Hemispheric CBF-alterations in the time course of focal and diffuse brain injury.

Hemispheric CBF-alterations were studied in the time course of focal and diffuse-brain injury in a series of 25 head injured patients. Repeated 133 Xe CBF measurements with a mobile 10 detector system were performed in order to evaluate hemispheric CBF and cerebral vasoreactivity after change of PaCO2. Focal brain injury influenced hemispheric CBF varying in the time course: A hyperperfusion could be found within the first seven days after injury in 55 percent, whereas a hypoperfusion could be detected during the whole examination period in 18 percent. In diffuse injury we never saw such CBF a

Delayed deterioration in patients with traumatic frontal contusions.

The clinical course of 18 head injured patients in whom CT had shown frontal contusions without diffuse brain injury or intracranial haematoma was reviewed. All 10 patients with unilateral frontal contusion made a good recovery. Only two of five patients with limited bilateral frontal contusions made a good recovery. Two of three patients with extensive bilateral frontal contusions deteriorated more than 24 hours after injury, and one died. Delayed deterioration is an important complication of extensive traumatic bifrontal contusions.

An analytical model of traumatic diffuse brain injury.

Diffuse axonal injury (DAI) with prolonged coma has been produced in the primate using an impulsive, rotational acceleration of the head without impact. This pathophysiological entity has been studied subsequently from a biomechanics perspective using physical models of the skull-brain structure. Subjected to identical loading conditions as the primate, these physical models permit one to measure the deformation within the surrogate brain tissue as a function of the forces applied to the head. An analytical model designed to approximate these experiments has been developed in order to facilita

Implanted pump systems for treatment of spasticity.

In about one third of patients with violent spasticity due to spinal trauma, multiple sclerosis, and diffuse brain injury adequate control with oral antispastic medication cannot be achieved and successful rehabilitation is severely handicapped. In the past these patients were subjected to destructive chemical procedures or extensive surgery. The authors present the results of management of uncontrollable spasticity by means of continuous intrathecal administration of baclofen with a totally implantable gas driven pump system (Infusaid). 30 patients were treated between June 1985 and January.

Traumatic intraventricular hemorrhage: report of twenty-six cases and consideration of the pathogenic mechanism.

A series of 26 patients suffering traumatic intraventricular hemorrhage (IVH) after closed head injury is reviewed, and the pathogenic mechanism of the traumatic IVH is discussed considering the site of origin of the IVH. Computed tomographic detection of the origin of the IVH was possible in 15 patients (Group 1): 6 had frontal or temporal contusional intracerebral hemorrhage spreading into the ventricle (Subgroup A), 5 had the original hemorrhage in the caudate nucleus (Subgroup B), and 4 originally bled in the thalamus (Subgroup C). The origin of the IVH was not determined in 11 patients (G

Aspects on pathology and neuropathology in head injury.

Some principal differences of the head injury materal available to clinicians, hospital pathologists and forensic pathologists are discussed with reference to the primary and secondary findings at focal and diffuse brain injury and intracranial injuries after trauma. The importance of cooperative understanding between the different disciplines is stressed in order to cover the whole head injury pattern. Examples are given. The importance of intracranial arterial hemorrhages is stressed and may also be the cause of a particular group of acute subdural hematomas; it may be of special prognostic

[Electroencephalographic findings in post-anoxic myoclonic encephalopathy].

The electroencephalographic (EEG) findings observed in nine patients with post-anoxic myoclonic encephalopathy are reported. The tracings showed typical myoclonus potential in some, and characteristic diffuse brain injury in others. In two of our cases the spike and wave complex was faster than in the others. Our EEG tracings are similar to those studied by Gastaut and Rémond in 1952.

Metabolic complications associated with severe diffuse brain injury.

Diffuse axonal injury (DAI), the severest form of diffuse brain injury, causes extensive damage throughout the cerebrum, diencephalon and brainstem. The effects of this injury, however, are not isolated to the brain. This paper discusses diffuse axonal injury, the metabolic complications to be anticipated from the systems' response to cerebral trauma in general, and the nursing implications.

Language disintegration in dementia: effects of etiology and severity.

The speech characteristics on a standardized picture description task of 26 subjects with presumed senile dementia of the Alzheimer type (SDAT) and 13 subjects with stroke-related dementia (SRD) were compared to 15 normal subjects over age 59 years. Compared to the normal subjects, the dementia subjects used fewer total words, fewer unique words, fewer prepositional phrases, fewer subordinate clauses, and more incomplete sentence fragments. Lexical deficits tended to be more severe than syntactic ones, confirming prior suggestions that lexicon is more vulnerable to disruption in dementia than

Magnetic resonance imaging after 'diffuse' nonmissile head injury. A neurobehavioral study.

The diagnosis of diffuse brain injury is considered when computed tomography provides no evidence of an intracranial mass lesion in acute nonmissile head injury. Magnetic resonance imaging (MRI) of a young woman who had sustained severe diffuse brain injury five years earlier disclosed multifocal lesions involving the frontal, temporal, parietal, and occipital lobes. We report the results of serial neurobehavioral assessment for clinical correlation with the brain lesions visualized by MRI. Pending confirmation of our findings in a series of patients studied by MRI during the acute and chronic

Intensive management of severe head injuries. A scheme of intensive management of severe head injuries.

Seventy-six severely head-injured patients, 67% of whom had Glasgow Coma scores of five or less on admission, were managed according to an intensive treatment regime which included controlled hyperventilation (under full muscular paralysis), high-dose steroids, dehydrating agents, diuretics and hypnotics (Althesin and thiopentone). Intracranial pressure (ICP) was measured throughout the period of controlled ventilation. Treatment was directed to keeping the mean ICP below 25-30 mmHg and to the prevention of increases in ICP during chest physiotherapy and other noxious stimulation. Six months a

Mutism after closed head injury.

Prospective study of patients admitted to a hospital for closed head injury showed that nine patients (nearly 3%) became mute for varying periods despite recovery of consciousness and communication through a nonspeech channel. Computed tomography (CT) showed subcortical lesions situated primarily in the putamen and internal capsule of four patients, whereas four of the five patients without subcortical lesions had left-hemisphere cortical injury. The patients without subcortical injury visualized by CT exhibited a longer duration of impaired consciousness consistent with severe diffuse brain i

Cardiac sequelae of acute head injury.

An intensive 72 hour study of the cardiovascular effects of severe diffuse brain injury was made in seven patients. No other major injury was present. Patients received optimal care very rapidly after injury and intracranial pressure, which was continuously monitored, was maintained at or just above normal by elective positive pressure ventilation techniques. Three patients showed evidence of progressive myocardial ischaemia on continuous electrocardiographic monitoring. Two of these patients died. Ventricular arrhythmias were uncommon but one of the patients with ischaemia developed ventricul

Cranial computed tomography in the diagnosis of symptomatic indirect trauma to the carotid artery.

Five patients who were involved in motor vehicle accidents sustained indirect, nonpenetrating carotid artery injuries leading to cerebral infarction. Four patients were conscious when a severe lateralized neurological deficit developed three to 12 hours after injury indicating the presence of a vascular injury and not a mass lesion. In 3 patients with occlusion of the main stem middle cerebral artery, computed tomographic (CT) scans performed between 1 and 7 hours after the onset of the ischemic signs were normal, but all scans performed after 11 or more hours revealed progressive edema in a v

[Post-anoxic myoclonic encephalopathy (Lance-Adams syndrome): report of 6 cases].

Six patients are studied (five females and one male) in which disorders of consciousness varying from stupor to coma have occurred following variable periods of cerebral hypoxia caused either by cardiorespiratory standstill (five cases) or acute pulmonary injury. After a few hours they had generalized convulsive crisis of tonic-clonic type. As consciousness was regained they developed generalized myoclonic trembling of great intensity. Hence post-anoxic myoclonic encephalopathy is characterized. It is also called the Lance-Adams syndrome, in honor of the authors that described it in 1963. Five

Cerebral blood flow in the acute phase after head injury. Part 2: Correlation to intraventricular pressure (IVP), cerebral perfusion pressure (CPP), PaCO2, ventricular fluid lactate, lactate/pyruvate ratio and pH.

In 40 comatose patients with severe head injury, cerebral blood flow (CBF) studies were performed with the 133Xenon washout technique over the most severely injured hemisphere. All patients were mildly sedated with diazepam, chlorpromazine and meperidine and subjected to respiratory support. Simultaneously with the CBF study, intraventricular pressure (IVP), systemic arterial pressure (SAP) and ventricular fluid (VF) lactate, pyruvate and pH were measured. The results indicate a positive correlation between CBF and lactate in patients with a good recovery, irrespective of the time after the tr

Neurological course and correlated computerized tomography findings after severe closed head injury.

This study includes 124 patients with closed head injuries and with Glasgow Coma Scale (GCS) scores of less than or equal to 8, who were admitted over a 7 1/2-month period. The time at which death occurred after injury was bimodal: deaths occurred either within 48 hours or after 7 days or longer after injury. Neurological deterioration, however, occurred with equal frequency on Days 2 to 7 after injury. Patients who survived the first 48 hours and then suffered neurological deterioration did not differ from the total population in age, sex, GCS scores on admission, or pupillary reactivity, but
