Millimeters from catastrophe: The pericardium as a lifesaving shield.

We report a rare case of a 2-year-old child presenting 48 h after accidental penetrating chest trauma due to a fall from a motorcycle, with a retained foreign body lodged within the pericardial sac. Imaging revealed an opaque object overlying the cardiac silhouette. On median sternotomy, approximately 50 mL of pericardial blood was evacuated, and a piece of metallic debris was found resting beneath the left atrial appendage without injury to cardiac structures. The foreign body was successfully removed without cardiopulmonary bypass, and the child had an uneventful recovery. Retained foreign b

Foreign bodies in the thyroid gland and foreign body reactions in the post-thyroidectomy bed.

Foreign bodies (FBs) in the thyroid gland are very rare and may create diagnostic and therapeutic problems. So far, there have been a few review articles concerning only ingested FBs in the thyroid gland, while other mechanisms remain poorly studied. FB reactions mimicking thyroid regrowth in the post-thyroidectomy bed may also pose a diagnostic challenge for clinicians due to their rarity. The article discusses ingested, traumatic, and iatrogenic FBs in the thyroid gland. Our search generated 56 relevant articles with 63 total cases of FBs in the thyroid gland. The review highlights the clini

Clinical profile and outcomes of posterior segment metallic intraocular foreign bodies following chisel-hammer injury managed with pars plana vitrectomy.

PURPOSE: Chisel hammer injury-associated retained intraocular foreign bodies (CHI-IOFBs) are a major occupational health hazard and can lead to significant visual decline. The objective of the current study was to evaluate the anatomical and functional outcomes of pars plana vitrectomy (PPV) in posterior segment CHI-IOFB. METHODS: This was a retrospective case series over 5 years involving patients who underwent 23-gauge PPV for CHI-IOFB. Patients with <3 months of follow-up after PPV were excluded. RESULTS: Overall, 29 patients (mean age 29.2 years, 96.6% male) underwent PPV. At presentatio

Once yearly cell-based therapy for sustained and dose-tunable delivery of monoclonal antibodies.

More than 200 monoclonal antibodies (mAbs) are approved for clinical use, yet their therapeutic potential is constrained by dependence on repeated injections or infusions that drive nonadherence, limit access in low-resource settings, and generate peak-trough pharmacokinetics linked to adverse effects and reduced efficacy. Here, we developed an immunomodulatory, encapsulated cell-based "biologics factory" that overcomes mAb instability, immunogenicity, and the fibrotic foreign body response that have limited previous approaches, enabling continuous in situ production of therapeutic antibodies

Rectal Insertion of Foreign Bodies: Motivations, Complications, and Management.

The Psychiatric Consultation Service at Massachusetts General Hospital sees medical and surgical inpatients with comorbid psychiatric symptoms and conditions. During their twice-weekly rounds, Dr Stern and other members of the Consultation Service discuss diagnosis and management of hospitalized patients with complex medical or surgical problems who also demonstrate psychiatric symptoms or conditions. These discussions have given rise to rounds reports that will prove useful for clinicians practicing at the interface of medicine and psychiatry. Prim Care Companion CNS Disord 2026;28(5):26f0424

Predictors of dysfunction in fully versus partially covered self-expanding metal stents for malignant esophageal strictures: a 10-year multicenter study.

BACKGROUND: Esophageal self-expanding metal stents (SEMS) effectively restore oral intake in malignant obstruction, yet stent dysfunction remains a frequent clinical challenge. We aimed to identify and compare predictors of stent dysfunction in fully versus partially covered self-expanding metal stents (fcSEMS vs. pcSEMS). METHODS: This multicenter retrospective cohort included patients undergoing SEMS placement for malignant esophageal strictures between 2014 and 2023. Stent dysfunction comprised migration, tumor overgrowth, or food bolus impaction. Patients with benign or anastomotic strict

Repair of Full Thickness Articular Cartilage Using a 3D-Printed Acellular Polymer Implant in a Caprine Model: A Pilot Study.

ObjectiveEvaluate a 3D-printed acellular polymer scaffold combined with microfracture for repairing critical-sized cartilage defects in a goat model.MethodsSix goats received bilateral 10 mm femoral condyle defects. In a paired design, one knee received microfracture plus a scaffold (95% or 45% infill); the contralateral control received microfracture alone (final n = 6). At 16 weeks, repair was assessed via histology (modified ICRS II), immunohistochemistry, and polarized light microscopy. Histological analysis included three scaffold-treated and three control defects; others were excluded du

Three-port gasless intermuscular approach for endoscopic thyroidectomy: A six-step method.

BACKGROUND: The global incidence of thyroid cancer is on the rise. Endoscopic thyroid surgery via an infraclavicular approach preserves anterior cervical function, with patients experiencing no postoperative numbness of the anterior cervical skin, neck muscle tightness, or foreign body sensation during swallowing. It also offers superior cosmetic outcomes due to the relatively concealed incision site, thereby gradually becoming a strong complement to conventional open surgery. However, this technique presents certain limitations in terms of incision size, extent of flap dissection, and surgica

Comparison of bone regeneration in rabbit calvarial defects following the use of two different xenografts with a barrier membrane: A histological, histomorphometric, and micro-computed tomography study.

BACKGROUND: This study compared bone regeneration in rabbit calvarial defects using two different xenografts with a barrier membrane by histological, histomorphometric, and micro-computed tomography (micro-CT) analyses. MATERIALS AND METHODS: This study was designed as a single-blind interventional experimental animal study that conducted on 20 male New Zealand rabbits. The rabbits were anesthetized, a full-thickness mucoperiosteal flap was elevated, and four bone defects with 8 mm diameter were created in their calvaria at the two sides of their midsagittal suture. Two defects were randomly

Necrotizing Pancreatitis Associated With an Ingested Screw Adjacent to the Ampulla of Vater.

A 78-year-old man presented with severe epigastric pain and was diagnosed with acute pancreatitis that progressed to necrotizing pancreatitis despite supportive management. No common etiologies were identified. During endoscopic nasojejunal feeding tube placement, a metallic screw was incidentally discovered in the second portion of the duodenum adjacent to the major papilla and successfully removed. The foreign body had not been detected on previous computed tomography, ultrasound, or abdominal radiography. The patient recovered completely without recurrence. This case highlights a rare peria

An Unexpected Journey: Migration of a Scoliosis Rod to the Groin.

Rod migration is a rare but potentially serious late complication of spinal instrumentation. A case of long-distance rod migration to the groin following anterior lumbar fusion for adolescent idiopathic scoliosis is presented. A 43-year-old female, who had undergone selective anterior lumbar fusion 20 years earlier, presented with progressive lumbar pain. Initial radiographs demonstrated screw loosening and rod dislodgment. Before the planned revision surgery, the patient re-presented with pain localized to the hip and groin. Imaging revealed distal migration of the rod toward the anterior asp

Endolysin XZ.700 preserves the functional activity of osteoclasts and foreign body giant cells on a bone substratum.

Periprosthetic bacterial infection is a major challenge in orthopedics and dentistry. The novel bacteriophage-derived endolysin XZ.700 has been developed to treat implant-associated bacterial infections. It exhibits low osteoblast-toxicity, and reduces osteoclast and foreign body giant cell formation on plastic substratum, suggesting short-term administration as a safe strategy. However, whether endolysin XZ.700 affects osteoclast and foreign body giant cell function in a bone microenvironment remains unknown. Therefore, this study aimed to investigate whether endolysin XZ.700 affects osteocla

Intentional nail ingestion with caecal impaction: colonoscopic removal using a cap-assisted technique.

Most foreign body ingestions in adult patients pass spontaneously while sharp objects often require endoscopic intervention. Endoscopic foreign body removals are usually performed in the upper gastrointestinal (GI) tract, with few reports involving the lower GI tract. We present a case of a woman with recurrent nail ingestion who presented with abdominal pain after swallowing multiple nails. Initial conservative management demonstrated progressive migration, with eventual clustering near the ileocaecal valve. In this case, a cap-assisted colonoscopy was successfully used to remove all ingested

Lightweight polypropylene mesh is associated with higher long-term recurrence after open ventral hernia repair.

BACKGROUND: Mesh selection in ventral hernia repair remains controversial. Lightweight mesh was developed as a replacement for heavier meshes to reduce foreign body response and improve abdominal wall compliance, but concerns regarding durability remain. This study evaluated long-term recurrence rates after open ventral hernia repair using lightweight mesh versus midweight mesh. METHODS: A prospectively maintained tertiary hernia center database was queried for elective open ventral hernia repair performed with lightweight mesh or midweight mesh. Propensity-score matching (1:1) was performed
