Surfing-related head injuries presenting to United States emergency departments.

This study examined the incidence and trends of surfing-related and mild traumatic brain injuries that presented to United States emergency departments between 2001 and 2016. Subjects with surging-related head injuries were retrieved from the National Electronic Injury Surveillance System. A weighted total of 34,337 surfing-related head injuries were identified. The annual incidence of surfing-related head injuries insignificantly decreased from 2001 to 2016 (R2 = .119; p = .19). Most common injuries included lacerations (50.4%), blunt head injuries (25.7%), and mild traumatic brain injuries (

Sleep Parameters and Overnight Urinary Melatonin Production in Children With Persistent Post-concussion Symptoms.

BACKGROUND: Sleep disturbance is common after a mild traumatic brain injury (mTBI) in children, yet its biology is poorly understood. We aimed to explore sleep-related problems (SRPs), sleep-activity patterns, and endogenous melatonin production in children with different recovery trajectories following mTBI. We hypothesized that children with delayed recovery would have more SRPs and abnormal sleep-activity patterns, which would correlate with lower overnight melatonin production. METHODS: In this prospective controlled cohort study, we enrolled 83 children with persistent symptoms, 26 child

Discovery of novel plasma biomarker ratios to discriminate traumatic brain injury.

Background: Traumatic brain injury (TBI) is a major cause of death and disability. Despite increased awareness, reliable biomarkers are urgently needed to aid in all forms of traumatic brain injury diagnosis and prognosis. Methods: Here, we aim to assess the diagnostic utility of known and novel TBI biomarkers in a pilot patient cohort of severe TBI (sTBI) patients and healthy controls. We analyzed concentrations of S100 calcium binding protein B (S100B), neuron specific enolase (NSE), human kallikrein 6 (hK6) and prostaglandin D2 synthase (PGDS) using ELISA immunoassays. Results: Plasma level

Concussion, Chronic Traumatic Encephalopathy and Sport in a Legal Setting.

This article highlights aspects of the medical condition Chronic Traumatic Encephalopathy (CTE) relevant to legal claims in negligence threatened by players of collision and contact sports in Australia against their sporting organisations. CTE is characterised by cognitive dysfunction, irritability, aggression, depression, short-term memory loss, heightened suicidality and ultimately death, which may, in advanced forms, be preceded by dementia and parkinsonism. It is neither the purpose nor intention of this article to consider each element of a negligence claim, but rather to provide a means

Rethinking Youth Sports.

When we consider the health and welfare of our children, few issues are of more concern than the risk and consequences of head trauma in sports. From participation in male-dominated football and hockey to girls soccer and softball, repeated impacts to the head that do not produce recognized concussion is the cutting edge of sports head injury research. Our author, a leading researcher in this field, provides the latest on what we know and don't know on this relatively new frontier of neuroscience.

Case Report: Buccal administration of hydrogen-producing blend after a mild traumatic brain injury in a professional athlete.

Background: Sport-related mild traumatic brain injury (TBI) is a serious trauma that could impair brain function of an injured athlete. Treatment solutions for mild TBI typically concentrate on complete rest, while non-traditional therapeutic options remain largely ineffective. Molecular hydrogen (H 2 ) is an innovative neuroprotective agent that can easily reach the brain, yet no data are available concerning its value as a first-aid intervention after a mild TBI. Case report: This case report demonstrates the efficacy and safety of a hydrogen-producing dissolving tablet administered buccally

Repeated mild traumatic brain injury causes persistent changes in histone deacetylase function in hippocampus: Implications in learning and memory deficits in rats.

Impaired attention and memory represent some of the major long-term consequences of brain injuries. However, little is known about the underlying molecular mechanisms of brain trauma-induced cognitive decline. Histone deacetylases (HDACs) in the hippocampus are believed to impact learning and memory. Herein, we have induced repeated mild traumatic brain injury (rMTBI) in rats by using weight-drop paradigm, examined the recognition memory using novel object recognition task, and assessed the HDAC activities in the hippocampus post 48 h and 30 days of rMTBI. The recognition memory was significan

Dynamic Functional Network Analysis in Mild Traumatic Brain Injury.

Mild traumatic brain injury (mTBI) is one of the most common neurological disorders for which a subset of patients develops persistent postconcussive symptoms. Previous studies discovered abnormalities and disruptions in the brain functional networks of mTBI patients principally using static functional connectivity measures which assume that neural communication across the brain is static during resting state conditions. In this study, we examine the differences in dynamic neural communication between mTBI and control participants through the application of a combination of dynamic functional

Neurophysiological abnormalities in individuals with persistent post-concussion symptoms.

Concussion injury results in a rapid onset of transient neurological impairment that can resolve quickly, or sometimes evolve over time, but usually resolve within seven to 10 days. However, a small but noticeable cohort (~10%) of individuals continues to experience persistent lingering effects, particularly fatigue, recognized as post-concussion symptoms (PCS). This study explored neurophysiological mechanisms in people with persistent PCS. Studies involved using self-report post-concussion fatigue scale, transcranial magnetic stimulation (TMS) and somatosensory stimulation in those with diag

Functional connectivity disruption of the substantia nigra associated with cognitive impairment in acute mild traumatic brain injury.

PURPOSE: Mild traumatic brain injury is known to have frequent cognitive impairment. Accumulating evidence is pointing to the malfunctioning of the substantia nigra (SN) as an important factor for head trauma. However, it remains unknown whether changes in the SN-based resting state functional connectivity following mTBI at acute stage and its relationship with cognitive function. MATERIALS AND METHODS: 58 patients with mTBI and 30 age-, gender-, and years of education-matched healthy controls were enrolled in the current study. All of participants received resting state functional magnetic r

Combining white matter diffusion and geometry for tract-specific alignment and variability analysis.

We present a framework for along-tract analysis of white matter (WM) fiber bundles based on diffusion tensor imaging (DTI) and tractography. We introduce the novel concept of fiber-flux density for modeling fiber tracts' geometry, and combine it with diffusion-based measures to define vector descriptors called Fiber-Flux Diffusion Density (FFDD). The proposed model captures informative features of WM tracts at both the microscopic (diffusion-related) and macroscopic (geometry-related) scales, thus enabling improved sensitivity to subtle structural abnormalities that are not reflected by either

Abnormal Motor Response Associated With Concussive Injuries: Biomechanical Comparison Between Impact Seizures and Loss of Consciousness.

CONTEXT: Loss of consciousness (LOC) and impact seizures associated with concussion represent different clinical presentations of concussion; however, they are often investigated and treated similarly. The biomechanical parameters differentiating these 2 distinct signs of injury are poorly described. OBJECTIVE: To differentiate between cases of concussions with LOC and those with impact seizures by comparing the impact velocity, peak linear and peak rotational acceleration, as well as brain tissue deformation in the cerebral cortex, white matter, brainstem, cerebellum, thalamus, and corpus ca

Head first - Review of epilepsy head injury risk and protection.

INTRODUCTION: The causes of falls and resultant head injuries in people with epilepsy are complex and represent important clinical issues significantly impacting on safety, quality of life and long-term prognosis. A general solution for many years has been to provide helmets for head protection. There is poor understanding of the influencing risk factors and solutions to give person-centred clinical advice. METHODS: A focused narrative review was conducted to investigate the risk factors for seizure-related head injuries and evidence of current helmets to offer protection. Four databases were

Towards an embedded symptom validity indicator for the rivermead postconcussion symptom questionnaire.

This simulation study aimed to develop and test an embedded symptom validity indicator for use with the Rivermead Post Concussion Symptoms Questionnaire (RPQ). Seven mild traumatic brain injury (mTBI) experts assisted in generating a shortlist of 15 atypical postconcussion symptoms. A modified RPQ (mRPQ) was constructed by embedding these items with the 16 standard RPQ items. Eighty-four mTBI simulators completed the mRPQ after random allocation to a symptom-exaggeration condition (ES group; n = 46) or simulation-only condition (S group; n = 38). They also completed the Test of Memory Malinger

Microstructural abnormalities in deep and superficial white matter in youths with mild traumatic brain injury.

BACKGROUND: Diffusion Tensor Imaging (DTI) studies of traumatic brain injury (TBI) have focused on alterations in microstructural features of deep white matter fibers (DWM), though post-mortem studies have demonstrated that injured axons are often observed at the gray-white matter interface where superficial white matter fibers (SWM) mediate local connectivity. OBJECTIVES: To examine microstructural alterations in SWM and DWM in youths with a history of mild TBI and examine the relationship between white matter alterations and attention. METHODS: Using DTIDWM fractional anisotropy (FA) and S
