Dry coating of intraocular lenses with bovine submaxillary mucin.

Uncoated intraocular lenses caused extensive corneal endothelial cell damage after in vitro contact (mean, 30.9% cell loss), whereas lenses coated with bovine submaxillary mucin type 1 caused minimal cell loss (mean 4.3%). No observable in vivo toxicity in the eye occurred when 0.1 ml of 3, 6, or 9% solution was injected into the anterior chamber. Mucin-coated lenses can be sterilized by ethylene oxide with no measurable adsorbence into the coating. Scanning electron microscopy confirmed the results from vital staining.

Endothelial cell morphology and corneal deturgescence.

In a series of 46 patients accepted for cataract removal and intraocular lens implantation, 23 showed visibly obvious variation in endothelial cell size (polymegethism) according to specular microscopic examination. In this group, mean increase in postoperative corneal thickness was 52.7% on the first postoperative day, 35.4% on the third, and 18.0% after 4 weeks. Mean corneal swelling was consistently and significantly less in the 23 patients exhibiting apparently normal endothelial cell patterns (homomegethism). In homomegethous corneas, preoperative cell density was significantly correlated

Clinical specular microscopy.

Clinical specular microscopy (CSM) has recently been introduced as a means of qualitative and quantitative examination of the human corneal endothelium at high magnification. With the aid of CSM, a decline in endothelial cell density with age has been documented and several endothelial abnormalities from disease or trauma can be detected. Donor material for corneal grafting can be examined by CSM and keratoplasty procedures can be designed to decrease endothelial damage. Cataract surgical procedures can cause endothelial cell loss. According to most studies, intracapsular extraction causes les

Time analysis of corneal endothelial cell density after cataract extraction.

Serial endothelial photographs were taken preoperatively and postoperatively in 200 eyes; 111 eyes contained a Rayner iris clip lens, 54 eyes contained a Fyodorov Sputnik lens, and 35 eyes had no lens. Central endothelial cell density was changed in all instances, with counts in implanted eyes declining 25 to 30%, and in nonimplanted eyes 10 to 15%. In both instances, the decline essentially ceased at about three months. The cause of the greater decline in implanted eyes appeared to be mechanical and subsequent cell loss after the 90-day period was virtually equal for the two groups. Methods t

300 primary anterior chamber lens implantations: gonioscopic findings and specular microscopy.

A total of 300 consecutive quadruped anterior chamber intraocular lenses were implanted. Gonioscopy was performed in all cases and prospective specular microscopy was performed in the last 97 cases. Iris tucking of at least one lens foot was noted in 28% of the cases. Tucking occurred less often (20% of the cases) when the lens length was 13 mm, increasing with lens lengths longer and shorter than 13 mm. Slight lens rotation was noted in 8% of the cases. Blood vessels (rarely appearing to be neovascular) were noted around an implant foot or in the anterior chamber angle near a lens foot in 30%

Use of air to decrease endothelial cell loss during intraocular lens implantation.

We performed 26 consecutive intracapsular cataract extractions with lens implantation. The last 17 implantations were performed with air instilled in the anterior chamber, whereas the first nine operations were performed without air. The central corneal endothelium of each eye operated on was photographed before and within five days after lens implantation. A computer analysis of the endothelial photographs showed that, on the average, 32% of the central corneal endothelial cells were lost during lens implantation without the use of air and 15% were lost when the lens was implanted with air. Central corneal endothelial cell counts in children.

Our data indicate that corneal endothelial cell density in a normal eye remains essentially unchanged from ages five to 20. Changes in cell density and morphology during the first five years of life need further investigation. In this study, the mean endothelial cell count of a normal population of children was 2696 cells/mm2. Children undergoing cataract aspiration had a mean cell loss from the normal of 7.2%. The cell loss rates for push-pull aspiration-irrigation or phacoemulsification cataract extraction were not statistically different. The implantation of an intraocular lens was associat

Corneal decompensation in Chandler's syndrome. A scanning and transmission electron microscopic study.

A corneal button obtained from a 58-year-old woman with Chandler's syndrome was studied by light and electron microscopy. Clinically, the patient had mild unilateral glaucoma, slight changes in the iris, endothelial dystrophy, and corneal edema that was increasingly sensitive to intraocular pressure. Corneal pathology included loss of large numbers of endothelial cells with consequent exposure of Descemet's membrane. Endothelial cells demonstrated irregular shape, partial loss of tight junctions, and increased numbers of cytoplasmic filaments. Some endothelial cells appeared metaplastic or per

Axonal transport and metabolism of [3H]fucose- and [35S]-sulfate-labeled macromolecules in the rat visual system.

The axonal transport of labeled macromolecules in retinal ganglion cells of rats was investigated from 1 to 20 days following intraocular injection of [3H]fucose and [35S]sulfate. Maximal incorporation of [3H]fucose into acid insoluble material in the retina was at 8 h, followed by a biphasic decline. Transported [3H]fucose (98% as glycoprotein) was in the optic nerve at 1 h, the optic tract and lateral geniculate body by 2 h, and the superior colliculus by 3 h after injection, indicating a rate of transport of approximately 200 mm/day. Radioactivity continued to accumulate in the superior col

Endothelial cells in the glaucomato-cyclitic crisis.

In 21 patients with unilateral glaucomatocyclitic crisis (Posner-Schlossman syndrome) the central corneal endothelium was photographed with a clinical specular microscope. In each patient the endothelial cell density was determined in both the affected and the contralateral healthy eye. In patients who had had several attacks of P-S the endothelial cell density was lower in the affected eye than in the healthy eye (16 cases). 4 patients seen after the first attack and 1 of 3 patients after the second attack had the same cell density in both eyes. Thus, loss of endothelial cells seems to be due

A one-year follow-up of cystoid macular edema following intraocular lens implantation.

Retrospective and prospective techniques were used to analyze a consecutive series of 406 eyes to identify factors contributing to cystoid macular edema following lens implantation. Vitreous loss and rupture of the posterior capsule were highly significant in the production and persistence of this complication.

Retinal damage produced by intraocular fiber optic light.

We exposed the maculas of owl monkey eyes to light from an intraocular fiber optic light source similar to that used for human pars plana vitrectomy. Retinal irradiance was calculated at 0.22 W/cm2. Eyes were exposed for time intervals ranging from 30 minutes to five minutes and were observed after light treatment by fundus photography and fluorescein angiography. Tissue was obtained for light and electron microscopy by animal killing at one hour, 24 hours, one week, and four weeks. Fundus lesions were seen ophthalmoscopically as early as five hours following 30 minutes of light exposure. Sign

Endothelial cell loss in intraocular lens placement.

Endothelial cell counts were done on eyes selected to fall into four categories: unoperated, intracapsular cataract extraction without implantation, and intracapsular cataract extraction with easy or difficult insertion of an intraocular implant. Uncomplicated cataract extraction resulted in less than 9% cell loss. Implantation in "easy" eyes resulted in 12% cell loss (p greater than 0.3). Eyes in which implantation was difficult suffered an average loss of 65% of the endothelial cells (p less than .001). The reasons for cell loss are discussed and some improvements in techniques are mentioned

Endothelial damage with phacoemulsification and intraocular lens implantation.

Specular microscopy was used to study the corneal endothelium in a series of patients undergoing phacoemulsification with and without intraocular lens implantation by one experienced surgeon. Cell loss after phacoemulsification alone was variable, but the mean was 18%, with a median of 11%. The patients undergoing phacoemulsification plus lens implantation showed a mean cell loss of 26%, with a median of 17%. This is considerably lower than has been reported with standard intracapsular extraction plus lens implantation. The decreased endothelial damage is attributed to the technique of inserti

Endothelial cell loss and trauma during intraocular lens implantation: a specular microscopic study.

We analyzed the results of a retrospective specular microscopic study of 80 cases with unilateral medallion suture implants and contralateral unoperated eyes. In all cases a clinical estimation of operative endothelial trauma (ET), graded from 0 to 3+, was made at the time of surgery. The mean decrease in endothelial cell density (compared to the unoperated eye) for those 33 patients with ET 0 was 19%. There was a direct correlation between ET and decrease in endothelial cell density up to a mean of 59% for those 18 patients graded ET 2.5 to 3.0. The role of sample bias and small case sampling

Corneal effects of intraocular air.

To resolve the apparent contradiction between Norn's work on human eyes suggesting that postoperative air protects the corneal endothelium and the work of Leibowitz on excised animal eyes suggesting that air damages the corneal endothelium, we have replicated Norn's findings in an animal model similar to that used by Leibowitz. Since fewer metabolically active cells may maintain corneal deturgesence even after cell loss we do not believe that Norn's work contradicts that of Leibowitz. We find that the eyes treated with air have significantly less corneal swelling.

Morphological appearance of the healing corneal endothelium.

We studied endothelial cell patterns in both the central and the superior regions of the cornea in 43 cases of cataract extraction and intraocular lens implantation. During the early postoperative period, the central endothelial cells maintained their preoperative appearance. The endothelium within 3 to 4 mm of the superior limbus, however, demonstrated large dark areas where no cell outlines were visible, bounded by large cells. During the late postoperative period, there was an increase in the size and a reduction in the density of the central endothelial cells. In the periphery the previous

Effects of intraocular miotics on cultured bovine corneal endothelium.

Two cases of severe corneal oedema occurred after the use of intraocular pilocarpine. Experimental investigations were conducted with cultured bovine corneal endothelial cells exposed for 5 minutes to 1% pilocarpine solutions of varying composition. Cells were destroyed in solutions not isotonic with aqueous humour, and calcium-free ionic solutions caused loss of cell adhesion without loss of viability. Low pH or the presence of 1% pilocarpine had no detectable effects; 1% acetylcholine chloride in 5% mannitol (Miochol) also caused cell destruction, and this preparation was found to be conside

Biomicroscopical observations on corneal endothelium in pseudophakia.

The principle and technique of Maurice's method of microscopy of the corneal endothelium at high magnificant is discussed. An instrument is now available for clinical use (clinical specular microscope). The experience of the authors in patients with an intraocular lens is reported. A rather high deficit of endothelial cells was found in a series of 26 children and young adults, that had suffered injury from 2 to 11 years previously. An average immediate endothelial cell loss of only 6-7% was found in extracapsular cataract extraction with simultaneous implantation of a 2-loop or 4-loop intraoc
