Commercially prepared dilutions (1:10,000) of epinephrine can cause marked increases in corneal thickness and loss of corneal endothelial cells when injected into the anterior chamber of the eye. Endothelial toxicity is related to the buffer capacity of the epinephrine solution, which is in turn controlled by the concentration of the antioxidant (sodium bisulfite) as well as by the vehicle formulation and a low pH value.

The corneal endothelial damage in extracapsular extraction and insertion of a two-loop lens (Binkhorst) using the deep chamber technique was investigated by specular microscopy. The endothelial cell loss in 25 patients was 15%. During a mean postoperative period of 15 months a further endothelial cell loss of 5% occurred. The effect of a direct contact between an intraocular lens and the corneal endothelium is demonstrated by a case of luxation of a two-loop lens into the anterior chamber.

In 20 patients, corneal endothelium was examined by specular microscopy after acute angle-closure glaucoma, and before any surgical treatment in order to assess possible damage during pressure induced abnormal hydration of the cornea. The mean intraocular pressure was elevated to 55 mm Hg and had lasted, on an average, 47 (5--192) hours. The mean central endothelial cell loss arising with the availability of Sodium hyaluronate (Healonid), from insertion of a Boberg Ans implant was 14 per cent following extracapsular cataract extraction and 16 per cent following intracapsular surgery. These figures compare favourably with those described by other authors following simple intracapsular extraction without lens implantation and the closed chamber techniques demonstrated should be regarded as safe in so far as they virtually eliminate the incidence of late corneal dystrophy as a complication of lens implantation.

Comparison of endothelial cell density by specular photomicroscopy of the right and left eyes after single eye surgery shows a similar reduction in endothelial cell density to that recorded between pre- and postoperative photographs in the operated eye. A review of 43 single-eye cataract extractions, with and without placement of a prepupillary intraocular lens, confirms that the major cause of endothelial cell loss is operative trauma to the endothelium. This case report deals with a 68-year-old man in whom a contusion resulted in a complete loss of the iris through an Elliot fistula. The abrupt increase in intraocular pressure caused the iris to be extruded through the fistula and the ruptured conjunctiva. A functional drainage bleb was reconstructed by pulling and suturing subconjunctival tissue underneath the ruptured conjunctiva.

Extracapsular cataract extraction may result in a lower incidence of cystoid macular edema, but it may be a different operation from a technical standpoint. We tried a method using 1% hyaluronate sodium (Healon) as the irrigating vehicle during the procedure. In implant surgery there is more damage to the endothelium than in normal cataract extraction. This damage results from the implant coming into direct contact with the endothelium during surgery. To minimize this contact, hyaluronic acid was instilled into the anterior chamber between the implant and the endothelium. The effect of this procedure post-operative endothelial cell density was investigated in a clinical study. The result was found to be a decrease in endothelial cell loss.

Intraocular pressure was artificially elevated above systolic blood pressure in 40 owl monkey eyes. Diffuse retinal whitening of the posterior pole and subsequent retinal pigment epithelial disruption were produced in nine of ten eyes with high pressure for 90 minutes or longer, but also in a few eyes with high pressure for a shorter duration. The use of sodium hyaluronate (Healon) presents a major new advance in facilitating lens implantation and improving safety margins during anterior segment surgery. Of 102 consecutive cases of extracapsular cataract extraction with lens implantation, 54 with the use of Healon, it was found that Healon could induce a reduction of central corneal endothelial cell loss from 29 to 17 per cent and that the only associated complication was a one-day rise of intraocular pressure.

Lens implant surgery has a significant complication rate, although most complications are treatable and occur within the first 2 months after operation. The short-term visual results are comparable with the published results of intracapsular extraction alone. Six cases of bullous keratopathy developed in 138 cases in 7 1/2 years. When endothelial cell density fell below 500/mm2, corneal oedema developed within the year. However, a more recent study showed that endothelial cell loss was much reduced by the closed chamber technique of insertion and by serum coating of the implant.

Specular microscopy performed six months postoperatively on 122 lens implant patients showed that endothelial cell loss rates varied according to method of cataract extraction and type of lens implant. Phacoemulsification with implantation of a Platina-style lens was associated with the highest cell loss; planned extracapsular cataract extraction with implantation of a Choyce-style or J-loop lens was associated with the lowest cell loss. The presence of marked striate keratopathy on the first postoperative day was directly related to endothelial cell loss.

Eighteen rhesus monkeys underwent lens implantation with Choyce Mark VIII, Binkhorst iridocapsular, or Shearing posterior chamber lenses. They were sacrificed 4-28 months postoperative. The eyes were compared clinically and histologically. Controls included unoperated eyes and eyes with lens extraction without implantation. Intraocular injection of kainic acid caused a marked decrease in GABA content in the rat retina and the almost complete loss of GAD activity in this tissue, within 3 days. Moreover, kainic acid produced a decrease of approximately 65% in the number of both [3H]diazepam and [3H]GABA binding sites without changing the apparent dissociation constants for their ligands. In contrast to that in brain areas, cyclic GMP content in the retina is neither influenced by kainic acid nor by the systemic administration of diazepam and muscimol.
