Neurologic disturbances in Paget disease of bone: response to calcitonin.

The neurologic manifestations of Paget disease and the therapeutic effect of calcitonin were studied in 49 patients. Twenty-four patients (49%) had neurologic disorders involving cranial nerves other than the auditory system, brainstem, spinal cord, or spinal roots and nerves. Eighteen of the 24 patients (75%) showed significant subjective or objective improvement after calcitonin treatment. The effect of calcitonin treatment on spinal cord compression was dramatic in three of six patients. The observations made of these patients support previous data suggesting that the neurologic signs and s

Progressive visual failure in acromegaly following external pituitary irradiation.

Four out of twenty-three acromegalic patients selected for treatment with external megavoltage pituitary irradiation between 1961 and 1975 developed progressive visual failure. They had received megavoltage external irradiation through multiple portals from a cobalt-60 unit over a period of 3 weeks. Visual deterioration began 2 months to 6 years after irradiation. In two patients the optic nerves were explored. In both, post-mortem later confirmed radiation damage to the optic nerves and hypothalamus. In one case there was also necrosis of the right frontal lobe with necrosis and inflammation

Restoration of selective facial nerve function by the nerve-muscle pedicle technique.

The nerve-muscle pedicle technique for selective reinnervation of paralyzed facial muscles, when combined with a modified muscle-fascia sling and neurotization from the temporal muscle, has been applied in 26 patients to date. The success rate has been 88 per cent for restoration of tonus and 77 per cent for restoration of voluntary motion as well. This procedure offers several significant advantages over other accepted means of reinnervation in patients in whom the seventh nerve itself is not available as a source for restoration of function. It also may have value as a method for rapid resto

The effects of acute and chronic morphine treatment on the process of facial nerve regeneration.

Prominent cellular responses to axonal interruption include enhanced synthesis of RNA and protein in the neuronal perikaryon, and proliferation of reactive Schwann cells. Since morphine has been shown to significantly depress cellular metabolism, we examined its effect on these and other reparative responses underlying nerve fiber regeneration. Rat facial nerve trunks from saline, acute morphine, and continuous morphine-treated animals were examined by light and electron microscopy at 3, 7 and 14 days after crush injury. The number of axonal sprouts/unit area and the diameters of regenerating

Clinical observations in cyclodeviations.

Cyclodeviations differ from other strabismus forms in several aspects. Adaptive mechanisms other than suppression cause the patient to remain asymptomatic in spite of ophthalmoscopically demonstrable torsion of the globe around the sagittal axis. These mechanisms include cyclofusion and a monocular sensory adaptation causing the images seen by the paretic eye to appear vertically and horizontally aligned even though they do not fall on the vertical and horizontal anatomic-geometric retinal meridians. Pure excyclotropias, ie, those not associated with vertical deviations, respond well to anteri

Selected pain states and the use of anesthesia in dentistry.

Certain pain disorders may be difficult to diagnose because their presenting symptoms are confusing and difficult to classify, or because they occur so infrequently that they are not commonly considered. This article has presented a classification and discussion of selected pain states which the practitioner may find difficult to diagnose. When faced with a perplexing pain problem, the dentist is encouraged to perform a careful evaluation to determine whether the patient's pain is or is not of dental origin. Certain pain states require that the patient be referred for evaluation to other denta

[Extra-dural hematoma of the posterior fossa (author's transl)].

10 cases of posterior fossa extra-dural hematoma are reported and 100 other published cases reviewed. It appears that the sole clinical evaluation frequently leads to wrong or delayed diagnosis. In only one out of five cases the cerebellar signs and the palsy of one or several cranial nerves (VII, IX, X, XI, XII) are prominent. In contrast, all other cases do not present specific signs. Furthermore, the presence of a concomitant supratentorial traumatic lesion may contribute to prevent the finding of evidences for cerebellar extra-dural hematoma. The possibility for such hematoma must always b

Hemifacial spasm and the facial nucleus.

Spontaneous and associated hyperkinetic facial movements and contracture which follow injury to the seventh cranial nerve (postparalytic hemifacial spasm) or arise without known previous injury (cryptogenic hemifacial spasm) are pathological motor phenomena not found in the distribution of other cranial or somatic motor nerves. The commonly expressed hypotheses of pathogenesis--aberrant regeneration and fiber excitation by false synapse formation (ephapses) at the site of injury--cannot account for all aspects of these phenomena or for the uniqueness of such movements to the distribution of th

Fine structure of reactive cells in injured nervous tissue labeled with 3H-thymidine injected before injury.

To examine the fine structure of blood mononuclear cells in injured nervous tissue, mice were given repeated injections of 3H-thymidine with the last injection at least 16 hours before injury. Under ether anesthesia the animals either were given a stab wound to the spinal cord or had their left hypoglossal nerve transected. The animals were killed at 2, 4, 8, or 16 days after injury. Tissue sections containing the spinal cord wound or both hypoglossal nuclei were prepared for electron microscopic radioautography, and all labeled cells were photographed. About half the labeled cells in the inju

The iris sphincter in aberrant regeneration of the third nerve.

Fourteen patients with aberrant regeneration of the third cranial nerve were examined using self-developing photographs, motion pictures, and pupillography. These pupillary phenomena were noted: (1) sector contractions of the iris sphincter in response to light, (2) sector contractions of the iris sphincter associated with eye movements, and (3) an abnormal pupillary unrest. The observations strongly support the concept that after injury, fibers regenerate and sprout collateral branches that finally reach muscles of the oculomotor group other than those originally innervated.

Uptake and retrograde axonal transport of various exogenous macromolecules in normal and crushed hypoglossal nerves.

Macromolecular tracers were injected into the tongue or around a crush in mouse hypoglossal nerves. At various times thereafter, the tracers were histochemically localized on the basis of peroxidase activity. The distribution of reaction product was then examined using light microscopy in order to study the influence of molecular charge and size on uptake and retrograde axonal transport from the periphery or from the crushed axon. Of various proteins with peroxidase activity, horseradish peroxidase and cytochrome-c showed the greatest penetration into axons proximal to the crush. Following inj

Central retinal vascular occlusion associated with oral contraceptives.

A 40-year-old woman took oral contraceptives for four years. She developed a vascular glaucoma and one year later the eye was enucleated. No other apparent cause for the occlusion was present. The vascular occlusion consisted of a combined central retinal artery and vein occlusion with severe intimal hyperplasia. An intravascular granulomatous reaction is attributed to chronic vascular injury.

Diagnostic significance of summating potential in sensorineural deafness.

The polarity of the summating potential wave induced by electrocochleographical examination has an important bearing upon the pathophysiology or the prognostic evaluation of sensorineural deafness. Electrocochleographical examination was performed on cases of sudden deafness, Ménière's disease, progressive sensorineural deafness, sensorineural deafness due to barotrauma, etc. When the polarity of the summating potential showed an increased negative wave, physiological blockage in sensorineural structures was revealed. In other words, under such conditions sensorineural structures have a potent

[Anastomosis of the facial and hypoglassal nerve using coagulated plasma. Our first experiences].

The authors present their first experience in the construction of an anastomosis between the facial and hypoglassal nerves. This anastomosis was made in 3 patients; in one of them who had a lesion of n. facialis, the anastomosis was made 8 months following the time of the lesion, and the result was very good. In the other 2 patients the results were satisfactory, even though the procedure was carried out 9-10 years following the injury to this nerve. The authors stress the importance of the promptness of the operation, and the postoperative rehabilitation course.

Analysis of trochlear nerve palsies. Diagnosis, etiology, and treatment.

At the McGill University neuro-ophthalmology unit, 52 patients with superior oblique palsy were seen during the 2-year period October 1973 to August 1975; these included patients with congenital, traumatic, vascular, and other more rare causes of trochlear paralysis. Half of the patients with congenital palsy had diplopia, requiring treatment, although surgery was rarely necessary. Patients with palsy due to trauma, the major cause in our series, included four with bilateral involvement. With the exception of recovery in a single muscle of one patient in this group with traumatic palsy, no spo

Intrathoracic injury to the motor nerve supply of the larynx.

A previously reported experimental study demonstrated that the intermediate (partly abducted) position of a paralysed vocal cord may be due to physiological inactivation, rather than paralysis, of the cricothyroid muscle. This inhibition was shown to be caused by interruption of vagal afferent impulses originating in pulmonary pressure receptors. The case reported here offered an unusual opportunity of studying this condition by serial section of a human larynx. A patient's left vocal cord was paralysed by cancer in the left pulmonary hilum and apex. There was aspiration and loss of voice. The

Immunological concept for Bell's palsy: further experimental study.

This report concerns itself with additional experimental evidence to support the immunologic concept for the pathogenesis of Bell's palsy, using the mast cell as an index of immunological activity. In a previous experimental study, we postulated that degranulation of mast cells activated by complement or specific allergens with release of histamine and other substances may be the mechanism leading to nerve edema, ischemia, and paralysis. In this study we observed a loss of granulated mast cells in the more severely damaged facial nerves of immunized dogs after the intrafallopian canal injectio

Management and rehabilitation of the facial nerve in parotid surgery.

The facial nerve is the single most important consideration in the surgical management of most parotid disorders. Its surgical anatomy should be well known by the head and neck surgeon. Very few conditions of the parotid gland necessitate the sacrifice of the facial nerve. When this decision is made, the benefits to be derived should be very thoughtfully measured against the tremendous cosmetic and functional sequelae that follow, and the patient should understand preoperatively to the fullest the magnitude of this decision and all its consequences. Most facial nerve deficits resulting from in
