Study on the Effects of Transcutaneous Auricular Acupuncture Stimulation of the Vagus Nerve on Post-Traumatic Cerebral Oedema.

BACKGROUND: Secondary cerebral oedema following traumatic brain injury (TBI) is a major cause of poor prognosis, primarily driven by neuroinflammation. High-mobility group box 1 (HMGB1) is a key damage-associated molecular pattern that initiates a potent inflammatory cascade, yet targeted pharmacological interventions face clinical translation challenges. Non-invasive transcutaneous auricular vagus nerve stimulation (taVNS) has shown anti-inflammatory potential, but its efficacy and specific mechanisms in treating traumatic cerebral oedema remain unclear. METHODS: A controlled cortical impact

Potentiating glymphatic drainage minimizes post-traumatic cerebral oedema.

Cerebral oedema is associated with morbidity and mortality after traumatic brain injury (TBI)1. Noradrenaline levels are increased after TBI2-4, and the amplitude of the increase in noradrenaline predicts both the extent of injury5 and the likelihood of mortality6. Glymphatic impairment is both a feature of and a contributor to brain injury7,8, but its relationship with the injury-associated surge in noradrenaline is unclear. Here we report that acute post-traumatic oedema results from a suppression of glymphatic and lymphatic fluid flow that occurs in response to excessive systemic release of

Improving hospital death certification in Viet Nam: results of a pilot study implementing an adapted WHO hospital death report form in two national hospitals.

BACKGROUND: Viet Nam does not have a system for the national collection of death data that meets international requirements for mortality reporting. It is identified as a 'no-report' country by the WHO. Verbal autopsy reports are used in the community but exclude deaths in hospitals. METHODS: This project was undertaken in Bach Mai National General Hospital and Viet Duc Surgical and Trauma Hospital in Viet Nam from 1 March 2013 to 31 March 2015. In phase 1, a modified hospital death report form, consistent with the International Statistical Classification of Diseases and Related Health Proble

Post traumatic cerebral oedema in severe head injury is related to intracranial pressure and cerebral perfusion pressure but not to cerebral compliance.

This was a prospective cohort study, carried out in the Neuro Intensive Care Unit, Department of Neurosciences, Hospital Universiti Sains Malaysia, Kubang Kerian Kelantan. The study was approved by the local ethics committee and was conducted between November 2005 and September 2007 with a total of 30 patients included in the study. In our study, univariate analysis showed a statistically significant relationship between mean intracranial pressure (ICP) as well as cerebral perfusion pressure (CPP) with both states of basal cistern and the degree of diffuse injury and oedema based on the Marsha

Decompressive craniectomy for the management of patients with refractory hypertension: should it be reconsidered?

INTRODUCTION: The management of refractory post-traumatic cerebral oedema remains a frustrating endeavor for the neurosurgeon and the intensivist. Mortality and morbidity rates remain high, despite refinements in medical and pharmacological means of controlling intracranial hypertension. METHOD AND MATERIAL: In this retrospective study we have evaluated the efficacy of decompressive craniectomy as a last resort therapy, from the data of nine patients with severe brain injury and delayed cerebral oedema (diffuse injury type III), treated between January 1997 and September 1999. The following p

[Decompressive craniotomy and its effect on recovery and rehabilitation of the patient].

The rehabilitation of patients who have to be operated on because of severe craniocerebral trauma and in whom the roof of the cranium must be removed because of severe post-traumatic cerebral oedema, is extremely difficult. The slow rehabilitation and recovery of these patients, who also show psychological changes in most cases, must be attributed to the pathological intracranial conditions which have arisen in consequence of large defects in cranial bones. A causal correlation between severe pathological intracranial conditions, which have arisen not only from trauma but also from bone flap d

Dose- and time-dependent effects of dexamethasone on rat brain following cold-injury oedema.

Experiments were carried out with a rat model of brain-injury oedema to establish the most efficacious dose and administration schedule for dexamethasone treatment. The results indicate that there are statistically significant dose- and time-dependent effects for dexamethasone treatment of cold-injury oedema. The equivalent of a 500 mg dose of dexamethasone had the highest anti-oedematous effect. With higher doses no further improvement could be achieved and the potential of hazardous effects increased. As expected, pretraumatic drug treatment had the greatest therapeutic effect. Dexamethasone

The influence of hyaluronidase on traumatic cerebral oedema.

As a result of the recent publication of a hypothesis concerning the possible fundamental role of hyaluronidase in tissue (2) it might be that hyaluronidase could have a better effect in the management of traumatic cerebral oedema than corticosteroids as an antagonist of this enzyme. An animal study of this subject will be discussed. Finally some general comments are made about the hypothesis with especial reference to cerebral cells in critical care medicine.

Acute intracranial hypertension occurring during infusion of stable plasma protein solution.

A case of traumatic cerebral oedema is reported in which the infusion of small volumes of stable plasma protein solution (SPPS) resulted in acute and massive increases in intracranial pressure. The batch of SPPS responsible was found to have high levels of prekallikrein activator which is thought to be the cause of the systemic hypotension previously described with plasma proteins solutions. A second batch of SPPS with low levels of prekallikrein activator generated much smaller increases in intracranial pressure. The mechanism of this phenomenon is discussed and a recommendation made for the
