PURPOSE: Descemet membrane endothelial keratoplasty (DMEK) is the preferred treatment for corneal endothelial dysfunction; nonetheless, its feasibility and outcomes in vitrectomized eyes remain uncertain. We evaluated the outcomes and surgical considerations of endothelium-in pull-through DMEK in vitrectomized eyes with bullous keratopathy.
METHODS: Eight vitrectomized eyes from eight patients (mean age, 71.9 ± 15.4 years; mean follow-up, 4.9 ± 1.6 months) underwent endothelium-in pull-through DMEK using the EndoGlide between August 2023 and October 2024. Best spectacle-corrected visual acuit

A 58-year-old man presented with pain, redness, watering, and sudden diminution of vision in the right eye following blunt trauma from a stone. Slit-lamp examination revealed mild corneal edema, a central corneal defect, a traumatic cataract with probable anterior lens subluxation, and a distinct air bubble within the anterior chamber. Seidel's test was negative, and no iris prolapse or obvious corneal laceration was identified. The presence of intracameral air raised a strong suspicion of prior communication between the anterior chamber and the external environment, suggestive of a self-seale

BACKGROUND: Orbital infarction syndrome (OrbIS) is a rare but devastating complication following mechanical thrombectomy for ischemic stroke, characterized by a near-global ischemia of intraocular and orbital tissues. It can mimic a more benign secondary orbital inflammation (SOIN), leading to diagnostic uncertainty. We present the largest case series of seven patients who developed acute periorbital swelling following endovascular thrombectomy (EVT) for anterior circulation large vessel occlusion with clinically diagnosed OrbIS or SOIN.
METHODS: A retrospective review of our EVT database was

BACKGROUND: Idiopathic dacryoadenitis is a diagnosis of exclusion and may present with unilateral or bilateral eyelid swelling, erythema, pain, and characteristic S-shaped ptosis of the upper eyelid. Glucocorticoids remain the mainstay of treatment when symptoms are functionally limiting or clinically significant. Although local triamcinolone acetonide (TA) injection provides targeted anti-inflammatory effects with fewer systemic adverse effects, it carries a rare but potentially catastrophic risk of iatrogenic vascular embolization and acute vision loss.
METHODS: We searched the PubMed datab

Ocular infections can affect the ocular surface and adnexal structures, intraocular structures, or both. Infections of the ocular surface and adnexa often arise from the invasion of commensals when typical barriers are compromised, which includes infectious blepharitis, conjunctivitis, keratitis, dacryocystitis, and orbital infections, among others. Intraocular infections may develop due to localised trauma or as part of a systemic condition. A variety of viruses, bacteria, fungi, protozoa, and helminths can cause intraocular illness. Occasionally, ocular abnormalities are the only external cl

Cytomegalovirus (CMV) infection of the anterior segment is increasingly recognized as an important cause of corneal endotheliitis and secondary glaucoma, even in immunocompetent individuals. CMV corneal endotheliitis typically presents with coin-shaped or linear keratic precipitates (KPs), corneal edema, mild anterior chamber inflammation, and recurrent intraocular pressure (IOP) elevation; persistent or episodic ocular hypertension may progress to glaucomatous optic neuropathy if inadequately treated. Definitive diagnosis relies on aqueous humor polymerase chain reaction (PCR) testing for CMV

PURPOSE: Corneal perforations necessitate prompt surgical intervention to restore globe integrity and mitigate the risk of irreversible vision loss. Larger corneal perforations often require more complex surgical strategies, including tectonic patch grafting or penetrating keratoplasty. However, limited availability of donor tissue presents significant challenges in meeting urgency of these cases. We describe an ab interno technique approach for corneal perforation repair, using off-the-shelf human cornea as an alternative viable surgical treatment.
METHODS: The surgery was performed at a ter

INTRODUCTION: The aim of the study was to describe a rare presentation of acute retinal necrosis (ARN) in an immunocompetent adult, with choroidal, optic nerve, and orbital inflammation with exudative retinal detachment (ERD), complicated by early rhegmatogenous retinal detachment (RRD). This is a retrospective case report of atypical ARN. Clinical data, laboratory testing, and multimodal imaging were reviewed.
CASE PRESENTATION: Examination revealed right eye granulomatous panuveitis, including granulomatous anterior chamber inflammation, dense vitreous haze, and peripheral necrotizing hemor

Background: Hypervirulent Klebsiella pneumoniae (K. pneumoniae) easily causes bacteremia and liver abscess, and invades the eye via blood circulation to trigger blinding endogenous endophthalmitis. Widespread multidrug resistance limits antibiotic treatment, while traditional capsular polysaccharide vaccines cannot provide cross-serotype protection. Methods: We established a mouse model of intraperitoneal infection-induced endogenous endophthalmitis. BALB/c mice received two intramuscular injections of LNP-encapsulated PstS-YidR fusion mRNA vaccine, followed by challenge with hypervirulent K1

Disseminated nocardiosis with endogenous endophthalmitis is a rare but life-threatening condition associated with significant morbidity and risk of permanent vision loss. We report a fatal case of bilateral chorioretinitis with unilateral endogenous endophthalmitis accompanied by soft tissue and central nervous system involvement caused by Nocardia farcinica. An 86-year-old woman presented with a five-day history of a progressively worsening blind spot in the left eye, and ophthalmic examination revealed bilateral chorioretinal lesions with marked anterior chamber and vitreous inflammation in

Mycobacterium tuberculosis primarily causes pulmonary diseases; however, it can also affect extrapulmonary organs, including the eyes. The choroid, as the intraocular tissue with the richest blood supply, is one of the most susceptible structures to intraocular tuberculosis. Abnormal ocular symptoms that occur during systemic anti-tuberculosis treatment (ATT) can easily be misdiagnosed as drug side effects or treatment failure. Abruptly discontinuing a specific medication could lead to the recurrence of extraocular tuberculosis and irreversible vision loss. This case study focuses on a young f

Ocular histoplasmosis typically presents as presumed ocular histoplasmosis syndrome, characterised by chorioretinal scars and peripapillary atrophy without intraocular inflammation. Active inflammatory presentations are exceedingly rare and may closely mimic tuberculosis in endemic regions, posing significant diagnostic challenges. We report a middle-aged immunocompetent male farmer who presented with acute bilateral visual loss, redness and floaters. Examination revealed bilateral granulomatous anterior uveitis with dense vitritis with mutton-fat keratic precipitates, dense vitritis and 4+ ant

Bacterial endophthalmitis, an intraocular infection and inflammation, often progresses rapidly and leads to irreversible vision loss, especially in culture-negative cases where the diagnosis is delayed. We profiled infection-associated metabolomic and lipidomic alterations in the vitreous of patients with microbiological and clinically confirmed bacterial endophthalmitis to understand pathogenesis and identify distinct markers that drive infection and retinal injury. Untargeted metabolomic and lipidomic profiling of vitreous samples from affected patients and non-infectious retinal controls wa

Morphological and biophysical techniques described in this study have shown that membrane derangement occurs in human cataractous lenses. The data suggest that these disruptions were globules, vacuoles, multilamellar membranes and clusters of highly undulating membranes. Deleterious structural damage of the lens fibre cell plasma membranes serve as the primary light-scattering centres that cause the observed lens opacity. Nuclear cataract, a major cause of loss of lens transparency in the aging human, has been thought to be associated with oxidative damage, particularly at the site of the nucl

Inherited genetic disorders are progressive in nature and lead to organ dysfunction or death in severe cases. At present, there are no permanent treatment options for >95% of inherited disorders. Different modes of inheritance, type of gene(s) involved, and population-based variations add further complexity to finding suitable cures for approximately 400 million patients worldwide. Gene therapy is a very promising molecular technique for the treatment of rare genetic disorders. Gene therapy functions on the basis of restoration, replacement, inhibition, and, most recently, editing of gene(s) t

BACKGROUND: Mirtazapine has a unique mechanism of antidepressant action, and thus is thought to have a different profile of adverse events from that of other antidepressants.
OBJECTIVE: To present a methodologically rigorous systematic review of the adverse event profile of mirtazapine and point to possible problems with safety reporting in randomized controlled trials (RCTs) of the acute-phase treatment of major depression in adults with mirtazapine in comparison with other types of antidepressant.
METHODS: The Cochrane Collaboration Depression, Anxiety and Neurosis Controlled Trials Regist

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Elevated intraocular pressure (IOP) is a major risk factor for the development and progression of glaucoma, the leading cause of irreversible vision loss and blindness. An overall increase in resistance to aqueous humor outflow causes sustained elevation in IOP. Glaucomatous insults in the aqueous humor outflow pathway, including the trabecular meshwork (TM), precede such chronic physiological changes in IOP. These insults include ultrastructural changes with excessive extracellular matrix deposition and actin cytoskeletal reorganization that leads to pathological stiffening of the ocular tiss

PURPOSE: Vascular impairments, including reduced capillary density (CD), impaired autoregulation capacity (Reg), and elevated intraocular pressure (IOP), have been identified as significant contributors to glaucomatous disease. This study implemented a theoretical model to quantify the impact of these impairments on retinal blood flow and oxygenation as intraluminal pressure (Pa) is varied.
METHODS: A theoretical model of the retinal vasculature was used to simulate reductions in CD by 10% (early glaucoma) and 30% to 50% (advanced glaucoma), a range in autoregulation capacity from 0% (totally
