Retrobulbar Hematoma in a Patient with Thyroid Eye Disease.

A 37-year-old female with a history of thyroid eye disease presented after trauma to the right side of her face. Visual acuity was light perception and a relative afferent pupillary defect was detected. There was gross proptosis of the unruptured right globe with diffuse conjunctival hemorrhage. Computed tomography revealed an intraconal retrobulbar hematoma (RBH) with anterior herniation of the globe. The extraocular muscles were thickened, consistent with her thyroid eye disease, and her superior rectus was avulsed. The optic nerve was on stretch, but there was no evidence of transection. Th

Surgical approach to repair of traumatic avulsion and external herniation of the levator palpebrae superioris aponeurosis.

This report describes the unusual case and surgical repair of avulsion and external herniation of the levator palpebrae superioris (LPS) aponeurosis and attached muscle fibres associated with superior tarsal plate fracture in an 8-year-old Indigenous girl. To the authors' knowledge, isolated and complete levator palpebrae superioris avulsion with external herniation in trauma is previously unreported. Open injury to LPS requires swift surgical repair, and an anterior ptosis repair approach may facilitate identification of anatomical structures to prevent upper eyelid ptosis. Ultimately, the su

A case of spontaneous avulsion of primary pterygium analyzed with anterior segment optical coherence tomography.

PURPOSE: This study reports a case of the spontaneous avulsion of primary pterygium with anterior segment optical coherence tomography (AS-OCT) findings. OBSERVATION: A 72-year-old woman complained of acute pain of the left eye. Primary pterygia were noted in both eyes on the initial examination 4 months ago. Slit-lamp microscopy revealed a nasal corneal epithelial defect, and the rolled elevated lesion in the corneal limbus of the left eye. She was diagnosed with a spontaneous avulsion of the corneal pterygium head. Then the avulsed pterygium head slowly recurred. The pterygium head of the f

Paracentral Acute Middle Maculopathy following Surgically Induced Branch Retinal Artery Occlusion During Vitrectomy.

Paracentral acute middle maculopathy (PAMM) is a spectral-domain OCT finding of a thickened hyperreflective band at the level of the intermediate layers of the inner retina, attributed to the acute phase of intermediate capillary ischemia. The purpose of this paper is to report a case of PAMM deriving from a surgically induced branch retinal artery occlusion (BRAO) during vitrectomy for vitreous hemorrhage. A 70-year-old female referred to the posterior segment office with a 1-month history of visual loss in her right eye. Best-corrected visual acuity (BCVA) was "counting-fingers" (CF) due to

Traction Applying Technique on Bicanalicular Crawford Silicone Tube to Overcome the Notching Deformity After Lacrimal Canalicular Repair.

Lower eyelid avulsion injury with lower canalicular laceration generally occur just medial to the punctum with insufficient skin remnant for repair causing tension on repair margins. The inevitable blinking force, along with the tension widens the repair margin, resulting in an aesthetically challenging notch at the medial lower lid. The authors attempted to minimize this notching deformity with a traction applying technique on bicanalicular silicone tube.Fifteen patients were enrolled and divided into 2 groups: the experimental group with 10 patients which received the traction technique, and

A case report of reconstruction of ocular and complete upper eyelid avulsion with severe facial soft tissue injuries using anterolateral thigh free flap.

INTRODUCTION AND IMPORTANCE: Ocular injury is second mostly caused by motor vehicle accident (MVA) and often leads to severe ocular injury even to visual loss and various aesthetic problems. The outcome is determined by the magnitude of the initial damage and treatment availability. Treating ocular and facial injury due to MVA is challenging given the scope of the damage and may result in various outcomes. CASE PRESENTATION: This case presented a 22-years old woman with a history of car accident assessed with total upper eyelid avulsion with corneal perforation and involvement of multiple fac

Optic nerve avulsion: Case report.

INTRODUCTION: Optic nerve avulsion is a traumatic disinsertion of optic nerve fibres from the globe at the level of the lamina cribrosa. It is an uncommon and severe complication of blunt ocular trauma. CASE PRESENTATION: We report the case of a 15 years old male presented to the emergency department after being kicked by a horse. Initial ophthalmologic examination of the left eye (LE), exhibited eyelid hematoma, subconjunctival hemorrhage, VA was limited to light perception and there was a left relative afferent pupillary defect. Dilated fundus examination of the LE revealed an extensive vit

Acute Abducens Nerve Palsy in a Patient who Sustained Mechanical Trauma to the Orbit.

BACKGROUND: Abducens nerve (Cranial Nerve VI) innervates the lateral rectus (LR) muscle. Head trauma is one of the most common causes of abducens nerve palsy. Orbital and/or facial injuries could also affect the LR muscle directly or the orbital course of abducens nerve and lead to palsy. We present a case of a young man with multiple orbital fractures and an impingement of the LR muscle resulting in a complete loss of abduction. CASE REPORT: A 29-year-old male reported falling 15 feet. He presented with diplopia and had complete abduction deficit of the left eye. Orbital CT imaging revealed

Traumatic chiasmopathy following mild trauma in a patient with thyroid orbitopathy.

PURPOSE: Traumatic injury to the optic chiasm is rare and most frequently caused by high-velocity head trauma. It classically results in bitemporal hemianopsia and often presents in conjunction with multiple other traumatic injuries, such as skull fractures and cerebrospinal fluid leaks. We present the case of a 40-year-old woman with pre-existing thyroid orbitopathy who struck her forehead after a fall from standing height. OBSERVATIONS: This patient suffered immediate profound unilateral vision loss from traumatic optic neuropathy and possible optic nerve avulsion. The fellow eye manifested
