Multidimensional immune ecological subtyping identifies RUNX1 as a prognostic factor in uveal melanoma.

Uveal melanoma (UVM) is an aggressive intraocular malignancy with a high risk of metastasis but few effective therapies. However, current molecular classification systems do not completely reflect the immune-stromal interactions underlying tumor progression and immune dysfunction. Therefore, we aimed to establish a multidimensional immune ecological framework for UVM by integrating bulk and single-cell transcriptomics with analyses of immune activity, metabolic programs, regulon states, and cell-cell communication. We identified two robust immune ecological subtypes, CS1 and CS2, with markedly

Hyaluronic acid concentration predictably modulates physicochemical properties and clinical performance in ophthalmic viscosurgical devices: integrated laboratory and prospective clinical assessment.

BACKGROUND: Ophthalmic viscosurgical devices (OVDs) are essential in cataract surgery for maintaining anterior chamber stability and protecting intraocular tissues. This study evaluated the physicochemical and rheological properties of three hyaluronic acid-based OVDs (OpHLINE® 1.4%, 2%, and 3%) and assessed their clinical performance. METHODS: A two-phase investigation was conducted: (1) characterization of OpHLINE formulations versus four commercial comparators, including cohesive-dispersive analysis and viscosity-shear profiling; and (2) a prospective clinical trial in 68 patients undergoi

Suprachoroidal injection in the treatment of diabetic macular edema: mechanisms, clinical advances, and future perspectives.

Diabetic macular edema (DME) is the leading cause of vision loss in patients with diabetes. Conventional therapies like intravitreal injections (IVI) of anti-vascular endothelial growth factor (VEGF) agents and corticosteroids are often limited by the frequent injections and risks of complications. Suprachoroidal space (SCS) injection has emerged as a novel drug delivery technique. It targets eye precisely, resulting in high drug concentration in the posterior segment while reducing exposure to anterior segment tissues-thereby potentially lowering risks of both cataract formation and elevated

Transcriptomic Atlas of Human Trabecular Meshwork Uncovers the Cellular Landscape and Provides Insights into Glaucoma Pathophysiology.

The trabecular meshwork (TM) is a specialized multicellular tissue that regulates aqueous humor outflow and intraocular pressure (IOP), and its dysfunction is a central driver of glaucoma. However, how cellular states and molecular mechanisms of TM cell populations are altered in human glaucoma remains poorly understood. Here, we present a comprehensive single-nucleus transcriptomic atlas of the human TM across normal and glaucomatous eyes. Analysis of 285,356 nuclei identified 17 distinct cell populations, including multiple TM structural subtypes, endothelial and neural-associated cells, and

Drug delivery across the blood-retinal barrier: Biological gateways for posterior segment therapy.

Posterior-segment retinal diseases remain a leading cause of irreversible vision loss worldwide, yet effective treatment continues to be limited by the challenge of achieving sufficient and durable drug exposure within target ocular tissues. Although intravitreal injection has transformed the management of retinal diseases, therapeutic success is determined not only by drug potency but also by transport across retinal interfaces, intraocular distribution, and long-term tissue exposure. Traditionally, these challenges have been attributed primarily to the restrictive nature of the blood-retinal

Acute effects of direct exposure to moxifloxacin on the porcine corneal endothelium: An ex vivo study.

Intracameral administration of an ophthalmic solution of moxifloxacin has been proposed as a prophylactic strategy for the prevention of postoperative endophthalmitis following intraocular surgery, especially after phacoemulsification. The present study evaluated the acute effects of direct exposure to moxifloxacin on the porcine corneal endothelium using a worst-case ex vivo exposure model. A total of 48 porcine corneas, obtained from six-month-old male pigs, were allocated into three experimental groups: group A (control; n = 24) received balanced salt solution applied directly to the endoth

Intermittent fasting reduces glaucomatous damage in an HSP27 autoimmune mouse model.

BACKGROUND: Currently, the treatment of glaucoma is limited to reducing intraocular pressure since other involved pathomechanisms are not well understood yet. Evidence points to an immune-mediated component in disease development. For example, elevated antibody levels against heat shock protein 27 (HSP27) were detected in glaucoma patients. In mice, we previously noted glaucoma-like damage after an intravitreal HSP27 injection. Now, we aimed to investigate if intermittent fasting protects from this glaucomatous damage. METHODS: CD1 mice were intravitreally injected with HSP27 into one eye. Th

Chitosan-rapamycin carbon dots rejuvenate autologous Sca-1⁺ stem cells paracrine function for neuroprotection in normal tension glaucoma.

BACKGROUND: Normal tension glaucoma (NTG) causes progressive retinal ganglion cell (RGC) loss without elevated intraocular pressure, and currently lacks effective neuroprotective therapies. OPTN(E50K) mutation, a pathogenic gene of NTG, compromises paracrine trophic capacity in bone marrow (BM) stem cells. Allogeneic young BM stem cells can provide trophic support and exhibit therapeutic potential; however, their clinical application is limited by immunogenic rejection and tumorigenic risks, prompting a renewed interest in rejuvenate autologous BM stem cells strategy. METHODS: To address thes

Advancements in Nanodrug Delivery Systems as Controlled-Release Systems for Glaucoma Therapy: An Inspirational Step Toward Translation from Research to Clinic.

Glaucoma is a collection of disorders that result in permanent vision loss and is characterized by a gradual decline in retinal ganglion cells. While it may not always be high, intraocular pressure (IOP) is the sole risk factor that can be modified according to extensive clinical research. Glaucoma remains the leading cause of irreversible blindness, yet early treatment lowering intraocular pressure is effective in slowing the rate of visual deterioration. Issues like poor absorption, low bioavailability, and short drug resistance time have thus made the management of glaucoma challenging when

A biodegradable hydroxyapatite-coated magnesium drainage plate enables medium-term suprachoroidal outflow preservation and stable intraocular pressure reduction in rabbits.

Postoperative scarring and failure of surgically created drainage pathways limit durable glaucoma surgery. We developed a solid plate-type suprachoroidal space (SCS) implant based on hydroxyapatite-coated magnesium (HA-Mg) to provide temporary pathway support, an HA-modified material-tissue interface, and progressive biodegradation. TGF-β2-stimulated human iris pigment epithelial cells were used to assess material-associated effects on migration and fibrotic activation. In a paired-eye normotensive New Zealand White rabbit model, trabeculectomy, sham SCS surgery, titanium, uncoated magnesium,

A Cross-Sectional Study on the Prevalence and Risk Factors of Steroid-Induced Glaucoma at a Tertiary Eye Care Hospital.

BACKGROUND: Steroid-induced glaucoma (SIG) is a well-recognized complication of corticosteroid therapy, leading to elevated intraocular pressure (IOP) and potential irreversible optic nerve damage. Despite widespread steroid use for ocular conditions, inadequate monitoring, especially with over-the-counter (OTC) preparations, increases the risk of SIG. This study aimed to determine the prevalence and identify the risk factors associated with SIG in a tertiary eye care setting. METHODS: This cross-sectional study was conducted at the Glaucoma Department of Poona Blind Men's Association (PBMA)'

Ocular choristoma with heterogeneous mesenchymal differentiation and concurrent anterior segment dysgenesis in a neonatal Holstein calf.

A 1-day-old Holstein calf presented with central corneal opacity and thickening, and a polypoid lesion in the medial canthus of the left eye, leading to vision loss and enucleation. Grossly, a mass extended from beneath the cornea to the medial intraocular region. Histologically, the mass was composed of proliferative foci of diverse mesenchymal elements, including mature cartilage and adipose tissue, immature cartilage and bone, and proliferating spindle-shaped cells in a myxoid stroma. Anterior segment abnormalities, including iris hypoplasia, iridocorneal adhesion, angle malformation, and l

Ergothioneine as a potential protective agent against macular degeneration and other eye disorders.

Age-related macular degeneration (AMD) is a leading cause of irreversible vision loss in ageing populations, with oxidative stress recognised as a key pathogenic driver. The dietary antioxidant and cytoprotectant, L-ergothioneine (ET), is avidly accumulated in many tissues, especially the eye. However its relationship to AMD is unclear. Here, we examined ET's distribution in human ocular tissues and measured serum and intraocular ET levels in patients with neovascular AMD. Compared with ocularly-normal age-matched individuals, AMD patients exhibited significantly lower serum ET; elevated level

Numerical Model of the Eye for Understanding Acute Microgravity-Induced Ocular Changes.

INTRODUCTION: Intraocular pressure (IOP) changes with acute microgravity exposure defy easy explanation. IOP increases above supine levels acutely in weightlessness, but studies show both central venous pressure and intracranial pressure (ICP) fall below supine values. These reductions would be expected to reduce IOP in weightlessness. We developed and validated a physiologically relevant numerical model of the eye to investigate how these ocular changes could occur. METHODS: A finite element numerical model of the 50th percentile adult human globe, optic nerve, and surrounding tissues was co
