One-year longitudinal study of corneal changes after different vitrectomy procedures using confocal microscopy.

This study aimed to evaluate corneal stromal and endothelial alterations after vitrectomy using in vivo confocal microscopy (IVCM), focusing on the implications of different surgical techniques and tamponades. In this prospective cohort study, a total of 99 eyes from 98 consecutive patients scheduled for vitrectomy were initially enrolled. Each subject underwent a comprehensive ophthalmic evaluation, including fundus examination, intraocular pressure assessment, and IVCM imaging. These evaluations were conducted preoperatively, as well as at 1, 3, 6, 9, and 12 months following surgery. Data we

Loss of trabecular meshwork stem cells is correlated with open angle glaucoma.

The trabecular meshwork (TM) of the eye is critical in maintaining aqueous humor outflow and intraocular pressure (IOP). The cellular density in the TM decreases with age and is particularly low in eyes with glaucoma. TM cells are thought to be derived from a population of stem cells, referred to as TM stem cells (TMSCs). To investigate the relationship between TM cellular density and TMSCs, the number of TMSCs and TM cells was compared in human eyes obtained from young donors, individuals with glaucoma, and age-matched controls. Findings obtained confirm that eyes of younger donors contain th

Tolerability and Efficacy of Topical Methotrexate in Ocular Surface Disease.

INTRODUCTION: Topical immunomodulators have gained popularity in the treatment of inflammatory dry eye disease (DED). In our tertiary DED population, ocular surface inflammatory signs and symptoms refractory to conventional treatments remain a diagnostic and therapeutic challenge. This two-part in vitro toxicity and in vivo clinical study aims to determine the safety and therapeutic potential of off-label use of topical methotrexate (MTX) for the treatment of recalcitrant ocular surface disease (OSD). METHODS: Aim 1 involved an in vitro toxicity assay where primary human corneal epithelial/st

Lamina Cribrosa Hypoxia Sensitivity to Variations of Anatomy and Vascular Factors.

Insufficient oxygenation in the lamina cribrosa (LC) may contribute to axonal damage and glaucomatous vision loss. To understand the range of susceptibilities to glaucoma, we aimed to identify key factors influencing LC oxygenation and examine if these factors vary with anatomical differences between eyes. We reconstructed three-dimensional, eye-specific LC vessel networks from histological sections of four healthy monkey eyes. For each network, we generated 125 models varying vessel radius, oxygen consumption rate, and arteriole perfusion pressure. Using hemodynamic and oxygen supply modeling

Enhanced glaucoma classification through advanced segmentation by integrating cup-to-disc ratio and neuro-retinal rim features.

Glaucoma is a progressive eye condition caused by high intraocular fluid pressure, damaging the optic nerve, leading to gradual, irreversible vision loss, often without noticeable symptoms. Subtle signs like mild eye redness, slightly blurred vision, and eye pain may go unnoticed, earning it the nickname "silent thief of sight." Its prevalence is rising with an aging population, driven by increased life expectancy. Most computer-aided diagnosis (CAD) systems rely on the cup-to-disc ratio (CDR) for glaucoma diagnosis. This study introduces a novel approach by integrating CDR with the neuro-reti

Assessing the Role of Compression Eye Patches for Orbital Surgery: A Comparative Analysis of 10 Patients Undergoing Bilateral Orbital Decompression Surgery.

Introduction Postoperative eye patches are commonly used in orbital surgery to prevent bruising and edema, but there are only a few studies on their usefulness. We have performed bilateral orbital fat decompression as a surgical treatment for thyroid eye disease, which is very useful for relieving orbital pressure and improving functional and cosmetic outcomes. In this study, we compared the postoperative outcomes of patients with and without a postoperative compression eye patch and evaluated its effect on postoperative status based on bilateral orbital fat decompression. Visual and ultrastructural analysis after splitting of recipient's Descemet membrane during Descemet membrane endothelial keratoplasty.

BACKGROUND: During Descemetorhexis in Descemet Membrane Endothelial Keratoplasty (DMEK), inadvertent lamellar splitting of the recipient's Descemet membrane (DM) may occur, leaving DM remnants on the posterior corneal surface. This study investigates the influence of lamellar splitting on surgical outcome as well as the histological ultrastructure of split DMs. METHODS: In this prospective, observational, single-centre cohort study 129 eyes of 102 patients were divided into two groups depending on the occurrence of intraoperative splitting. If splitting of the recipient's DM occurred, the rem

Current concepts and recent trends in endothelial keratoplasty.

Endothelial keratoplasty (EK) is defined as an umbrella term comprising methods for selective surgical replacement of corneal endothelium and adjacent corneal tissue, which retains healthy portions of a patient's cornea while replacing diseased innermost corneal layer(s) with healthy donor tissue, to achieve corneal dehydration and transparency before the onset of irreversible stromal edema and permanent loss of corneal clarity. Recently, the pathophysiology of corneal decompensation is increasingly being researched upon. Consequent improvement in pharmacotherapy is progressively leading to re

Characterizing the expression profile of Dexras1 in human trabecular meshwork cells.

Corticosteroids are a mainstay therapy for the treatment of ocular and systemic inflammatory conditions but are associated with a significant risk of intraocular pressure elevation, or ocular hypertension. If intraocular pressure is inadequately controlled, steroid-induced glaucoma may develop, which can result in permanent vision loss and irreversible blindness. Pathological changes akin to fibrosis in the trabecular meshwork, the tissue responsible for intraocular pressure regulation, have been well described and contribute to the development of steroid-induced ocular hypertension and glauco

Multilayered trabecular meshwork for dynamic in vitro studies in glaucoma research.

Glaucoma, an eye disease causing incremental vision loss, currently has no cure. Its primary cause is the malfunction of the trabecular meshwork (TM), a multilayered tissue in the eye responsible for draining aqueous humor (AH) from the anterior chamber. TM clogging increases outflow resistance, elevates intraocular pressure (IOP), and damages optic nerves, leading to irreversible blindness. Existing in vitro TM models are suboptimal, as they lack the hierarchical structure of the TM. This article introduces a dynamic in vitro TM model, featuring a multilayered scaffold architecture 3D printed

Magnetically steered cell therapy for reduction of intraocular pressure as a treatment strategy for open-angle glaucoma.

Trabecular meshwork (TM) cell therapy has been proposed as a next-generation treatment for elevated intraocular pressure (IOP) in glaucoma, the most common cause of irreversible blindness. Using a magnetic cell steering technique with excellent efficiency and tissue-specific targeting, we delivered two types of cells into a mouse model of glaucoma: either human adipose-derived mesenchymal stem cells (hAMSCs) or induced pluripotent cell derivatives (iPSC-TM cells). We observed a 4.5 [3.1, 6.0] mmHg or 27% reduction in intraocular pressure (IOP) for 9 months after a single dose of only 1500 magn

Case report of spontaneous corneal clearance after subtotal graft detachment following combined Descemet's membrane endothelial keratoplasty and cataract surgery.

PURPOSE: Fuchs' Endothelial Corneal Dystrophy (FECD) is a bilateral, sporadic or autosomal dominant, non-inflammatory corneal dystrophy characterized by the progressive loss of corneal endothelial cells. While DMEK has been proven to attain better post-operative best corrected visual acuity (BCVA), it has presented the challenge of more frequent postoperative graft detachments.1,2 Combined cataract surgery along with DMEK has also been reported to increase the risk of early postoperative graft detachment.3While several case reports have illustrated spontaneous corneal clearance after significa

Transcriptomic Profiling of Small Molecule Atf6 Modulators in the Retina.

The Unfolded Protein Response (UPR) maintains endoplasmic reticulum (ER) homeostasis and is essential for retinal health. Activating Transcription Factor 6 (ATF6) controls a key UPR branch and upregulates genes that mitigate ER stress. Small molecule modulators of ATF6 have been characterized in cell culture models that increase or decrease the amount of the cleaved, transcriptional activator domain of ATF6 generated from the full-length precursor. However, the effects of these small molecule ATF6 modulators remain unclear in vivo, in part because of the lack of antibodies that robustly detect
