Transcranial penetrating skull base trauma with metal rod: Endoscopic repair of CSF leak.

BACKGROUND: Penetrating skull base injuries are complex clinical scenarios requiring multidisciplinary management to address both immediate life-threatening conditions and long-term complications.Anterior skull base fractures account for 21% of skull fractures from which 4% were caused by head trauma [1]. Post-traumatic cerebrospinal fluid (CSF) leaks may arise, becoming a major source of morbidity; these can lead to the development of severe intracranial infections [2]. Endoscopic Endonasal Approach is a conservative technique that uses a multilayer coverage including a nasoseptal flap to cre

Penetrating intraorbital wooden foreign bodies involving the right frontal lobe of the brain: A rare case report.

BACKGROUND: Penetrating orbital trauma from a wooden foreign object is rare and challenging to identify, as it cannot be detected with a computed tomography (CT) scan. This report highlights the clinical presentation, diagnosis, and management of such a case. CASE DESCRIPTION: A 19-year-old female experienced penetrating orbital trauma from a wooden object following a traffic accident. She intermittently visited the hospital with eye swelling and headache, without visible penetrating wounds on the palpebra. The swelling persisted for weeks, accompanied by increasing signs of ocular and cerebr

Metallic Intraocular Foreign Bodies Following Torsional Phacoemulsification Surgery.

PURPOSE: This study aimed to determine the rate of retained metallic intraocular foreign bodies (IOFBs) following torsional phacoemulsification cataract surgery by an experienced surgeon and elucidate related surgical factors, particle origin theories from existing literature, and postoperative visual impact. METHODS: An observational study was conducted on 300 consecutive cases of cataract who underwent torsional phacoemulsification by a single surgeon using two-handed stop-and-chop technique. The incidence of retained IOFBs was observed and correlated with the surgical parameters and the de

The usefulness of computed tomography with the reduction of metal artifacts in the location of an intraorbital foreign body - a case report.

Intraorbital foreign bodies still remain an unsolved and serious diagnostic problem. Due to the complicated anatomical orbital structure, both the diagnosis and treatment of these injuries are considered to be one of the most challenging. We present a case of a metallic foreign body (a bullet) imbeded in the orbit of a 10-year-old boy. Only after performing computed tomography imaging using the metal artifacts reduction algorithm it was possible to properly localize the foreign body. The issue of intraorbital foreign bodies is interdisciplinary and requires the co-operation between many specia

ULTRASOUND BIOMICROSCOPY AND ECHOGENIC EXTERNAL MARKER ASSISTED INTRAOCULAR FOREIGN BODY REMOVAL.

PURPOSE: We report a novel method of intraoperative localization of a retained anterior intraocular foreign body (IOFB), using a combination of ultrasound biomicrosopy and an insulated needle. METHODS: A retrospective case report of a 56-year-old man who presented with a right eye IOFB. RESULTS: On presentation, vision was decreased in the right eye to count fingers with a small subconjunctival hemorrhage, but no other signs of a penetrating laceration. Orbital CT revealed an IOFB, and the initial vitrectomy failed to retrieve the IOFB. Then, during the subsequent vitrectomy, using an ultras

The use of anterior segment optical coherence tomography (AS-OCT) in the imaging diagnostics of intracorneal foreign bodies.

BACKGROUND: Eye injuries are a serious problem and may result in reduced visual acuity. The most common eye injuries include corneal damage, often involving the presence of a foreign body. OCT examination of the anterior segment - AS OCT - may be a valuable test in the diagnostics and follow-up of patients in whom the presence of a foreign body after eye trauma is suspected. CASE PRESENTATION: The authors present the case of a young man diagnosed with multiple, glass, intrastromal corneal foreign bodies. A 30-year-old patient visited an ophthalmologist a few days after an eye injury accompani

Review of Management and Outcomes of Delayed Repair Open Globe Injuries.

The standard of care for open globe injuries is prompt surgical closure, as delay in repair is a reported risk factor for post-traumatic endophthalmitis and is associated with worse visual outcomes. This article serves as a review of the current management and outcomes of open globe injuries repaired greater than 24 hours from the time of injury, specifically evaluating the rates of endophthalmitis in cases with and without intraocular foreign bodies, visual outcomes and rates of primary enucleation or evisceration.

Acquired torticollis due to an ocular surface foreign body.

Ocular torticollis is traditionally attributed to eye misalignment, nystagmus, ptosis, or refractive error. We present 3 pediatric cases of acquired torticollis caused by a foreign body beneath the upper eyelid. The head posturing presumably developed to minimize contact of the foreign body with the corneal surface and mitigate ocular discomfort. Torticollis resolved in all cases after removal of the foreign bodies. These cases demonstrate that acquired torticollis in a child can be a clinical sign of an ocular surface foreign body.

Post-traumatic retinal siderosis, a case report.

A thirty-year-old patient attended a few hours after an ocular trauma while hammering, receiving trauma with a metal splinter at the left eye. Due to an unfavorable clinical picture, surgical management was decided, which was initially rejected by the patient. He returned six months later with a profound decrease in left eye visual acuity, reaching counting finger at one meter despite optical correction. The anterior segment shows a lower scarring leukoma, associated with Tyndall (++), retrokeratic pigment, lower posterior synechiae and a total cataract. Cataract surgery with intraocular lens

An Unusual Case of Low Vision and Anisocoria Considered a Neurological Finding in the Emergency Department: Ocular Siderosis.

We present the case of a patient who came to the emergency department with a significant decrease in vision and dilated pupil in the left eye. Since neurological pathologies were primarily considered, diffusion brain magnetic resonance imaging (MRI) and brain computed tomography (CT) were requested. After the results were reported as normal, we were consulted. On examination, the anterior segment was normal but we detected shiny pearl-like formations in the anterior vitreous, condensation at the inferior of the posterior vitreous, and a scar in the macula. When we evaluated the orbital section

Demystifying the role of magnetic resonance in identifying intraocular foreign bodies: a case of ocular siderosis.

BACKGROUND: Ocular siderosis (OS) is a significant cause of visual loss due to retained ferrous intraocular foreign bodies (IOFB). Despite its rarity, OS can lead to severe visual impairment if not promptly diagnosed and treated. This case is notable due to the occult nature of the IOFB, which was undetected by standard imaging modalities, emphasizing the critical role of magnetic resonance imaging (MRI) in such scenarios. CASE PRESENTATION: A 51-year-old Caucasian male presented with progressive vision loss in his right eye over 20 days. Best corrected visual acuity (BCVA) was 20/1000 in the

A rare intraocular foreign body resulting from a workplace accident.

A 54-year-old man presented to the ophthalmic emergency department of our center with eye pain and blurred vision in his right eye following a workplace accident. Examination revealed a penetrating corneal injury with the presence of an intraocular foreign body (IOFB) involving the corneoscleral limbus, perforating the cornea, iris, anterior lens capsule, and lens. Immediate surgical intervention was carried out with the extraction of the IOFB, identified as an 8mm mussel shell fragment, and the removal of the resulting traumatic cataract. Both preoperative and postoperative examinations showe
