A multilevel variation of the sensory branches of the lumbar plexus with an accessory genitofemoral nerve.

The lumbar plexus is one of the most variable nerve plexuses in the human body. Located posteriorly to the psoas major muscle, it is the origin of several nerves that cross the retroperitoneal space and are responsible for the innervation of the lower abdominal wall and a portion of the lower limb. Relative to their anatomical route, these nerves can be divided into several major sections: intrapsoas, retroperitoneal, and distal to the inguinal ligament, and all of those are liable to nerve injury during surgical interventions. Here, we describe a complex variation involving the lateral femora

A Cadaveric Study on the Variations in the Infraclavicular Part of Brachial Plexus in Axillary Region and Upper Arm.

BACKGROUND: The brachial plexus shows frequent anatomical variations that can complicate diagnosis, surgical planning, and anesthetic procedures. Detailed knowledge of these variations is essential to prevent iatrogenic injury and improve clinical outcomes. MATERIALS AND METHODS: Eighteen donors (36 upper limbs) were dissected at Mercer School of Medicine, Columbus, Georgia, United States. The sample included 10 males and 8 females, aged 36-89 years (mean age: 67.67 years). Dissections followed Grant's Dissector, 17th edition, with all variations in the infraclavicular brachial plexus and its

[Painful shoulder… not only the rotator's cuff].

A painful shoulder is a frequent complaint in general practice. In this clinical vignette, we discuss the case of an elderly patient with pain and lack of strength in the right shoulder one month after a hospital stay. The diagnosis is an injury of the accessory nerve after the placement of a central venous catheter in an emergency setting. The different causes of non-orthopaedic causes of shoulder pain are reviewed with emphasis on neurological etiologies.

Assessment of the canalis sinuosus and its branches in Taiwanese individuals using cone beam computed tomography.

BACKGROUND: The popularity of dental implant surgeries, particularly in the maxillary anterior region, is growing, and understanding variations in relevant anatomical structures such as the canalis sinuosus (CS) and its accessory canals (ACs) is critical to avoid complications, including nerve injury and tissue necrosis. This study determined the prevalence, diameter, location, and anatomical variations of the CS and AC in a Taiwanese population by using cone beam computed tomography (CBCT). METHODS: CBCT data were collected from 240 participants (aged 20-80 years, with an equal sex distribut

Combined Nerve Grafting and Myoelectric Orthosis Bionic Approach (MOBA) for Functional Restoration After Pan-Plexus Injury.

A variety of approaches have been described to obtain a rudimentary grasp following traumatic pan-plexus injury in the adult. The aim of this report is to present a novel reconstructive algorithm based on the combination of reconstructive surgery and the fitting of a myoelectric orthosis (Myoelectric Orthosis Bionic Approach [MOBA]), to gain grasp function after a pan-plexus injury. A 44-year-old patient presented with a pan-plexus injury after a heavy branch fell on his right shoulder during forestry operations. After 5 months without spontaneous recovery, the patient underwent a brachial ple

High-resolution ultrasound imaging of the spinal accessory nerve and associated injuries based on a prospective normative study and retrospective analysis.

This study assessed the feasibility of high-resolution ultrasound (HRUS) for visualizing the cervical spinal accessory nerve (SAN) and explored its sonographic characteristics, anatomical landmarks, and normative diameter measurements. A prospective study was conducted in 60 healthy volunteers, in whom the SAN was categorized into three segments: between the trapezius and levator scapulae (S1), from the surface of the levator scapulae to the posterior border of the sternocleidomastoid muscle (SCM) (S2), and from the posterior border of the SCM to the upper cervical region (S3). HRUS was utiliz

A Modified Posterior Approach to the Nerve Transfer of the Spinal Accessory Nerve to the Suprascapular Nerve.

PURPOSE: Restoration of shoulder abduction and external rotation is critical in patients with upper brachial plexus injuries (BPIs). The transfer of the spinal accessory nerve to the suprascapular nerve is a well-established surgical technique; however, the optimal approach remains debated. The traditional posterior approach may compromise muscle integrity because of trapezius detachment and splitting of the supraspinatus. This study introduces a muscle-sparing modification to the posterior approach and evaluates functional outcomes in pediatric and adult patients with obstetric or traumatic B

Computed Tomography Detection of Accessory Foramen in the Jugular Foramen: Implications for Skull Base Surgery.

Accurate anatomic knowledge of the jugular foramen (JF) is essential in skull base surgery, where morphologic variations-particularly ossified dural septations-can significantly impact surgical planning and outcomes. A recently described variant, the accessory foramen (AF) located on the inferior surface of the JF, may represent such ossified septations. This study aimed to assess the radiologic presence of AF using high-resolution computed tomography (CT). A retrospective analysis was conducted on 168 high-resolution head CT scans (336 sides). Scans were reconstructed in three dimensions (3D)

Variations in the Formation of the Spinal Accessory Nerve: Spinal Root.

Introduction The spinal accessory nerve (SAN), traditionally recognized as the 11th cranial nerve, consists of a cranial root arising from the medulla oblongata and a spinal root arising from the upper cervical spinal cord. Despite more than a century of investigation, significant uncertainty persists regarding its precise cervical segmental origins, segmental contribution, and dorsal rootlet pattern, which are critical for safe surgical navigation during skull base procedures, posterior cervical exposure, and neck dissections. This study aims to document the length, segmental origins, and dor

Diagnostic domains, differential diagnosis and conditions requiring further medical attention that are considered important in the assessment for Achilles tendinopathy: a Delphi consensus study.

The absence of agreed methods to diagnose Achilles tendinopathy impedes research and clinical practice. This gap results in heterogeneous and/or poorly described study samples, making it challenging to apply findings in clinical practice. The aim of this Delphi study was to define consensus on (1) diagnostic domains; (2) differential diagnoses; and (3) conditions requiring further medical attention, when assessing for Achilles tendinopathy. We conducted a sequential three-stage process which included: (1) identifying diagnostic domains, differential diagnoses and conditions requiring further me

Comparison of Spinal Accessory Nerve Transfer versus C5 Grafting for Suprascapular Nerve Reinnervation in Brachial Plexus Birth Injury.

BACKGROUND: Nerve roots of the upper trunk are usually affected in brachial plexus birth injury. The suprascapular nerve originates from the upper trunk and innervates the infraspinatus muscle, which is the most important external rotator of the shoulder. Two common options for reinnervating the suprascapular nerve are from C5 with an interposition nerve graft or directly from the spinal accessory nerve. The aim of this study was to compare improvement in shoulder external rotation after brachial plexus reconstruction in patients with suprascapular nerve grafted from C5 or with spinal accessor

Biportal endoscopic non-facetectomy foraminal decompression and discectomy (ligamentum flavum turn-down technique).

This study introduces a novel biportal endoscopic foraminal decompression technique that minimizes bone removal while ensuring safe and effective nerve root decompression. Leveraging the accessory process as a key surgical landmark, this technique enables precise navigation and controlled turn-down of the ligamentum flavum (LF). A key advantage of this technique is its reduced requirement for bone resection, differing from traditional microscopic or uniportal endoscopic surgeries that often necessitate resection of the lateral isthmus or superior articular process. This technique is particular

The Great Auricular Point: A Landmark for Safe Sternocleidomastoid Myotomy in Selective Cervical Denervation for Cervical Dystonia.

BACKGROUND AND OBJECTIVES: Cervical dystonia is the most common focal dystonia, characterized by involuntary muscle contractions leading to abnormal postures and significant discomfort. Although neuromuscular blockade with botulinum toxin and other agents is the primary treatment, selective cervical denervation, which includes sternocleidomastoid (SCM) and posterior cervical muscle denervation, is a surgical option for refractory cases. The aim of this study was to evaluate the great auricular point (GAP) as a reliable intraoperative landmark for selective cervical denervation, optimizing spin
