[Isolated trochlear nerve paralysis following head trauma].

39 cases with isolated trochlear nerve palsies of traumatic origin have been analyzed retrospectively. 18 patients (46%) had had cerebral contusion, 15 (39%) cerebral concussion, and 6 patients (15%) a minor head trauma. 33 patients had unilateral trochlear nerve palsies and 6 (all of them with cerebral contusion) bilateral. The degree of the palsies did not correlate with the severity of the head trauma. Essential pathogenetic mechanisms were frontal or occipital blows. We emphasize a fact hitherto underestimated in the literature, that even a relatively mild head trauma (cerebral concussion

A radiographic and histologic study of the topographic relations in the temporomandibular joint region: implications for a nerve entrapment mechanism.

A radiographic and histologic investigation was performed in 18 temporomandibular joint (TMJ) autopsy specimens. Disc position was determined arthrotomographically. The pathway of the nerve branches in the vicinity of the joint was reconstructed from serial sagittal or frontal histologic sections. The relationship between the joint components and the different nerves running in the vicinity of the joint was studied. The results revealed the existence of topographical prerequisites for mechanical influence upon the nerve branches passing in the TMJ region. In two joints, both with a displaced d

A new orthosis for central cord syndrome and brachial plexus injuries.

Proximal upper extremity weakness may develop secondary to central cord syndrome due to spinal cord injury or brachial plexus injury. Functional deficits, pain, decreased upper extremity arm swing during gait, and shoulder subluxation are common sequelae of these injuries. This report describes a new orthotic design that can be easily fabricated in two to four hours from readily available materials to compensate for these deficits. This orthosis allows for early participation in activities of daily living for patients with greater proximal than distal upper extremity weakness. The orthosis con

[Skullbase-penetrating injuries caused by umbrella tips: case reports].

Two cases of skullbase-penetrating injuries caused by umbrella tips are reported. Case 1: 24-year-old male. Admitted with disturbance of consciousness, left hemiparesis, nasal bleeding, and laceration of left lower eyelid because of having been stabbed by an umbrella tip. Pupils and fundi revealed no definite findings. Plain skull X-ray showed turbid ethmoid sinus and fracture of planum sphenoidale. Cranial CT showed right putaminal hematoma with intraventricular hemorrhage and pneumocephalus. Increased ICP necessitated surgery two days after the injury. Dural laceration of planum sphnoidale,

Transient central retinal artery occlusion and contralateral amaurosis after retrobulbar anesthetic injection.

A transient, ipsilateral central retinal artery occlusion and contralateral amaurosis developed following retrobulbar injection of 2% lidocaine and 0.75% bupivacaine. A computerized tomography scan obtained 1 1/2 hours following the injection demonstrated an air bubble within the ipsilateral optic nerve sheath. This case further supports the view that intranerve sheath injection can cause these two complications associated with retrobulbar anesthesia.

Natural history of atlanto-axial subluxation in rheumatoid arthritis.

In 1969 the authors started their prospective study to determine the natural history of atlanto-axial subluxation in patients suffering from rheumatoid arthritis. Between January 1969 and July 1971 they accumulated 41 cases of atlanto-axial subluxation. These patients were studied clinically and radiologically, and the initial findings were published in the Journal of Bone and Joint Surgery, Volume 55B, August 1973. All of these patients were followed clinically and radiologically until 1981 or until their demise. Roentgenographically, 61% have remained unchanged, 27% have shown progression of

Recurrent laryngeal nerve palsy following heart-lung transplantation: three cases of vocal cord augmentation in the acute phase.

Three cases of left recurrent laryngeal nerve palsy following heart-lung transplantation are described. In each case, within twelve hours of extubation, the left vocal cord was injected with Teflon, and the paralyzed vocal cord thus displaced to the midline. Effective closure of the glottis was then possible, permitting an adequate cough, adequate clearing of the bronchial tree and minimising the risk of aspiration. Augmentation under general anaesthesia as soon as possible after discovery of vocal cord dysfunction is advocated. Suitable materials for injection are discussed. To our knowledge,

Paralyzed left vocal cord associated with ligation of patent ductus arteriosus.

We reviewed the records of 167 neonates and older children undergoing ligation of a patent ductus arteriosus to determine the prevalence of recurrent laryngeal nerve injury and identify risk factors that might increase the likelihood of injury. Paralysis or paresis of the left vocal cord was identified by fiberoptic laryngoscopy in seven patients. All seven weighted less than 1500 gm at the time of operation and the ductus had been ligated with a metal clip. All had persistent symptoms when observed for 5 to 19 months. Injury to the left recurrent laryngeal nerve occurs infrequently but may ha

[Optic nerve damage following craniocerebral trauma. A critical analysis of trans-ethmoid decompression of the optic nerve].

Over a period of 10 years 39 patients who suffered an optic nerve compression after a craniocerebral trauma underwent transethmoidal decompression surgery. The operation was performed bilaterally on 5 patients. Fifty per cent of the patients involved suffered a blunt head or brain injury, the others a brain concussion or space-occupying haemorrhage. At the side of optic nerve compression we found specific signs and symptoms, such as negative or sluggish direct light reaction of the pupil, a wound on the lateral side of the eyebrow, bleeding from the nose, eyelid haematoma, maximum of skull fra

[Nerve dysfunctions following mandibular surgery on an outpatient basis].

The extent and reversibility of any functional impairment of the inferior alveolar and lingual nerves are important criteria for establishing the indication for microsurgical reconstruction. Postoperative dysfunctions were observed by 31.4% of 384 patients operated in the posterior segment of the lower jaw. In more than half of these cases the dysfunctions disappeared after 8 weeks. After 6 months the remission tendency was very slight. After a mean 4 years following operation 6% of the patients reported subjective disorders. Impaired functions were demonstrated objectively in 11.4% of the cas

[Can inferior alveolar nerve lesions be avoided during sagittal splitting? Anatomy, surgical technique and neurological aspects].

The overriding principle in surgery should always be "nihil nocere". This is particularly true for maxillofacial surgery and assumes critical importance in orthognathic operations in which the prevention of motor and sensory nerve lesions should have priority. Considering that orthodontic surgery is increasingly done in an ever growing number of situations, it is obvious that this issue has become central to the decision-making process in each and every case. This contribution is designed to review the basic anatomy, to outline a useful surgical management concept and present the results obtai

[Anatomy of the atrophic mandible. 1. The location of the mandibular canal in the atrophic mandible].

43 atrophic left hemimandibles were divided by 6 saw cuts made between the mental foramen and the third molar to analyze the location of the mandibular canal. Mandibles were classified by the severity of alveolar absorption using Atwood's classification and changes in the distances of the mandibular canal from the superior and inferior borders of the mandibular body were correlated wit the degree of atrophy. Measurements were obtained with a digitizer and a computer and results were analyzed statistically. The following statistically significant observations were made: The distance of the mand

[Interdisciplinary investigations on the criticism of the term 'anesthesia dolorosa'].

3291 patients with facial pain and/or complaints in the orofacial region were examined from 1972-1988 by both the maxillofacial surgeon and the neurologist at the departments of maxillofacial surgery and psychosomatic medicine of the Zentrum für Zahn-, Mund- und Kieferkrankheiten of the University of Münster. Only 27 cases of genuine trigeminal neuralgia could be found in those 17 years. At the same time, however, 31 patients presented with anaesthesia dolorosa after various operations on the fifth cranial nerve. Referring to the criteria set up by Marxkors and Müller-Fahlbusch (1981) these fi
