Brainstem auditory evoked response in the diagnosis of inner ear injury in the horse.

Brainstem auditory evoked response (BAER) testing was done to evaluate inner ear/VIIIth cranial nerve (CN8) function in the horse. The BAER test consisted of stimulating the auditory system with clicks and recording far-field responses of the brainstem auditory components via cutaneous electrodes and a signal averaging system. The normal response was shown to be a series of waves occurring within the first 10 msec after the stimulus click. Functional loss of the auditory receptor organ (cochlea) or CN8 results in loss of the entire response on the side of the injury. Because of the anatomic re

Neoplasms induced by megavoltage radiation in the head and neck region.

Radiation-induced cancer, although fortunately a rare complication of radiotherapy, is nonetheless observed occasionally even after megavoltage radiation has been used. Over a 22-year period at the Curie Institute, four patients were found to have malignant neoplasms within the fields of megavoltage treatment given for various cancers of the head and neck region. Three of the neoplasms were sarcomas, two osteogenic and one fibrosarcoma, and the other tumor was a sarcomatoid epithelioma. The latent period ranged from 3 1/2-15 years. Although the evidence is strong that the neoplasms were causal

Selective percutaneous thermocoagulation rhizotomy in essential glossopharyngeal neuralgia.

Percutaneous radiofrequency (rf) thermocoagulation of the inferior petrous ganglion of Andersch at the jugular foramen has been used to treat three patients with essential glossopharyngeal neuralgia. Two of these patients needed a second rf thermocoagulation, one within 1 week due to an incomplete result, and the other 8 months after the first procedure for recurrence of some neuralgic pain. Thus, a total of five rf thermocoagulations of the ganglion of Andersch have been performed. The patients are now pain-free and without significant side-effects. Deglutition and phonation have remained int

[Surgery of the recurrent nerve. Studies on the problem of functional rehabilitation after recurrent nerve injuries].

One of the most important problems involved in rehabilitation surgery of the recurrent nerve arises from the fact that this nerve carries fibers displaying antagonistic activities which is an accordance with the different functions of the larynx as an organ of phonation, respiration and swallowing. As a consequence of random reinnervation of the internal laryngeal musculature taking place after reconstruction of the N. recurrens, the complicated sequence of vocal cord movements, varying with the different functions of the larynx, cannot be restituted in its proper order. To avoid such function

Influences of the glial environment on the elongation of axons after injury: transplantation studies in adult rodents.

Tissue transplantation methods, previously used to study neural development, myelination and inherited disorders of myelin can be applied also to the investigation of repair and regeneration in the mammalian CNS. The elongation of axons from injured peripheral nerve of CNS has been studied in adult mice and rats by observing the growth of axons into PNS or CNS tissue grafts. Following spinal cord injury and also after transplantation of optic nerves into the PNS there is axonal sprouting but these neuronal processes fail to elongate more than a few mm into the surrounding glia. On the other ha

Facial paralysis in children: differential diagnosis.

The differential diagnosis in 170 patients between birth and 18 years of age is reviewed. There are a number of obvious physical findings and historical features that allow one to make a diagnosis rather quickly. Pain, vesicles, a red pinna, vertigo, and sensorineural hearing loss suggest herpes zoster oticus. Slow progression beyond three weeks, recurrent facial paralysis involving the same side, facial twitching, weakness, or no return of function after six months indicate a neoplasm. Bilateral simultaneous facial paralysis indicates a cause other than Bell's palsy, such as Guillain-Barré sy

Surgical treatment of vertigo.

Endolymphatic saccotomy, vestibular neurectomy via middle fossa, labrinthectomy and translabyrinthine vestibular neurectomy have been used by the authors in 54 patients with episodic vertigo, chiefly from Ménière's disease. Over a mean follow-up period of 15 months hearing retention and vertigo control were of the same general order as in the reports of other surgeons. Our current ideas on indications for the different surgical procedures are discussed.

Radiation-induced malignant and atypical peripheral nerve sheath tumors.

The reported peripheral nerve complications of therapeutic irradiation in humans include brachial and lumbar plexus fibrosis and cranial and peripheral nerve atrophy. We have encountered 9 patients with malignant (7) and atypical (2) peripheral nerve tumors occurring in an irradiated site suggesting that such tumors represent another delayed effect of radiation treatment on peripheral nerve. The neoplasms appeared 4 to 41 years following radio-therapy, at a time when the patients (6 females and 3 males) were 16 to 70 years old and cured of their original tumors. Eight patients had received irr

Pattern shift visual evoked response in carotid occlusion.

Pattern shift visual evoked response (PSVER) latency has been shown to be an extremely sensitive but nonspecific measure of optic nerve dysfunction. The test has been most useful in detecting unsuspected demyelinating lesions of the optic nerve, but abnormal results have been reported in a variety of other conditions affecting the optic nerve, including ischemic optic neuropathy. We report a patient with left internal carotid artery occlusion, possibly secondary to neck trauma, with normal ophthalmological examination and abnormal PSVER. The suggested mechanism is subclinical optic nerve ische

Trigeminal sensory neuropathy and bilateral carpal tunnel syndrome: the initial manifestation of mixed connective tissue disease.

A patient is described who devloped a bilateral carpal tunnel syndrome and a trigeminal sensory neuropathy as the initial manifestations of mixed connective tissue disease. Either condition can occur in other connective tissue diseases, but this combination has not been reported previously in mixed connective tissue disease, in which, however, trigeminal sensory neuropathy is frequently seen.

The response of ornithine decarboxylase during neuronal degeneration and regeneration in olfactory epithelium.

Mature olfactory neurons are continually replaced from a population of progenitor cells. Olfactory nerve section, bulbectomy, or treatment with certain chemicals induces degeneration of olfactory neurons followed in some cases by regeneration. Ornithine decarboxylase (ODC) activity was measured in mouse olfactory tissues as an indicator of cellular regeneration. ODC activity in olfactory tissue (0.2-0.4 nmol/mg protein/h) is 10-30 times higher than in a variety of other cerebral tissues. Within 3 h after unilateral olfactory nerve section, ODC activity in the epithelium declines to 50% of cont

Axonal uptake of horseradish peroxidase isoenzymes during Wallerian degeneration.

Horseradish peroxidase isoenzymes were applied around crushed mouse hypoglossal nerves to study the influence of electrical charge on the uptake and ultrastructural distribution of macromolecules in axons distal to an injury. Both isoenzymes tested (Sigma type IX, cationic and type VII, anionic) were readily taken up into axons and moved in a distal direction along the nerve. Samples taken 1-3 mm below the crush showed that reaction products from both enzymes covered the inner surface of the axonal plasma membrane and were attached to organelles, particularly microtubules and neurofilaments. Facial nerve paralysis secondary to mandibular fracture.

When facial nerve paralysis complicates a mandibular fracture, it may be difficult to locate the point of injury, since the nerve may be injured in the fallopian canal by a secondary temporal bone fracture or in soft tissue by mandibular fragments. Following a review of eight previously reported cases, this paper presents two additional cases, one with bilateral, complete paralyses and the other with a unilateral paresis. In the first, the condyles were driven posteriorly, resulting in bilateral temporal bone fractures, a unilateral external canal stenosis, and a unilateral sensorineural heari

Destruction and regeneration of jawbone after cryogenic application. An experimental study.

On 13 rabbits part of the lateral aspect of the jawbone was frozen with liquid nitrogen in two sessions. The animals were sacrificed 2, 4, 12 and 24 weeks after the cryosurgery. The specimens were analyzed with microangiography according to the Spalteholz technique and with light microscopy. The whole width of the lateral cortex, measuring around 3 mm, was subject to bone destruction two weeks after cryosurgery. New bone formation, on the other hand, was evident in the frozen area and on the undamaged periosteum some distance from the lesion. The same area, 12 weeks after the injury, was compl

Regional difference in susceptibility to damage of vestibulo-semicircular canals in experimental labyrinthine lesion.

In the study on regional difference in susceptibility to damage of vestibulo-semicircular canals upon causing labyrinthine lesion by four different procedures, namely, by intracranial approach, through the middle ear, through the facial nerve and by means of experimental endolymphatic hydrops, the following results were obtained. 1. The crista of posterior semicircular canal among the semicircular canals and the macula of the saccule within the vestibule were most susceptible to impairment. In other words, the most susceptible region was the so-called pars inferior. In regard to the impairment

Influence of site of origin of lung carcinomas on clinical presentation and central nervous system metastases.

A retrospective survey of 100 patients dying from carcinoma of the lung showed that neurological presentation and central nervous system metastases are more frequently present when the primary carcinoma is situated in the peripheral lung tissue (including lung apex). The high incidence of cases presenting neurologically and the high incidence of single secondary deposits involving the central nervous system from peripheral or apical growths suggest a difference in the mode of spread or other properties of such growths compared with the more common central carcinoma.

Function of submaxillary gland following iatrogenic damage to chorda tympani nerve.

Flow rate, protein and amylase content of submaxillary saliva were determined before and after iatrogenic damage to the chorda tympani nerve during otosurgery. Parallel to these investigations, gustatory function was checked. Chronic otopathy itself did not lead to a disturbance of either the sense of taste or the function of the submaxillary gland in any of the 14 patients. On the other hand, the flow rate of the affected gland dropped after chorda division (8 patients) and after chorda stretching (6 patients), and the protein concentration of the saliva was increased while the concentration

[Disorders of the coordination of masticatory muscles in lesions of the brain stem].

Observations of 10 patients were reported. In addition to other neurologic deficits, coordination of the trigeminus, a motor nerve, may also be disturbed with brain lesions. Disturbance of bilateral synergism can be demonstrated with the EMG. The muscles closing the jaw on the affected side are paradoxically innervated when the jaw is opened, i.e., they function like antagonists to the muscles responsible for jaw closure on the opposite side. This condition may produce trismus. Similar central disturbances in bilateral coordination of the ocular muscles have also been established. Bilateral sy

Neurological complications of anticoagulation therapy: a neurologist's review.

Anticoagulant-induced bleeding may involve the brain, spinal cord, or peripheral nerves. It may occur with either coumarin or heparin anticoagulants, in the absence of trauma, and while prothrombin times, clotting times, or partial thromboplastin times are in accepted therapeutic ranges. Bleeding involving the brain occurs usually as a subdural hematoma, with intracerebral or subarachnoid hemorrhages occurring less frequently. There may be diagnostic problems in the recognition of these anticoagulant-related subdural hematomas. The spinal cord involvement usually appears in the form of an extr

Utilization of a central auditory processing test battery in evaluating residual effects of decompression sickness.

Six professional divers were examined as patients who presented peripheral auditory, vestibular, or central symptoms attributed to incidents which occurred while diving. Each diver received a central auditory processing (CAP) test battery consisting of: (1) a dichotic sentence listening task, (2) a monotic filtered word task, (3) a binaural fusion task, and (4) an alternating speech task. The CAP test results, in all cases substantiated other neurological/vestibular test battery results which we have reported in detail previously.
