A case of recurrent early gastric cancer with a giant injection-related granuloma after endoscopic submucosal dissection resulting in non-curative resection.

A 69-year-old man underwent endoscopic submucosal dissection (ESD) for early gastric cancer of the lower gastric body. The lateral margin was positive, corresponding to an eCura C-1 classification, and surveillance with upper gastrointestinal endoscopy every 6 months was performed. Local recurrence was detected 4 years later, and repeat ESD resulted in an eCura C-2 classification, prompting additional surgical resection. No lymph node metastasis was identified, and the patient remained recurrence-free for 11 months. Histopathological examination of the surgical specimen revealed extensive fore

Accidental Bottle Cap Ingestion in an Adult in a Resource-Limited Setting: A Case Report.

Foreign body ingestion in adults may become life-threatening depending on the site of impaction and the nature of the object. We report a 53-year-old man who accidentally ingested a bottle top while opening a pressurized reused beverage bottle with his teeth. He presented with drooling of saliva, dysphagia, throat pain, and foreign body sensation. Plain radiographs failed to identify the radiolucent foreign body but excluded airway involvement. Direct laryngoscopy identified the foreign body impacted at the cricopharyngeal opening, and successful removal was achieved using Magill forceps under

Oropharyngeal leech infestation as an unusual cause of severe anemia and hypovolemic shock in a 2-year-old child: A case report from Ethiopia.

BACKGROUND: Leech infestations can cause significant, often unrecognized, blood loss. This type of infestation occurs most commonly in young children living in rural settings who consume unfiltered water from rivers. CASE PRESENTATION: A two-year-old female patient presented to a general hospital from a rural area of Ethiopia with a five-day history of epistaxis, blood-stained saliva, fatigue, and poor appetite. On examination, hemoglobin was 3 g/dL. A dark brown, mobile foreign body - subsequently identified as a leech - was found attached to the posterior oropharynx. The patient (weight 11

Unity Brings Mutual Benefits: The Huddling Strategies of Temperate-Living Primates in Cold Environments.

Behavioral thermoregulation is a key adaptive tactic shaped by natural selection and species evolution, enabling animals to cope with variable thermal environments, particularly facing cold challenges. Quantifying the thermal and energetic consequences of behavior in the wild provides key insights into how animals cope with environmental challenges. Our study examines huddling, a typical social thermoregulatory behavior in the golden snub-nosed monkey (Rhinopithecus roxellana), the northernmost distributed colobine primate, using behavioral observations and infrared thermography to assess its

Tribochemical Degradation at the Implant-Abutment Interface and Its Implications for Marginal Bone Loss and Peri-Implant Mucositis: A Narrative Review.

The implant-abutment interface is the only non-osseointegrated junction of an implant-supported restoration and is continuously exposed to cyclic mechanical loading. Micromovement and fretting at this junction repeatedly disrupt the protective titanium dioxide film that confers corrosion resistance on titanium, so that mechanical wear and electrochemical corrosion act together in a process termed tribocorrosion. The resulting release of titanium ions and particulate debris into the peri-implant environment has been proposed as a contributor to peri-implant inflammation and marginal bone loss,

A Loose Screw: An Open Surgical Approach for Removing Large Laryngotracheal Foreign Bodies.

Unusual airway foreign bodies have long interested airway specialists, and in the present case, the unusual feature is not the type of foreign body but its size. We describe the surgical management of a large, high-risk airway foreign body in a pediatric patient when an endoscopic approach is unsuccessful. A 22-month-old male aspirated a 3.5-inch drywall screw. The size of the screw in the airway of a patient of this age prompted a multidisciplinary approach to surgical removal, and we detail the preoperative, operative, and postoperative care of this patient, focusing on the surgical techniqu

Self-inflicted thoracoabdominal nail-gun injury presenting as STEMI: diagnostic challenges in a time-critical care pathway.

A 60-year-old male with a history of depression and a prior nail-gun thoracic injury was found in shock at home, with inferior ST-segment elevation on the paramedic electrocardiogram prompting pre-hospital ST-elevation myocardial infarction pathway activation. Initial high-sensitivity troponin was elevated, consistent with the presumed diagnosis. Normal coronary angiography and preserved ventricular function were discordant with the patient's ongoing haemodynamic instability. Four small anterior chest puncture wounds and radiopaque foreign bodies demonstrated on chest radiograph and angiograph

Bone missile injury with an intact host femur after high energy road traffic trauma: a case report.

A 28 year old woman presented with local wound infection two weeks after sustaining an anterior thigh laceration in a high energy motorbike crash that killed the co-riding motorcyclist. X-ray revealed a non-identifiable radio-opaque fragment and computed tomography identified a bony foreign body deep in the thigh adjacent to the femoral vessels, with the host femur intact. A 6 x 3 cm fragment of cortical bone was removed at surgery. Histopathology and genetic studies to confirm its origin were not feasible in this resource-limited setting, but an intact host femur suggested it originated from

Tirzepatide-induced pneumatosis intestinalis: a rare but clinically significant gastrointestinal adverse effect.

OBJECTIVES: Glucagon-like peptide 1 receptor agonists such as tirzepatide are increasingly prescribed to treat type 2 diabetes mellitus and obesity because they improve glycemic control and promote weight loss. Although gastrointestinal adverse effects such as nausea, vomiting, constipation, and delayed gastric emptying are well recognized, very rare cases of pneumatosis intestinalis have also been reported in association with this class of medications. We report the first documented histologic diagnosis of a possible tirzepatide-associated pneumatosis intestinalis. METHODS: A 55-year-old man

Posterior Mediastinal Biopsy Gone Awry: Managing Biopsy Needle-Induced Intubation Challenges in Emergency Surgery.

Foreign bodies entering the thorax via a posterior trajectory present distinct challenges in positioning and intubation, particularly during emergency exploration. We present the case of a 37-year-old female who sustained a subclavian artery perforation during a computed tomography-guided biopsy for a posterior mediastinal mass. The biopsy needle advanced to the left common carotid artery. She was brought in the prone position for emergency surgical exploration, which posed significant challenges for airway management. This case highlights the importance of flexible airway strategies, multidis

Optimizing pediatric radiology workflow amidst workforce shortages: A single-center experience reducing off-hours fluoroscopy.

Pediatric radiology is facing an escalating workforce crisis, fueled primarily by a constrained fellowship pipeline and rising imaging volumes. This imbalance has created difficulty providing and maintaining off-hours fluoroscopic services, which require on-site radiologist presence. The resulting burden on the existing labor force exacerbates burnout, challenges radiologist retention and recruitment, and risks undermining the long-term sustainability of pediatric radiology practice. There is a shared imperative to seek alternative, creative strategies to alleviate these issues while achieving

Asia induced by hyaluronic acid, tattoos, hyaluronidase and polypropylene mesh.

The growing use of cosmetic fillers, tattoos, and surgical meshes has brought increasing attention to their potential inflammatory and immunologic complications. Autoimmune/inflammatory syndrome induced by adjuvants (ASIA) proposes a framework for systemic manifestations reported after exposure to these materials in susceptible individuals. Although hyaluronic acid is a natural component of the extracellular matrix with low immunogenicity and high biocompatibility, Hyaluronic acid fillers (HAF) have been associated with delayed inflammatory and granulomatous foreign-body reactions, and ASIA. Extracting the Unexpected: A 35-mm Curved Nail as an Intraocular Foreign Body.

BACKGROUND Management of posterior segment intraocular foreign bodies is challenging, requiring the preservation of ocular integrity and restoring vision. Traumatic endophthalmitis occurs in 7% to 13% of penetrating injuries, particularly with coexisting intraocular foreign bodies. Managing posterior segment intraocular foreign bodies poses significant challenges due to their complex clinical presentation and the need to achieve multiple goals during treatment. This unusual case of a large metallic intraocular foreign body associated with traumatic cataract, retinal detachment, and endophthalm

Breast cancer metastasis mimicking pacemaker pocket infection: a diagnostic pitfall and long-term preservation of the cardiac implantable electronic device-a case report.

BACKGROUND: Cardiac implantable electronic device (CIED) pocket infection is the most common infectious complication following pacemaker implantation or generator replacement and generally requires complete device and lead extraction according to current international guidelines. Nevertheless, non-infectious disorders, including malignant disease, may closely mimic pocket infection, creating a challenging differential diagnosis and potentially leading to delayed diagnosis or unnecessary CIED extraction. Neoplastic involvement of the CIED pocket is exceptionally rare. CASE SUMMARY: An 81-year-

Contained transverse colonic perforation caused by an ingested fish bone presenting as chronic abdominal pain: a case report.

INTRODUCTION AND IMPORTANCE: Foreign body ingestion is common; however, gastrointestinal perforation is rare and often poses a diagnostic challenge, particularly when symptoms are chronic and nonspecific. Colonic perforation due to ingested foreign bodies is uncommon, with the transverse colon being an especially rare site due to its mobility and wide lumen. CASE PRESENTATION: We report the case of a 19-year-old male who presented with central abdominal pain and intermittent diarrhea for 1 month, followed by acute worsening of abdominal pain, fever, nausea, and vomiting over the proceeding 3

Intravascular bullet embolization: a multicenter three-case series and literature review.

INTRODUCTION AND IMPORTANCE: Intravascular bullet embolization (IBE) is a rare but potentially life-threatening complication of penetrating trauma. It may present as either arterial or venous embolization and is most commonly associated with low-velocity, small-caliber gunshot wounds (GSWs). Diagnosis is often challenging, and management remains controversial because of the limited evidence available to guide therapeutic decision-making. CASE PRESENTATION: We present a multicenter case series of three patients with venous bullet embolization treated using a hybrid endovascular-surgical approa

Use of sole-agent ketamine sedation to facilitate laryngoscopy and foreign body removal in acute supraglottic airway obstruction.

A 78-year-old man presented with stridor and dysphonia after choking at home. He deteriorated, with respiratory distress and an oxygen saturation of 55% despite oxygen via a non-rebreather mask. Flexible nasendoscopy demonstrated a food bolus between the epiglottis and glottis. The team judged apnoea and assisted positive-pressure ventilation before extraction to be hazardous, while deterioration and limited cooperation made an awake technique impracticable. Sole-agent ketamine sedation facilitated videolaryngoscopy with clinically observed spontaneous breathing; the bolus, a slice of carrot,

Chronic Neuropathy of the Lateral Antebrachial Cutaneous Nerve after Endoscopic Radial Artery Harvest: A Case Report.

INTRODUCTION: Neurologic complications after radial artery harvest for coronary artery bypass grafting (CABG) remain incompletely characterized, particularly after endoscopic radial artery harvest (ERAH). As the radial artery has become the preferred second arterial conduit due to its superior long-term outcomes, ERAH has gained popularity as a minimally invasive alternative to open harvest with favorable cosmetic and wound outcomes. Although prior studies suggest neurologic symptoms are often transient, persistent sensory deficits may be underrecognized. The lateral antebrachial cutaneous ner
