Magnetic resonance imaging and pathologic studies on lateral fluid percussion injury as a model of focal brain injury in rats.

In this study, morphologic changes in brain lesions initiated by moderate lateral fluid percussion injury in rats were investigated chronologically using high-resolution magnetic resonance imaging (MRI) and histopathologic methods. Rats were subjected to moderate fluid percussion injury (average 2.80 +/- 0.48 atmospheres) over the exposed dura overlying the right parietal cortex. MRI obtained in vivo were compared with corresponding pathologic findings at 1, 6, and 24 h and at 3, 6, 14 and 80 days after injury. T2-weighted images showed scattered low-signal intensity in the injured cortex with

Functional brain reorganization in children.

Developmental brain plasticity in association with focal brain injury is dependent on a number of factors, including age of the individual at the time of injury, size and topography of the brain lesion, maturational state of the brain system injured, integrity of brain areas surrounding and contralateral to the lesion, presence and duration of epilepsy, and medication effects. Recently developed functional neuroimaging tools now make it possible to study non-invasively several aspects of human brain functional reorganization in response to injury. Clinical models which are suitable for the stu

Focal brain injury and upregulation of a developmentally regulated extracellular matrix protein.

Tenascin is an extracellular matrix glycoprotein expressed during both normal development and neoplastic growth in both neural and nonneural tissues. During development of the central nervous system (CNS), tenascin is synthesized by glial cells, in particular by immature astrocytes, and is concentrated in transient boundaries around emerging groups of functionally distinct neurons. In the mature CNS, only low levels of the glycoprotein can be detected. The present study demonstrates that following trauma to the adult human cerebral cortex, discrete populations of reactive astrocytes upregulate

Focal brain injury induces multiple immediate early genes encoding zinc finger transcription factors.

Focal brain injury is known to markedly induce the fos and jun families of immediate early genes (IEGs). This study employed in situ hybridization to examine the effects of focal brain injury, produced by the intracerebral injection of saline or ibotenic acid on the expression of IEGs encoding zinc finger transcription factors. Thirty minutes after the injections, NGFI-A, NGFI-B, NGFI-C, and egr-3 mRNAs were induced in dentate gyrus, hippocampal pyramidal cells, cerebral cortex, caudate-putamen and piriform cortex of the injured hemisphere. Nurr1 was induced in hippocampal pyramidal cells and

Common drugs may influence motor recovery after stroke. The Sygen In Acute Stroke Study Investigators.

Studies in laboratory animals indicate that certain centrally acting drugs (eg, the antihypertensives clonidine and prazosin, neuroleptics and other dopamine receptor antagonists, benzodiazepines, and the anticonvulsants phenytoin and phenobarbital) impair behavioral recovery after focal brain injury. Even single doses may have long-term harmful effects. To determine whether these medications have a similar negative impact in humans, we analyzed the recoveries of control patients who were enrolled in the Sygen in Acute Stroke Study, a multicenter study of the effects of GM1 ganglioside after i

Focal brain damage enhances experimental allergic encephalomyelitis in brain and spinal cord.

The immunological basis of multiple sclerosis (MS) is well recognized but the factors inducing MS lesions are unclear. In this study, we test the hypothesis that focal brain injury, inflicted during the pre-clinical stages of experimental allergic encephalomyelitis (EAE), will enhance the severity of immunological damage in the cerebral hemispheres and spinal cord. Acute EAE was induced in 30 Lewis rats by the injection of guinea pig spinal cord homogenate in complete Freund's adjuvant. A cryolesion to the surface of the left cerebral hemisphere was induced at 3 days (n = 6) or 8 days (n = 10)

New magnetic resonance imaging techniques for the evaluation of traumatic brain injury.

Although current computerized tomography (CT) and magnetic resonance imaging (MRI) techniques have shown great utility in diagnosing various aspects traumatic brain injury, damage resulting from mild diffuse brain injury often goes undetected with these procedures. Newly developed MRI techniques, including magnetization transfer imaging (MTI) and diffusion-weighted imaging (DWI), have been proposed to have enhanced sensitivities for identifying damage induced by both diffuse and focal brain injury. Results from recent initial studies with experimental models of brain injury suggest that MTI ma

Focal brain injury: histological evidence of delayed inflammatory response in a new rodent model of focal cortical injury.

Cortical contusions are one of the most common characteristics in head injury and are regarded by many as the hallmark of significant injury. No experimental study has clarified the roles of mechanical forces, haemorrhage and ischaemia in the process of progressive acute brain damage and later neurobehavioural dysfunction. We have devised a new, simple reproducible rodent model of focal cortical injury which employs a 'pure' mechanical/physical force applied through the intact dura. Using this model we have investigated the time course and pattern of changes in neurons, glia and microvasculatu

Photothrombotic infarction triggers multiple episodes of cortical spreading depression in distant brain regions.

The purposes of this study were to determine whether cortical spreading depression occurs outside of the infarct produced by photothrombotic vascular occlusion, and also the direction of spreading. Focal cerebral thrombotic infarction was produced by irradiating the exposed skull of anesthetized rats with green light (560 nm) following systemic injection of rose bengal dye. At proximal sites (approximately 2 mm anterior to the infarct border), transient, severe hyperemic episodes (THEs) lasting 1-2 min were intermittently recorded. THE frequency was greatest in the first hour and declined over

[Application of transcranial Doppler ultrasonography in the assessment of cerebrovascular reactivity studies and cerebral vascular reserve estimation in patients with focal brain injury].

Cerebrovascular reactivity and cerebrovascular reserve were studied using transcranial Doppler sonography (TCD). Investigations were carried out in acute cases--8 patients with severe head injury within 5 days after trauma and in chronic cases with brain tissue defect (12 patients), and in cases with cerebral circulation insufficiency (15 patients). In acute cases the pCO2 cerebrovascular response was studied and in chronic cases cerebrovascular reserve was estimated using acetazolamide. TCD was found suitable for local vascular reactivity disturbances determination independently in three intr

Traumatic brain injury. Predicting course of recovery and outcome for patients admitted to rehabilitation.

OBJECTIVE: To demonstrate that the prognosis for patients with traumatic brain injury (TBI) admitted to rehabilitation can be established with use of principled neurologic diagnosis and predictor variables of established value in neurosurgical populations. DESIGN: A cohort of patients with TBI accumulated at rehabilitation admission were followed up for 1 year. Severity measures (Glasgow Coma Scale score, length of coma, and duration of posttraumatic amnesia) and information to generate neuropathologic profiles were gathered retrospectively and prospectively; outcome measures were obtained pr

Early cognition, communication and language in children with focal brain injury.

The authors report a longitudinal study of the first stages of cognitive, communicative and linguistic development of six Italian-speaking infants with unilateral brain lesions acquired before the point at which language acquisition normally would begin. Substantial variability was observed in the language-cognition profiles displayed by these children. To unify these diverse profiles, the authors propose a 'cognitive infrastructure' or 'threshold' model of early language development, in which the appearance of speech depends on the presence of certain cognitive prerequisites; once those are i

Photochemical brain injury in rats triggers DNA fragmentation, p53 and HSP72.

The aim of the study was to examine whether apoptosis, apoptosis-related protein p53 and heat-shock protein (HSP) 72 participate in the response of the brain to focal injury. Male Sprague-Dawley rats received intravenously a photosensitive dye rose bengal. Unilateral cortical thrombosis was induced by illuminating the skull of rose bengal-treated rats for 10 min with a focused beam of light. Animals were killed and brains were processed for immunohistochemical detection of DNA fragmentation, p53, and HSP72 kD. DNA fragmentation and p53 were increased only in the perifocal area in the cortex ip

Management of intracranial hypertension in head injury: matching treatment with cause.

Raised intracranial pressure (ICP) is common after head injury and strongly associated with mortality and morbidity. Empirical and prophylactic therapy with steroids and barbiturates has proved unsuccessful. Ideally, therapy should be targeted at the predominant cause of the increase in ICP. In head injury these may be (1) an increase in cerebral blood volume best treated by hyperventilation and hypnotic drugs. (2) an increase in brain water content best treated by osmotherapy and (3) increased CSF outflow resistance best treated by CSF drainage. This last cause seldom predominates in head inj

[Differences in the symptomatology of paroxysmal supraventricular tachycardias in relation to the different sites of localization of the arrhythmic reentry circuit. Clinical picture, semiologic and genetic aspects].

Transesophageal, electrophysiologic studies were conducted in 47 patients, with clinical and ECGgraphic diagnosis of paroxysmal reciprocating supraventricular tachycardia. After admission to hospital, the patients were enrolled in the study in accordance with the criterion concerning the exclusion of patients with signs and symptoms of severe heart pump failure (ie, NYHA III and IV class were excluded). The transesophageal study was performed during paroxysmal tachycardia in each patient to measure the V-A interval and to localize the site of reentry. Thereby, the patients could be grouped int

Differential expression of immediate-early proteins in non-nerve cells after focal brain injury.

We investigated the expression of the immediate-early proteins (IEPs, Fos, Fos B, Jun, Jun B, Jun D, Krox 20 and Krox 24) in non-nerve cells in rat brain after mechanical brain injury. Injury produced by infusion of 5 microliters of saline into the hippocampus produced a time-dependent expression of Fos, Jun and Krox 24, but not Fos B, Krox 20 or in non-nerve cells around the wound margin, in cells lining the lateral and third ventricles and in cells in the pial surfaces of the brain. Jun B and D were weakly induced in non-nerve cells 1-4 hr after brain injury. This differential expression of

The influence of cryogenic brain injury on nociception in the rat.

BACKGROUND: Previous studies have suggested that focal cryogenic brain lesions that cause functional cerebral depression may increase anesthetic potency. To determine whether this effect was caused by changes in nociception, this study prospectively evaluated the influence of an experimental focal brain injury on the analgesic effects of the opioids, fentanyl and alfentanil, in rats. METHODS: The cortical freezing lesion was made with a brass probe cooled to -50 degrees C, applied through a craniotomy to the intact dura for 5 s. The analgesic effects of the opioids were quantified by tail-fli

[Neuropsychological development in children with focal brain injury].

The study of children with focal brain injury has important implications from a clinical and theoretical perspective. Clinical data on children with congenital or early acquired lesions indicates that the cognitive sequelae are different from those resulting from similar damage sustained in adulthood. These differences depend in part on the differential effect that damage has on ongoing developmental process and in part on the different recovery capacity of the Central Nervous System of the child. From a theoretical perspective, focal lesion data is important for analyzing the issues of early

Basic and clinical studies of pharmacologic effects on recovery from brain injury.

Investigations in laboratory animals indicate that certain drugs that influence specific neurotransmitters can have profound effects on the recovery process. Even small doses of some drugs given after brain injury facilitate recovery while others are harmful. Preliminary clinical studies suggest that the same drugs that enhance recovery in laboratory animals (e.g., amphetamine) may have similar effects in humans after stroke. In addition, some of the drugs that impair recovery of function after focal brain injury in laboratory animals (e.g. haloperidol, benzodiazepines, clonidine, prazosin, ph
