[The painflu stiff shoulder following radical neck dissection (author's transl)].

72 patients following radical neck dissection were studied. The accessory nerve was sacrificed in 60 patients and preserved in the remaining patients (for a control group). The most important clinical findings in all patients are reported. Most patients with loss of the accessory nerve complained more of postoperative pain and limitation of shoulder movement than of other clinical difficulties or cosmetic defects.

[Etiology and pathogenesis of neurological symptoms in low bite].

50 patients were subjected to complex examination. In bite lowering the head of the condyle is displaced backwards and upwards into the depth of the glenoid fossa. The pressure of the displaced mandibular joints upon the arterial cord and the tympanic vein as well as upon the venous network around the mandibular joint capsule may produce disturbed blood supply to the mucosa of the middle ear and of the auditory tube, and finally, obstruction of the auditory tube. Disturbances in the blood supply of the middle ear irritate the tympanic cord and tympanic plexus, which causes burning of the tongu

[The trigeminus nerve in neuro-otological diagnosis (author's transl)].

The Trigeminus nerve is represented by a large area in brainstem and therefore we often find symptoms of this nerve, if there are dysfunctions in this region. Especially when we suspect disorders of the central part of the vestibular or auditory systems the Trigeminus can give distinct signs. Besides other qualities (tactile, nociceptive) the sensitivity for warmth can be measured exactly with the thermic-impulse-test (TITT). The importance of this test is shown in some cases (tumor of the cerebello-pontine angle, tumors in the brainstem and multiple sclerosis). On the other hand in lesons of

Bilateral injury to the hypoglossal nerve.

The hypoglossal nerve supplies motor function to the tongue. Two cases of bilateral, post-traumatic injury to the hypoglossal nerve are described. Physical and electromyographic examinations in these cases showed evidence of bilateral injury to the hypoglossal nerve as well as the absence of injury to the other closely associated cranial nerves. (A review of the English literature in the past 20 years shows no cases of isolated bilateral damage to the hypoglossal nerve, although there are several cases of unilateral or combined injury.) The disabilities resulting from hypoglossal nerve palsy a

Glaucoma in oculodermal melanocytosis.

Case reports of oculodermal melanocytosis (nevus of Ota) and ipsilateral glaucoma often have other ocular disorders which could account for the glaucoma. It has not been clearly shown that the oculodermal melanocytosis alone can cause secondary glaucoma. In the present case report, optic nerve head and visual field changes, suggestive of a long-standing unilateral elevation of intraocular pressure, occurred in an eye with oculodermal melanocytosis and no other apparent ocular disorder to account for the chronic glaucoma.

Solitary oculomotor nerve palsy in childhood.

In most cases of isolated oculomotor nerve palsy in 30 patients under age 20 years the palsy was congenital, and aberrant regeneration was present. The most common causes of acquired oculomotor nerve palsy were blunt trauma and infectious processes, both local and systemic. Neoplasms, aneurysms, and ophthalmoplegic migraine caused the other cases. The causes of isolated oculomotor nerve palsy in childhood differ from those in adults both in nature and in frequency of occurrence.

[Temporal cochlear changes evoked by prolonged exposure to low intensity noise. Electrophysiological study in guinea pigs].

Behavioral temporal threshold shifts (TTS) have been shown in the chinchilla, to be correlated with cochlear microphonic (CM) and whole-nerve action potential (AP) losses in the sensitivity as well as in maximum voltage. The Guinea-pig's ear differs significantly both anatomically and physiologically from the chinchilla's; as a result, it does not seem to be as susceptible to TTS at non-injurious noise levels, except at those frequency bands were middle-ear impedance and other factors are more favorable. At times varying from 2 hrs to 48 hrs after termination of noise exposures of 48 hrs durat

The authors report a series of 9 cases of meningoradiculitis after tick bites and compare them with 56 other cases in the literature. Clinically, the bite is followed by a free interval, then more or less extensive local erythema, pain then paralysis is undoubtedly the most typical presentation. Uni- or bilateral paralysis of the 7th cranial nerve was found in more than 50% of cases. Erythema may exceptionally be absent. There may be forms with pain alone. Finally, there may sometimes be pyramidal signs or signs of brain irritation. As concerns the course, one should note the absence of respir

The influence of upper respiratory tract receptors stimulation on the respiration and heart action in rats.

1. Stimulation of the upper respiratory tract with water releases reflex respiratory arrest and very marked bradycardia due to conduction and impulse generation disturbances in the heart accompanied by ventricular hyperexcitability. 2. Asphyxia causes bradycardia as a result of an inhibition of sinus node and the development of vicarious rhythms with excessive ectopic ventricular excitability. The intensity and duration of bradycardia during asphyxia are less than during water stimulation of airways. 3. Urethane anaesthesia did not prevent the development of the above described changes appeari

[Radiological studies in otology].

Skull X-rays with special projections, particularly tomography of the temporal bone, are very helpful procedures in the differential diagnosis of diseases of the temporal bone. Tomography serves to delineate the size and distribution of tumors in the middle or inner ear. Furthermore, in fractures of the temporal bone the fracture line can be seen throughout the entire osseous structure. Special projections are reported for tomography of the temporal bone, with special emphasis on the indications for such procedures. The contralateral effect of antidromic stimulation of the trigeminal nerve on the rabbit eye.

The effect on contralateral eyes after injuries to one eye has been called the consensual reaction and has been postulated to be either the consequence of a neural reflex or one achieved by circulating substances. Trigeminal stimulation always causes ipsilateral miosis, ocular hyperemia, intraocular hypertension, and a disruption of the blood-aqueous barrier. Disruption of the blood-aqueous barrier in the contralateral eye after stimulation of the trigeminal nerve always occurs and depends on intact sensory innervation to that globe in rabbits. The disruption is not prevneted by pretreatment o

The recurrent laryngeal nerve.

Nonrecurrence and variations in the ascending course of the recurrent laryngeal nerves make it essential to identify the nerve to avoid injury to it during thyroidectomy. We believe that visual identification of the nerve without undue handling is all that is necessary. The recurrent nerve is no more delicate than other similar nerves. Unilateral injury to the recurrent nerve may result in temporary hoarseness which will improve with time. Some restriction of the airway during exertion may be present. Bilateral injury to the recurrent nerves may produce initially a loss of voice without airway

Typical results of computer-olfactometry.

After pulsed peripheral olfactory impulses it is possible, under suitable testing conditions, for specific sensory potentials to be read from the surface of the skull and, after repetitions, added up (olfactorially evoked cortical potentials). Typically, there developes a so-called twin-potential containing the equivalents of the electrical activites of the nervus trigeminus and the nervus olfactorius in two peaks of the cortex equivalents. In an objective manner, in other words without intentional or vegetative influence by the test person, it is possible by the weakening or lack of one or th

[The activity of motor units of the musculus splenius capitis in pheripheral vestibular lesions (author's transl)].

The activity of single motor units of the splenius-capitis muscle of healthy human subjects and patients with unilateral vestibular lesions was recorded by means of microelectrodes. The major part of these units reacted to vestibular stimulation in the same way as primary and secondary vestibular neurones of other vertebrates. The reactions of these units to trapezoidale stimuli is identical to that of vestibular neurones. An angular acceleration of more than 0.6 degrees/s2 is for these units a stimulus which is above threshold, this too corresponds to findings in the vestibular nerve. In pati

Histopathologic correlates of otoneurologic manifestations following head trauma.

The clinical course of patients dying of head trauma and the physiologic evaluation of audition in guinea pigs subjected to several forms of head trauma are correlated with the histopathologic findings in the brains and temporal bones. In the patients there was wide spread hemorrhage, edema, disorganization and disruption of neural tissue. Loss of Purkinje cells and other neural elements was prominent. The temporal bone findings in the patients included laceration of and hemorrhage into the VIIIth nerve as well as bleeding into the scala tympani. The membranous labyrinths demonstrated no speci

Etiology of bilateral abductor vocal cord paralysis: a review of 389 cases.

The etiology of 389 cases of partial or complete bilateral abductor vocal cord paralysis has been determined and classified. One hundred and forty-nine were infants and children 12 years of age and under; 240 were adults, age 13 and older. In the infants and children the paralyses were congenital in 82 cases, of which 43 were associated with other congenital anomalies, and 39 were without associated anomalies. Fifty-nine cases were considered acquired, most being secondary to underlying congenital anomalies, particularly the associated findings of meningomyelocele, Arnold-Chiari malformation,

[Surgery of vertebral aneurysms at the origin of PICA (author's transl)].

Thirteen cases of vertebral aneurysm at the origin of PICA (VA-PICA aneurysm) were operated on at the Department of Neurological Surgery of Kitano Hospital from March, 1970 through July, 1975. Those included 9 cases of saccular aneurysm and 5 cases of fusiform aneurysm (Table 1). The incidence of VA-PICA aneurysms among our whole series of intracranial aneurysms was 4.2%. Patients with subarachnoid hemorrhage were subjected to our routine 4 vessel angiography. For those with suspected vertebral aneurysm vertebral angiography was performed in a transoral projection. In this method, when the ang

General review of the literature makes it apparent that there exist various combinations of associated movements between cranial nerves. Most of them involve four of nerves, the oculomotor, the abducens, the facial, and the motor potion of the trigeminal nerve. Tobramycin sulfate: a summary of worldwide experience from clinical trials.

Tobramycin sulfate, a new bactericidal aminoglycoside antibiotic, has been evaluated in 3,506 patients. Its in vitro activity is similar to that of other aminoglycoside antibiotics, except that it is more active against Pseudomonas aeruginosa. Clincal studies have shown that tobramycin sulfate is well tolerated and effective in the treatment of the infections listed below when they are caused by susceptible gram-negative organisms and by staphylococci: infections of the respiratory tract (including cystic fibrosis), central nervous system, skin, soft tissue, and bone (including burns); urinary
