A Randomized Controlled Trial of Intraoperative Nerve Monitoring in Resident-Performed Thyroidectomy.

OBJECTIVE: Intraoperative nerve monitoring (IONM) is widely used in thyroidectomy to aid recurrent laryngeal nerve (RLN) identification and preservation. This study assessed whether IONM improves RLN identification time and overall operative time in less experienced otorhinolaryngology residents performing thyroidectomies. METHODS: This randomized controlled trial was conducted at Siriraj Hospital, Bangkok, from January 2020 to February 2021. A total of 80 patients were randomly assigned to an IONM or non-IONM group. A total of 10 residents performed eight thyroidectomies each, using IONM in

The Great Auricular Nerve as a Nerve Graft Donor: An Anatomical and Clinical Study of the Maximum Harvestable Length and Branches.

BACKGROUND: For immediate facial nerve reconstruction during head and neck tumor resection, a great auricular nerve (GAN) graft can be potentially harvested in the same surgical field during tumor resection. However, it is often avoided because a GAN graft is only approximately 5 cm long without any branches, and a sural nerve graft is recommended for larger defects. We investigated the length of the GAN that can be harvested in 18 patients, along with the evaluation of postoperative facial nerve palsy. METHODS: We retrospectively analyzed 18 cases of immediate facial nerve reconstruction usi

Usefulness of Somatosensory Evoked Potentials in Non-traumatic Spinal Cord Injury in the Absence of MRI Abnormalities: A Case Report.

Non-traumatic spinal cord injury (ntSCI) in childhood rarely occurs during activities of daily living, such as stretching, surfing, or physical education classes. Its pathogenesis remains to be elucidated; however, excessive spinal extension has been suggested as the causative etiology. A previously healthy 14-year-old Japanese girl ran many shuttles in her physical education class and suddenly experienced back pain, followed by severe muscle weakness in the lower extremities. As she was unable to walk independently, she was admitted to the hospital for investigation. Upon admission, she exhib

Extreme lateral infrajugular transtubercular exposure for resection of a glomus jugulare: Microsurgical operative video.

BACKGROUND: Glomus jugular tumors (GJTs), a subset of paragangliomas, are highly vascularized neoplasms arising in the jugular foramen, typically affecting middle-aged women. Although benign, GJTs often present with critical cranial neuropathies such as hearing loss, swallowing disturbances, and facial weakness and can cause venous outflow obstruction from jugular vein compression. In symptomatic cases, early intervention is warranted and involves surgical resection. Approaches include transmastoid approach, juxtacondylar approach, infratemporal approaches, transotic approach, extreme lateral

Immediate repair of the recurrent laryngeal nerve during thyroid surgery via a tension-free end-to-side anastomosis with the Vagus.

The unprecedented technical and technological evolution in thyroid surgery has labelled it as an extremely safe and efficient procedure, and indeed "typifies perhaps better than any other operation the supreme triumph of the surgeon's art."-William Halsted, 1852-1922. Surgeon's experience reflected by annual case load is the most important denominator in thyroid surgery. Nevertheless, even high-volume thyroid surgeons in high-volume centres are not immune to its complications. Despite the advances in surgical technology and techniques, recurrent laryngeal nerve (RLN) injury is still a wellknow

Scholarly Dialogue on Risk to the Recurrent Laryngeal Nerve with Thermal Ablation Procedures: A Reflection on Empirical Practices.

Background: Radiofrequency and other ablative treatments for thyroid nodules and thyroid malignancies are increasingly being adopted into clinical practice. The safety of these procedures with respect to the recurrent laryngeal nerve (RLN) is not well-characterized. Summary: Our current understanding of RLN injury with thermal ablation procedures is nascent. Review of surgical literature and practices offers a framework for voice and laryngeal evaluation and reveals important features of RLN injury. At present, current strategies to mitigate and manage RLN injury in thyroid ablation procedures

AI-assisted recurrent laryngeal nerve identification during endoscopic/robotic thyroid surgery based on the CMC-UNet model: a multicenter retrospective study.

During endoscopic or robotic-assisted thyroid surgery, the field of view may be restricted by tissue swelling or bleeding. These Limitations make delicate surgical manipulation in the confined space more challenging. This study proposes an artificial intelligence model designed to identify the recurrent laryngeal nerve. During thyroid surgery and thereby reduce the risk of accidental injury. This study retrospectively collected imaging data from three tertiary care hospitals, comprising 7482 images from 103 patients. The model relied on this dataset to optimize training. Recurrent laryngeal nerve paralysis as a potential mediator of complications in esophagectomy following lymph node dissection.

Complications from esophagectomy often interact with each other, with those related to recurrent laryngeal nerve (RLN) paralysis (RLNP) being particularly significant. Aggressive dissection of RLN lymph nodes (RLN-LNs) is considered a major contributing factor to RLNP. This study seeks to validate the hypothesis that RLNP acts as a mediator, not only resulting from RLN-LN dissection but also amplifying the likelihood of other postoperative complications. Data were retrospectively extracted from the Chinese 12th Five-Year Major Science and Technology Project on esophageal diseases, including a

Galectin-1 overexpression protects damaged optic nerve and visual function in mice by inhibiting the p38/ MAPK-NF-κB signaling pathway.

The role of galectin-1 (Gal-1) in traumatic optic neuropathy (TON) remains unclear. Therefore, we investigated the role and action mechanism of Gal-1 in TON induced by optic nerve crush (ONC). Forty male C57BL/6J mice were randomly divided into five groups: control (Con), ONC, phosphate-buffered saline (PBS) intravitreal injection, PBS intravitreal injection combined with ONC (PBS + ONC), and Gal-1 overexpression combined with ONC (Gal-1+ONC) groups. The PBS + ONC group was intravitreally injected with PBS and the Gal-1+ONC group was intravitreally injected with a recombinant adeno-associated

Characterization of optic nerve regeneration following Nd:YAG laser-induced injury in Xenopus tropicalis.

A stable, precise, and replicable amphibian model of optic atrophy would advance the study of the mechanisms underlying post-injury optic nerve (ON) regeneration. A recent study developed a transgenic Xenopus laevis model that induced ON transection via laser microdissection, enabling fluorescence microscopy-based monitoring of axon dynamics. However, no studies have used Xenopus tropicalis to investigate optic atrophy. In the current study, we developed an ON injury model in Xenopus tropicalis using an Nd:YAG laser that exhibited several histopathological features resembling traumatic optic n

Beyond Surgical Injury: Systemic Etiologies of Recurrent Laryngeal Nerve Palsy.

The recurrent laryngeal nerve (RLN), a branch of the vagus nerve, plays a crucial role in phonation, respiration, and airway protection. Dysfunction of the RLN can result from various factors, including trauma, surgery, endotracheal intubation, infection, and systemic diseases, leading to clinical symptoms such as hoarseness, stridor, dysphagia, and severe respiratory distress. This systematic review examines the impact of systemic diseases on RLN function. Through an analysis of cases from the English literature, the review demonstrates that RLN dysfunction in the context of systemic diseases

Efficacy of MEDI0618, a pH-dependent monoclonal antibody targeting PAR2, in preclinical models of migraine.

Protease activated receptor 2 (PAR2) is a G-protein coupled receptor expressed in meningeal neurons, fibroblasts and mast cells that may be targeted to treat migraine. MEDI0618, a fully humanized PAR2 monoclonal antibody, engineered to enhance FcRn-dependent recycling and currently in clinical development, was evaluated in human and rodent in vitro assays, in multiple murine in vivo migraine models and in a model of post-traumatic headache. MEDI0618 bound specifically and with high affinity to cells expressing human PAR2 (hPAR2) and prevented matriptase-induced increase in cytosolic calcium. S

Neuroimmune Control of Inflammation in Acute Kidney Injury and Multiorgan Dysfunction.

The nervous and immune systems engage in critical bidirectional communication that influences both physiologic regulation and disease progression. This review discusses the neuroimmune axis and its role in controlling inflammation in AKI and multiorgan dysfunction. Central to this regulation is the inflammatory reflex pathway, which consists of sensory afferent and motor efferent arcs. The cholinergic anti-inflammatory pathway (CAP) suppresses inflammation through vagus nerve activation, norepinephrine release, and α 7 nicotinic acetylcholine receptor signaling in macrophages. Inflammatory med

Utilization of Adipose-Derived Stem Cells in Cranial Nerve Regeneration: A Comprehensive Review.

Large peripheral nerve injuries may require surgical reconnection. Cell-based therapies have also been investigated for nerve regeneration. Within the context of this modern treatment paradigm for peripheral nerve injuries, we investigated the role of adipose-derived stem cells (ADSCs) in the regeneration of cranial nerves. PubMed and Embase databases were used to search for primary studies reporting the use of ADSCs in the regeneration of cranial nerves. A total of 12 studies were included, all of which presented data on specific neural injury, therapy, and functional outcomes. Eight studies

Atypical diabetic neuropathies.

Diabetes is one of the most common conditions in the world and is associated with a broad range of adverse effects in multiple organ systems. Roughly half of all patients with diabetes will develop a typical distal, symmetric polyneuropathy, but several other atypical peripheral nerve conditions can also occur. Treatment induced neuropathy of diabetes is an acute and severely painful small fiber neuropathy that occurs in association with a precipitous drop in glycated hemoglobin levels. Radiculoplexus neuropathies include lumbosacral, cervical, and thoracic forms in which pain and weight loss

Transcutaneous Facial Nerve Frontal Branch Stimulation to Restore Dynamic Elevation of the Paralyzed Eyebrow in Synkinetic Patients.

BACKGROUND: While patients commonly recover motion following facial nerve injury, aberrant neural regeneration can lead to synkinesis and chronic nonflaccid facial palsy (NFFP); many patients fail to recover volitional eyebrow elevation. OBJECTIVE: To assess if transcutaneous electrical stimulation of injured frontal branches can achieve eyebrow elevation. METHODS: We studied healthy volunteers (N = 10) and patients with unilateral NFFP (N = 36). We performed monopolar transcutaneous stimulation of healthy and paralyzed frontal branches with 4-10 mA pulse trains. We measured eyebrow elevatio

The alternative complement pathway drives neuroinflammation and neurodegeneration in mouse models of glaucoma and optic nerve injury.

Glaucoma is a leading cause of irreversible blindness worldwide, characterized by progressive retinal ganglion cell (RGC) loss and optic nerve degeneration. Although elevated intraocular pressure (IOP) is a major risk factor, disease progression can occur despite normal IOP, highlighting the need for neuroprotective strategies beyond IOP reduction. Neuroinflammation has been implicated in glaucomatous neurodegeneration through complement system activation via the classical and lectin pathways. However, the role of the alternative pathway (AP), which functions as an amplification loop for centr
