Traumatic subarachnoid haemorrhage.

The presence of subarachnoid blood following head injury is a significant risk factor for poor outcome. Treatment aims to prevent and treat secondary hypoxia, hypotension, intracranial haematoma, epilepsy and infection. There is good evidence for the benefit of nimodipine in aneurysmal subarachnoid haemorrhage. Its role in preventing cerebral ischaemia following trauma is currently under investigation.

Correlation of computed tomography findings and serum brain damage markers following severe head injury.

The objective of our study was to investigate the association between the initial levels of serum S-100B protein and neuron specific enolase and the severity of radiologically visible brain damage and outcome after severe head injury. Admission computed tomography (CT) scans of forty-four patients with severe head injury were analysed. Initial levels of S-100B protein and neuron specific enolase were compared between the different outcome groups at 6 month, the different categories of the Marshall classification, the presence of traumatic subarachnoid haemorrhage, the type of haematoma and the

CT diagnosis of non-traumatic subarachnoid haemorrhage in patients with brain edema.

The aim of the study is to prove, retrospectively, that it is unlikely that the computerized tomography (CT) diagnosis of subarachnoid haemorrhage (SH) accompanies the CT diagnosis of generalized brain edema. A total of 100 comatose patients underwent CT of the brain. Of this number, 42 underwent an enhanced CT scan. In 26 patients, lumbar puncture was also performed. A control group of ten patients diagnosed with headache and having a normal CT scan underwent NECT and ECT. Measurements of the white and gray matter density in Hounsfield units (HU) were performed in all 110 cases, including the

A review of cerebral vasospasm. Part IV. Post-traumatic vasospasm.

Cerebral vasospasm is increasingly recognized as contributing to ischaemia after head injury. The reported incidence of post-traumatic vasospasm (PTV) varies between 10% and 90%, probably largely because of differences in patient selection, in definitions of vasospasm and in methods of detecting spasm. In severe head injury, based on studies with similar criteria, the incidence is approximately 40%. PTV is often associated with traumatic subarachnoid haemorrhage (tSAH), but has been reported without tSAH. These two factors are independently associated with poor outcome, but the direct links be

Evaluation of traumatic subarachnoid haemorrhage on computed tomography.

Traumatic subarachnoid haemorrhage (TSAH) is a computed tomography (CT) scan finding frequently found in the acute phase of brain injury. However, the clinical evaluation of TSAH is controversial. The subjects in the present series consisted of 46 patients in whom the initial CT scan within 6 h after injury revealed a high density area in the subarachnoid space. The subjects were divided into three types: type 1 n = 10) had massive haemorrhage in the basal cisterns; type II (n = 9) had localized haemorrhage in the basal cisterns; and type III (n = 27) had localized haemorrhage in the cortical

[Subarachnoid hemorrhage in Vestfold county. Occurrence and prognosis].

This prospective study comprises all patients treated for spontaneous non-traumatic subarachnoid haemorrhage between May 1991 and December 1995 in the county of Vestfold, Norway. A total of 76 patients was recorded, giving an incidence of 8.1 per 100 000 per year. Mean age at time of bleeding was 52.7 years. In 36 patients the cause of the bleeding was an intracranial aneurysm; most of the aneurysms were localized to the anterior communicating artery and middle cerebral artery. In seven patients the cause was arteriovenous malformation. 23 patients (30%) died because of their subarachnoid haem

[Perimesencephalic subarachnoid hemorrhage].

Acute, non-traumatic subarachnoid haemorrhage is usually caused by a ruptured aneurysm. This is a serious condition involving high mortality. Perimesencephalic haemorrhage has recently been identified as a clinical subset of subarachnoid haemorrhage. A pattern of haemorrhage predominantly limited to perimesencephalic cisterns on CT is highly predictive of a normal angiogram, and is connected with an excellent prognosis. We describe two patients with perimesencephalic haemorrhage.

Nimodipine in traumatic subarachnoid haemorrhage: a re-analysis of the HIT I and HIT II trials.

Two large randomised controlled trials have been performed to study the effect of the calcium antagonist nimodipine on the outcome of severe head injury, HIT I [1] amd HIT II [4]. Both trials showed a modest and statistically non-significant increase in the proportion of favourable outcomes in patients treated with nimodipine. A subgroup analysis of the HIT II trial [4, 5] suggested, however, that there could be a substantial protective effect of nimodipine in patients with traumatic subarachnoid haemorrhage (SAH). This report provides a re-analysis of the HIT I data to see whether it provides

The importance of traumatic subarachnoid haemorrhage.

Post-traumatic cerebral vasospasm is being increasingly recognised as a possibly significant complication after head injury. To assess the relationship between post-traumatic subarachnoid haemorrhage (tSAH) and post-traumatic vasospasm, 63 patients with severe head injury (GCS 3-8) were studied. Forty-seven patients had cerebral contusion on the initial CT scan. In 25 of these (Group I) there were only contusions, while 22 (Group II) also had tSAH. All patients had daily measurements of blood flow velocity in the basal cerebral arteries using transcranial Doppler ultrasound (TCD). The incidenc

[Predictive mortality factors and the development of major complications in non-traumatic subarachnoid hemorrhage].

Our aim was to analyze death prognostic factors in the first six months or the development of major neurological complications (delayed brain ischaemia and rebleeding) or general complications (cardiorespiratory, hydroelectrolytic or infectious) in non-traumatic subarachnoid haemorrhage. We carried out an observational study of a historical cohort of 111 patients admitted consecutively to our Centre between 1986 and 1992 and who were diagnosed as having subarachnoid haemorrhage based upon clinical data and confirmed in all cases by brain computerized tomography (CT) scan and/or lumbar puncture

Intracranial traumatic and non-traumatic haemorrhagic complications of warfarin treatment.

An increased referral of patients with anticoagulation related haemorrhages necessitated an analysis of causes and outcome of these complications in a patient group that reflected Swedish therapeutic traditions of anticoagulation treatment. Prospectively, all patients from Stockholm evaluated for warfarin related intracranial haemorrhage occurring during 1987 were analysed and their 6 month outcome recorded. Sixty-eight patients were included. The results of intracranial haemorrhagic complications were catastrophic with a 77% mortality rate. Their incidence was much higher than expected. Forty

Prognostic value of the amount of post-traumatic subarachnoid haemorrhage in a six month follow up period.

Clinical and radiological patterns from 148 patients with post-traumatic subarachnoid haemorrhage (TSAH) were analysed with specific regard for the amount and distribution of blood in subarachnoid spaces to verify if these variables have any influence on overall outcome. The degree and extent of TSAH were classified according to Fisher's criteria: in 93 patients it was grade 1, in 36 grade 2, in 13 grade 3, and in six grade 4. There was a significant correlation between increasing clinical severity at admission and the amount of subarachnoid bleeding and a direct relation between a favourable

Clinical significance of the finding of subarachnoid blood on CT scan after head injury.

The population analysed consisted of 268 out of 819 patients of a European nimodipine multicentre trial on severe head injury, whose first CT scan after injury showed signs of subarachnoid bleeding. The study demonstrated the importance of traumatic subarachnoid haemorrhage (tSAH) per se as a prognostic factor. The outcome of patients with tSAH is significantly worse than that of patients whose first CT does not show subarachnoid blood (noSAH). The outcome was unfavourable (dead, persistent vegetative state, severe disability) in 60% of tSAH patients compared to 30% of noSAH patients (p < 0.00

Traumatic subarachnoid haemorrhage: analysis of 89 cases.

Between 1985 and 1990, 2056 patients with head injuries were treated in the Department of Neurosurgery at the Izmir State Hospital. Among them, 89 patients with traumatic subarachnoid haemorrhage (TSAH) were analysed retrospectively. It was noted that focal or global contusion accompanying TSAH was the most common pathology which could be detected in computer tomographic (CT) imaging. CT did not show any other intracranial lesion in 13 cases. Vasospasm developed in one patients and hydrocephalus in two others in the acute stage. Considering the relationship between the severity of a subarachno

Cerebral intraventricular haemorrhage in a young adult.

A case of a 26-year-old man who suffered a fatal intraventricular cerebral haemorrhage following an episode of trauma is described. The initial appearance at necropsy suggested a traumatic subarachnoid haemorrhage and initial investigation was directed towards the anterior neck structures and the vertebral arteries with negative results. Dissection of the fixed brain showed a massive intraventricular bleed with secondary involvement of the subarachnoid space and dissection into the cerebral parenchyma. No bleeding points or natural disease of the cerebral vessels could be identified. The pract

[Surgical indications in space-occupying contusion hemorrhage].

Traumatic intracerebral haematomas are a common neurosurgical emergency. The role of surgical removal is controversial. The diagnosis and treatment of life-threatening thoracic or abdominal haemorrhage is more urgent than are measures undertaken by the neurosurgeon. In the present paper we report on 318 patients with severe head injury. In 161 cases (38 female, 123 male) CT investigation showed a a resulting from contusion. About a third of each group of patients, both with and without contusion-induced haematomas, were 21-30 years old. The mortality rate was 18.6%. Multiple contusion haematom

Blunt basal head trauma: rupture of posterior inferior cerebellar artery.

Two cases are reported of rupture of posterior inferior cerebellar artery (PICA) from blunt basal head trauma. The anatomy at the site of rupture is discussed as a prerequisite for rupture. The rapid fatal course is also discussed. The authors propose that the forensic term "traumatic subarachnoid haemorrhage" ought to be abandoned and replaced by the nature and localization the source of bleeding, analogous to clinical practice at the spontaneous haemorrhage from rupture of aneurysm.

Blunt basal head trauma: aspects of unconsciousness.

Two cases of street violence directed to the skull base level and transverse to the cervical axis are described. No skeletal damage. The violence resulted in the so-called "traumatic subarachnoid haemorrhage", an often used, unspecified forensic "diagnosis"; it was here revealed to be due to rupture of the wall of the posterior inferior cerebellar artery (p.i.c.a). However, this was only one of the possible explanations for the acute symptoms of unconsciousness (concussion) and almost immediate death. The careful examination of these two cases and of a series of control cases revealed that at

Vasospasm after traumatic subarachnoid haemorrhage: transcranial Doppler evaluation. Case report.

A case of vasospasm after traumatic subarachnoid haemorrhage (SAH) is reported here. Transcranial Doppler Sonography (TCD) was used to evaluate mean flow velocity (MFV) changes of the basal cerebral arteries related to vasospasm. Acceleration of MFV of the right middle cerebral artery (MCA) indicating vasospasm was first noted on TCD evaluation, and then proved by carotid angiography (CAG). Evaluation of all TCD results revealed that the process of relaxation or normalization of the spastic artery started from the proximal side of the basal intracranial artery and gradually moving to the dista
