Uncorrected Tetralogy of Fallot Complicated by Brain Abscess and Postoperative Intracranial Haemorrhage: A Case Report.

Cerebral abscess is a rare but potentially fatal complication of cyanotic congenital heart disease, in which chronic hypoxia, polycythaemia and right-to-left shunting allow bacteraemia to bypass pulmonary filtration. A woman in her 20s with uncorrected tetralogy of Fallot presented with severe headache, right-sided weakness and seizures. Examination showed hemiparesis, hypoaesthesia and hyperreflexia; blood tests revealed secondary polycythaemia and leucocytosis. No dental, sinus, otological or respiratory source was identified. Unenhanced computed tomography demonstrated a left parietal space

The Influence of Different Factors on Brain Weight.

BACKGROUND: A postmortem brain weight examination can provide valuable diagnostic information on probable causes of death. Deviations from normal brain weight can indicate the presence of different factors such as psychoactive substance use, the presence of neurological conditions, tumours, brain oedema or traumatic brain injury. The aim of this study is to analyse these factors and their role in understanding the underlying causes of death. MATERIALS AND METHODS: This research was designed as a retrospective study. The study sample consisted of 651 autopsy cases from 2013 to 2023. The brain

Single piece craniotomy in the management of a patient with large vertex epidural haematoma and deteriorating brain function in a limited resources community. Case report from Southwestern Uganda.

INTRODUCTION AND IMPORTANCE: Vertex epidural haematoma is a rare form of epidural haemorrhage and a complication of head trauma that is both a diagnostic and therapeutic challenge for the trauma surgeon. It's usually as result of laceration in the superior sagittal sinus. Whereas most patients with small vertex epidural hematomas improve with conservative treatment, large or rapidly expanding lesions are fatal and require emergency craniotomy. CASE PRESENTATION: Ours was a 28-year-old male patient who arrived at our emergency department in rural Southwestern Uganda with Glasgow coma scale of

Sectional Anatomy Quiz-IΧ.

This series lists a pictorial quiz pertaining to identification of normal and abnormal anatomical structures and landmarks at a given level on computed tomography (CT). Readers are expected to identify and appreciate the changes from normal anatomy and variations of a given pathology. The main structures assessed in this quiz are the pons, ventricular system of the brain, and the basal cisterns. Particular emphasis is placed on the presentations of intra-cranial haemorrhages, particularly sub-arachnoid and epidural haemorrhages, and masses around the region of the pons, midbrain and cerebellum

Lumbar vertebral canal stenosis due to marked bone overgrowth after routine hemilaminectomy in a dog.

BACKGROUND: Bone overgrowth after decompressive surgery for lumbar stenosis resulting in recurrence of neurological signs has not been reported in veterinary literature. However, there are few cases described in human medicine. CASE PRESENTATION: A 13-month-old entire female dog, a crossbreed between a Springer Spaniel and a Border Collie, weighing 24 kg, was referred with a 5-day history of progressive spastic paraplegia, indicative of a T3-L3 myelopathy. Magnetic resonance (MR) imaging revealed a right-sided L2-L3 compressive extradural lesion, compatible with epidural haemorrhage, which wa

Successful surgical and medical treatment of a severe, acute epidural bleed in a young dog due to steroid responsive meningitis-arteritis.

BACKGROUND: Steroid responsive meningitis-arteritis (SRMA) is an immune-mediated disease of the leptomeninges and its associated blood vessels, typically responsive to corticosteroids. Clinically relevant haemorrhage is a rare finding in such patients and for this reason surgical decompression of the spinal cord is normally not considered. The diagnosis of SRMA is supported by serum C-reactive protein (CRP) increase, cerebrospinal fluid (CSF) examination, including cytology (polymorphonuclear pleocytosis in the acute form), nucleated cell-, red blood cell- and protein count, as well as by eval

Arterial spin labeling in neonatal magnetic resonance imaging - first experience and new observations.

PURPOSE: Arterial spin labeling (ASL) is a noninvasive non-contrast technique of perfusion imaging that uses endogenous water from the blood as the perfusion tracer, with very scant data on its use in neonates. The authors present the added value of ASL in the examined babies in their own material and discuss it in the light of the existing literature. MATERIAL AND METHODS: During the first 10 months after the purchase of a new magnetic resonance imaging (MRI) scanner, 123 neonates were examined using it in an MR-compatible incubator, 117 of them had brain MRI, and in 104 ASL was incorporated

Non-traumatic myositis ossificans of the thigh. A case report.

INTRODUCTION: Myositis ossificans (MO) is an ectopic ossification characterized by an appearance of bone formation predominantly in muscle tissue. Trauma is the most common etiological factor, observed in almost 60-75% of cases, whereas a non-traumatic MO is rarely described in the literature. A diagnosis is based on clinical and radiological findings. PRESENTATION OF CASE: A 75-year old female patient has been admitted to our clinic with a localized swelling of the posterior femoral compartment, presented on magnetic resonance as a calcification in the biceps femoris muscle. Assessment of left atrial function and dyssynchrony by real time three-dimensional echocardiography predicts recurrence of paroxysmal atrial fibrillation after radiofrequency ablation.

OBJECTIVE: Left atrial volume and function are associated with recurrence of paroxysmal atrial fibrillation (AF) after radiofrequency ablation. A relationship between left atrial mechanical dyssynchrony and AF recurrence is presently unclear. The aim of this study was to investigate whether left atrial volume, function, and dyssynchrony were associated with AF recurrence in patients with normal left ventricular function, and normal or mildly enlarged left atrium, if assessed by the Real-time three-dimensional echocardiography (3DE). PATIENTS AND METHODS: We included 88 patients with AF who ha

An Unusual Case of a Large Hematorrachis Associated with Multi-Level Osteoporotic Vertebral Compression Fractures; a Case Report.

Spinal epidural haemorrhage may present as back pain associated with radicular symptoms and can be a catastrophic clinical scenario with progression to paraplegia or even sudden death. Being a rare entity, it needs a high index of clinical suspicion to diagnose it. Fractures have been documented as a cause of hematorrachis but such hematomas only extend to one or two vertebral segments. Large epidural hematomas are usually associated with conditions like bleeding diathesis, arterio-venous malformations, plasma cell myeloma, and non-Hodgkin's lymphoma. Surgical management with immediate evacuat

Prothrombin complex concentrate facilitates emergency spinal surgery in anticoagulated patients.

BACKGROUND: Oral anticoagulants are commonly used in the ageing population and therefore, spine surgeons are increasingly confronted with anticoagulated patients requiring surgical therapy. 'Bridging therapies' with heparins are established in elective settings, but the time frame for haemostasis restoration may be too long for patients presenting with acute spinal pathology and impending disability. The goal of this study was to analyse the feasibility of prothrombin complex concentrate (PCC) administration to facilitate emergency spinal surgery in anticoagulated patients. METHOD: A retrospe
