Hookworm infection, caused by Necator americanus and Ancylostoma duodenale, is prevalent in areas with poor sanitation. Coprology is the gold standard for diagnosis, but it has low sensitivity in low parasite loads. The objective of this study was to evaluate four Polymerase Chain Reaction (PCR) targets for molecular diagnosis. Stool samples were collected from 180 residents of the rural community Tierra Blanca, Bolívar State, Venezuela, in which coproparasitological tests were performed. Four DNA extraction methods were standardized, and four PCR techniques were studied: NaITS1F/NaITS1R (ITS1

Hookworm infects over 400 million people globally and causes gastrointestinal morbidity, yet its physiological effects remain poorly defined. Controlled human hookworm infection is also being explored as a therapy for gut diseases. We performed an exploratory study to evaluate the impact of experimental Necator americanus infection on gastrointestinal transit, motility, and luminal pH in 10 healthy adults (mean age 41 yr, 60% females) infected with 30 larvae via skin application. Assessments using the SmartPill Wireless Motility Capsule were performed at baseline, week 6 (acute infection), and

BACKGROUND: Hookworms remain problematic in Dak Lak province, Vietnam, despite a school preventive chemotherapy (PC) program since 2007. As hookworms depend on favourable ecological conditions, geospatial modelling incorporating environmental and climatic variables can predict high-risk areas for targeted interventions. This study provides geospatial risk predictions for hookworm infection and intensity among school-aged children in Dak Lak.
METHODS: Hookworm infection status and intensity from 7,964 school-aged children from 64 schools collected in 2019-2020 during the Community Deworming fo

BACKGROUND: Necator americanus glutathione S-transferase-1 (Na-GST-1) performs a crucial enzymatic step in hookworm blood feeding and is a leading target for vaccine development. Phase 1 trials in Brazil, Gabon, and the USA of recombinant Na-GST-1 adsorbed on Alhydrogel (Na-GST-1/Al), co-administered with or without AP 10-701 (Toll-like receptor [TLR] 4 agonist) or CpG 10104 (TLR9 agonist), showed the vaccine was well tolerated, safe, and immunogenic. A controlled human hookworm infection (CHHI) model was previously developed to evaluate the effect of candidate hookworm vaccines on infection.

BACKGROUND: The antigen Na-GST-1, expressed by the hookworm Necator americanus, plays crucial biochemical roles in parasite survival. This study explores the development of mRNA vaccine candidates based on Na-GST-1, building on the success of recombinant Na-GST-1 (rNa-GST-1) protein, currently assessed as a subunit vaccine candidate, which has shown promise in preclinical and clinical studies.
METHODOLOGY/FINDINGS: By leveraging the flexible design of RNA vaccines and protein intracellular trafficking signal sequences, we developed three variants of Na-GST-1 as native (cytosolic), secretory,

BACKGROUND: Hookworm infection remains of considerable importance to public health. However, because critical cases caused by hookworm infection are rarely observed in China, accurate and prompt diagnoses are difficult to achieve in clinical practice. In this study, we describe how we combined morphological and molecular approaches to achieve the clinical diagnosis of hookworm infection.
CASE PRESENTATION: A 75-year-old Chinese woman who presented with dizziness, poor appetite, poor sleep, and weakness in her limbs was diagnosed with chronic atrophic gastritis and was positive for Helicobacte

BACKGROUND: Necator americanus is the predominant species causing hookworm infections in humans. Despite advancements in prevention strategies, mild cases of infection still occur, highlighting the need for improved detection technology. Recombinase Polymerase Amplification (RPA) is an isothermal molecular diagnostic known for its sensitivity, speed, portability, and widespread application in detecting various pathogens. Although several molecular assays are available for N. americanus, they have limitations in detecting mild N. americanus infections.
METHODS: Fluorescent RPA primers and prob

Hookworms infect 450 million people globally and account for the loss of 5 million disability-adjusted life years annually. Over the last decade, the Human Hookworm Vaccine (HHV) candidate N. americanus Glutathione-S-Transferase-1 (Na-GST-1) has advanced to efficacy testing. This manuscript describes the manufacture of third-stage N. americanus larvae (NaL3) as an infectious challenge agent to provide "proof-of-concept" for the efficacy of Na-GST-1 prior to more extensive and more resource-intensive vaccine field trials in hookworm endemic areas. NaL3 were produced from fecal samples of three

Reverse vaccinology is a time- and cost-effective approach to identify potential vaccinable antigens for further in vivo experimental validation. Despite its wide application to multiple organisms, the use of in silico vaccine development tools to parasitic nematodes has been limited. Herein, we have used the rodent hookworm Nippostrongylus brasiliensis as a mouse model for the human hookworm Necator americanus to identify potential vaccine candidates against the latter. Our strategy combined advanced bioinformatic evaluations with knowledge-based criteria. A cumulative rating of antigenic pro

We report human infections with Necator gorillae and Strongyloides fuelleborni, zoonotic helminths from nonhuman primates, in Gabon. We also detected a cryptic Ancylostoma species helminth. Infections occurred in settings of localized deforestation and environmental degradation, which increase human-animal contact. Surveillance to clarify the extent of human infections is needed.

Microbial translocation drives chronic immune activation that is linked to HIV disease progression. Invasive parasitic gut nematodes induce microbial translocation and downregulate the expression of pro-inflammatory cytokines. We evaluated the impact of albendazole anthelmintic therapy on serum markers of microbial translocation and inflammation among helminth-infected Ugandans living with HIV. Participants were randomized to an immediate or delayed 400 mg of daily albendazole and followed for 1 month. Baseline stool analysis, conducted via multiparallel real-time quantitative polymerase chain

In this chapter, we describe the scientific, technical, clinical and regulatory aspects of establishing a controlled human hookworm infection (CHHI) model in non-endemic and endemic geographical regions, to facilitate a pathway towards accelerated vaccine development. The success achieved in establishing the CHHI platform specifically allows the Human Hookworm Vaccine Initiative (HHVI) to accelerate its progress by establishing a human hookworm vaccination/challenge model (HVCM) in a hookworm endemic area of Brazil. The HVCM will permit the rapid and robust determination of clinical efficacy i

Increased permeability of the intestinal epithelial layer is linked to the pathogenesis and perpetuation of a wide range of intestinal and extra-intestinal diseases. Infecting humans with controlled doses of helminths, such as human hookworm (termed hookworm therapy), is proposed as a treatment for many of the same diseases. Helminths induce immunoregulatory changes in their host which could decrease epithelial permeability, which is highlighted as a potential mechanism through which helminths treat disease. Despite this, the influence of a chronic helminth infection on epithelial permeability

UNLABELLED: Controlled Human Infection Models (CHIS) involve administering human pathogens to healthy participants in controlled medical settings, which can elicit complex bioethical issues. Understanding how the community perceives such studies can significantly increase the participant's sense of cooperation and increases the researcher's and the participant's transparency. The current study describes the development of an educational intervention to achieve these ends as it aims to (1) analyze perceptions of the Controlled Human Infection Studies (CHIS), and (2) evaluate the participants' c

Hookworm infection remains a significant public health concern, particularly in low- and middle-income countries, where mass drug administration has not stopped reinfection. Developing a vaccine is crucial to complement current control measures, which necessitates a thorough understanding of host immune responses. By leveraging controlled human infection models and high-dimensional immunophenotyping, here we investigated the immune remodeling following infection with 50 Necator americanus L3 hookworm larvae in four naïve volunteers over two years of follow-up and compared the profiles with nat

INTRODUCTION: Soil-transmitted helminth (STH) is a group of helminths that are considered to be neglected tropical diseases (NTDs) and, globally, affect more than 1.5 to 2.6 million people yearly. Depending on the species, they can be acquired by ingesting embryonated eggs from contaminated matter or by skin penetration. Most species of STH are found in the tropics, such as the Philippines. Despite the Mass Drug Administration (MDA), the cases of STH infection continue to rise in the country. Surveys from the Research Institute of Tropical Medicine (RITM) indicate that a high prevalence of STH

BACKGROUND: Although there is unprecedented interest in experimental human hookworm infection, details of hookworm manufacture and characterisation have been sparsely reported. In this report, we detail the production and characterisation of Necator americanus larvae for use in a recently published clinical trial.
METHODS: Faeces was obtained from an experimentally infected donor. Faecal hookworm DNA was determined by quantitative PCR. Paired samples were incubated in either sterile water or sterile water mixed with antimicrobials (amphotericin and gentamicin). Coproculture was performed by m

Various host and parasite factors interact to determine the outcome of infection. We investigated the effects of two factors on the within-host dynamics of malaria in mice: initial infectious dose and co-infection with a helminth that limits the availability of red blood cells (RBCs). Using a statistical, time-series approach to model the within-host ‘epidemiology’ of malaria, we found that increasing initial dose reduced the time to peak cell-to-cell parasite propagation, but also reduced its magnitude, while helminth co-infection delayed peak cell-to-cell propagation, except at the highest m
