planned SAN to SSN transfer. Charts were reviewed to collect demographic, injury, and surgical data. Descriptive statistic

Outcomes of shoulder function in spinal accessory nerve-to-suprascapular nerve transfer in adult patients with complete palsy due to brachial plexus injury: a comparison between the anterior and posterior approaches.

OBJECTIVE: The spinal accessory nerve (SAN) is frequently used as a donor for reinnervation of the suprascapular nerve (SSN) in the reconstruction of brachial plexus injuries (BPIs). This procedure can be performed using either the anterior or posterior approach. However, no studies have compared the postoperative outcomes between these two approaches in adult patients with complete BPI. Thus, this study aimed to compare postoperative shoulder functional outcomes following the anterior and posterior approaches. METHODS: Between 2017 and 2022, 48 adult patients with complete palsy following a

Active shoulder external rotation recovery in upper brachial plexus birth injury following nerve surgery: is spinal accessory nerve-suprascapular nerve transfer superior to C5-suprascapular nerve grafting?

OBJECTIVE: A key residual deficit in brachial plexus birth injury (BPBI) is the lack of active glenohumeral external rotation (ER). Surgical options for reinnervating the suprascapular nerve (SSN) include nerve grafting from a proximal stump and nerve transfer using the spinal accessory nerve (XIN) as a donor. The optimal surgical strategy is unclear from the current literature. This study aimed to compare the outcomes of nerve grafting and nerve transfer for SSN reinnervation in upper trunk BPBI. METHODS: The authors conducted a retrospective study (1990-2020) on prospectively collected data

Motor Nerve Transfers in Complete and Incomplete Brachial Plexus Injuries: A State-of-the-Art Review.

Brachial plexus injuries are challenging conditions. Over the past decades, nerve transfer surgery has progressively evolved from proximal nerve reconstruction toward selective distal neurotization strategies, considerably expanding the possibilities for functional restoration. As the number of described donor-recipient combinations has increased, the literature has become increasingly fragmented, often focusing on isolated techniques or specific functional targets. The aim of the present study was to provide a comprehensive state-of-the-art overview of currently available motor nerve transfer

From zoster to polyneuropathy: manifestations, mechanisms, and differential diagnosis of cranial nerve involvement in Ramsay Hunt Syndrome.

Ramsay Hunt Syndrome (RHS) arises from reactivation of varicella-zoster virus (VZV) in the geniculate ganglion, clinically manifesting as facial palsy and an auricular rash. Involvement of cranial nerves (CN) beyond the facial nerve (CN VII) occurs in up to half of cases, complicating the clinical presentation. Current knowledge regarding polycranial involvement in RHS remains fragmented, largely drawn from isolated case reports and small series. This review synthesizes available evidence on the epidemiology, clinical spectrum, differential diagnosis, and mechanisms of cranial nerve involvemen

Facial nerve schwannoma of the accessory parotid gland region in a child managed through a transoral approach.

Facial nerve schwannomas arising in the accessory parotid gland region are extremely rare, and paediatric cases have not been reported previously. We present the case of a girl in middle childhood who developed a painless, gradually enlarging mass in the mid-cheek area. Radiologic evaluation revealed a well-defined lesion with imaging features suggestive of a benign neoplasm. A transoral surgical approach was chosen to avoid cutaneous scarring and minimise surgical morbidity. Complete excision was achieved without injury to the facial nerve or Stensen's duct, and histopathology revealed a beni

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Surgical Technique: Spinal Accessory to Infraspinatus Nerve Transfer in Brachial Plexus Birth Injury.

Nerve transfers, nerve grafts, and tendon transfers have been used to restore shoulder active external rotation in patients with brachial plexus birth injuries. Traditionally used nerve surgery techniques are nerve transfer from the spinal accessory nerve to a suprascapular nerve (SSN) or nerve grafting from C5 to the SSN. However, results are often suboptimal. A more distal and more targeted transfer from the spinal accessory nerve directly to the infraspinatus branch of the SSN has previously been described and mid-term outcomes are encouraging. Herein, we describe a modification of this tec

Lymph node metastasis in level IIb neck dissection for clinically node-negative oral squamous cell carcinoma patients: an 11-year retrospective study.

The most common complication associated with selective neck dissection is spinal accessory nerve dysfunction and shoulder disability, which result from level IIb dissection. The main objective of this study was to evaluate the incidence of level IIb lymph node metastasis in clinically node-negative (cN0) oral squamous cell carcinoma (OSCC) patients. Patients presenting with cN0 OSCC between November 2012 and November 2023 were included retrospectively. The primary endpoint was the incidence of level IIb lymph node metastasis in these patients. A total of 389 patients (527 supraomohyoid neck di

Resolution of Iatrogenic Spinal Accessory Neuropathy Following Delayed Surgical Intervention.

We present an interesting case of a 74-year-old female who presented with spinal accessory neuropathy following an ipsilateral right-sided carotid endarterectomy 6 months after the initial injury. Subsequent surgical exploration revealed a surgical clip that had been placed directly across the spinal accessory nerve. Clip removal and neurolysis was subsequently performed, leaving the nerve intact. After surgical intervention, she immediately demonstrated increased bilateral shoulder shrug motion and continues to improve her function with physical therapy. This case demonstrates a fascinating c

Anatomic variation of the spinal accessory innervation pattern of the sternocleidomastoid muscle: A cadaveric study.

Several studies have described the anatomical course of the spinal accessory nerve (SAN) and its associated landmarks. In particular, the SAN course through the sternocleidomastoid (SCM) has been reported as either intramuscular or branching without penetration of the SCM. However, it remains unclear what the occurrence of either anatomical course is, as various studies have reported inconsistent findings. For example, Lee SH et al. (South Korea) and Kierner SA et al. (Austria) reported that the SAN more commonly penetrates the SCM (54.1% and 63%, respectively) rather than branching in a clini

Morphometric Analysis of Accessory Palatine Canals in Human Dry Skulls: Clinical Implications for Maxillofacial Surgery.

Introduction The premaxilla, located anterior to the incisive canal and including the central and lateral incisors, plays a crucial role in oral and maxillofacial surgeries due to its neurovascular structures. Accessory palatine canals are of clinical importance as they help prevent complications such as anesthesia failure and nerve injury. While many radiological studies have examined these canals, research on dried skulls remains scarce. This study aims to offer detailed anatomical insights into accessory palatine canals, with the goal of enhancing surgical precision and minimizing complicat
