Construction of nomogram model for early death risk in patients with severe traumatic brain injury.

BACKGROUND: Traumatic brain injury (TBI) poses significant challenges in prognostication and clinical management, particularly in severe cases. The need for precise prognostic tools to predict outcomes, including early death, in severe TBI is crucial. This study aimed to construct a nomogram model for early death risk in patients with severe TBI to enhance clinical decision-making. METHODS: This retrospective cohort study included severe TBI patients categorized into non-survivors and survivors groups from August 2018 to March 2024. Data on demographic, clinical, laboratory, and imaging param

Utilization of Neuromuscular Blocking Agents in Acute Brain Injury and Associations with Outcomes: A Post Hoc Analysis of the ENIO Study.

BACKGROUND: Neuromuscular blocking agents (NMBA) have been used in mechanically ventilated patients with moderate-to-severe acute respiratory distress syndrome (ARDS), in cases of clinically significant ventilator dyssynchrony, and in patients with elevated intracranial pressure (ICP). However, practice patterns around NMBA utilization and their impact on outcomes in acute brain injury (ABI) remain insufficiently explored. METHODS: This study carried out a post hoc analysis of a multicenter, prospective observational study (NCT03400904) including adult patients with ABI (Glasgow Coma Scale ≤

Past, present and future perspectives of traumatic brain injury.

Despite extensive research, traumatic brain injury (TBI) imposes significant mortality and morbidity burden globally. This narrative review provides a comprehensive overview of how TBI classification, surgical and critical care management, and physiological treatment goals have evolved based on recent evidence, shaping future directions for TBI care. We highlight the conceptual shift in TBI classification from the Glasgow Coma Scale to a new framework that integrates clinical and imaging features, biomarkers, and modifiers to improve disease characterization and describe disease trajectories.

Cognitive bias and diagnostic overshadowing in trauma: The case of substance use disorder in patients with traumatic brain injury.

BACKGROUND: Diagnostic overshadowing is a type of cognitive bias where a pre-existing condition overshadows the evaluation of other diagnoses. We aimed to explore diagnostic overshadowing in trauma, using as a proxy the effects of substance and alcohol use disorder on time to surgical intervention in patients with traumatic brain injury. METHODS: The 2017-2020 American College of Surgeons-Trauma Quality Improvement Program database was used to identify patients of at least 18 years of age with isolated traumatic brain injury who underwent craniotomy, intraventricular drain, or intracranial pr

Impact of Early Mobilization and Rehabilitation on Functional Outcomes Following Craniocerebral Gunshot Injuries.

Craniocerebral gunshot injury (CGI) is a severe form of traumatic brain injury associated with profound neurological, cognitive, and functional impairment, often requiring prolonged rehabilitation. We report the case of a 17-year-old boy with a penetrating CGI who presented with a Glasgow Coma Scale score of 3/15, intracranial hemorrhage, a depressed frontal skull fracture, retained bullet fragments, and complete dependence in activities of daily living. Following neurosurgical stabilization, the patient underwent a structured, multidisciplinary neurorehabilitation program over 12 weeks (48 se

Workflow Patterns and Clinical Consequences of External Ventricular Drain Timing: A 6-Year Analysis of 26 020 Cases From the American College of Surgeons National Trauma Data Bank.

BACKGROUND AND OBJECTIVES: External ventricular drains (EVDs) are critical for monitoring and managing intracranial pressure in acute traumatic brain injury patients. However, there is a lack of consensus on optimal EVD placement timing. Previous studies suggest benefits in early placement but are limited by smaller cohorts and lack of adjustment for illness severity. We sought to characterize EVD timing patterns and examine clinical characteristics and outcomes associated with early placement. METHODS: We performed a retrospective study of patients in the National Trauma Data Bank (2018-2023

Outcomes following shunt for secondary hydrocephalus in patients with prolonged disorders of consciousness: a matched cohort study.

The effectiveness of surgical intervention for hydrocephalus in patients with prolonged disorders of consciousness (pDoC) remains controversial. To investigate the impact of ventriculoperitoneal shunt (VPS) on secondary hydrocephalus patients with pDoC following traumatic brain injury (TBI) and intracranial hemorrhage (ICH). We conducted a retrospective matched cohort study based on age, duration and state of consciousness. Patients in the vegetative state/minimally conscious state (VS/MCS) with secondary hydrocephalus due to TBI/ICH who underwent VPS and non-VPS patients who served as a match

An Eye on Survival: Association Between Initial Optic Nerve Sheath Diameter and 30-Day Mortality in Patients With Traumatic Brain Injury.

Background Traumatic brain injury (TBI) is a major cause of morbidity and mortality worldwide, necessitating early identification of high-risk patients for timely management. Elevated intracranial pressure (ICP) is associated with poor outcomes in TBI, and optic nerve sheath diameter (ONSD), measured by bedside ultrasonography, has emerged as a useful noninvasive marker of elevated ICP. This study aimed to evaluate the association between initial ONSD measurement and 30-day mortality among patients with TBI. Methods A prospective observational study was conducted over one year in the Departmen

Structural basis for Mg²⁺-glycine synergy in the tandem glycine riboswitch revealed by cryo-EM.

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

Patient Characteristics to Inform Military Medical Evacuation Decision-Making in Mild Isolated Head Injury.

INTRODUCTION: Management of head trauma in austere military environments is challenging as current head injury scoring systems have limited triage utility in the absence of advanced imaging. Additionally, unlike the recent past, future conflicts will be characterized by limited opportunities for aeromedical evacuation. We hypothesize that safe hospital discharge within 48 h may be an effective surrogate endpoint to identify patients appropriate for averted or delayed evacuation in military settings. To analyze this, we assess mild head-injured patients for associations between clinical factors

Frequency and Indications for Anesthesia Exposure in a Pediatric Traumatic Brain Injury Cohort.

BACKGROUND: Pediatric traumatic brain injury (pTBI) is a leading cause of morbidity in the pediatric population. These patients often undergo multiple anesthetic exposures (AE) during hospitalization for their injuries. In this retrospective study, we sought to analyze the hospital course and outcomes in pTBI patients, hypothesizing that these patients required multiple AEs and that repeated AEs increased the risk for morbidity. METHODS: A chart review of pTBI patients from the Westchester Medical Center Pediatric Trauma Registry was conducted from 2012 to 2021. Demographics, injury data, and

Transient spinal neuropraxia in pediatric patients: analysis of an institutional experience 4 decades after the introduction of spinal cord injury without radiographic abnormality.

OBJECTIVE: The concept of spinal cord injury without radiographic abnormality (SCIWORA) was introduced in the 1980s. Unfortunately, the nomenclature creates confusion in the modern MRI-based era when applied to pediatric traumatic spinal injuries. The authors investigated the incidence and clinical characteristics of pediatric patients with true imaging-negative traumatic cervical spinal cord injuries (SCIs). METHODS: A prospective pediatric level I trauma center database at Primary Children's Hospital was queried to identify patients with cervical spine injuries tagged with "SCIWORA" or "tra

Platelet function testing does not predict hemorrhage progression in mild traumatic brain injury.

BACKGROUND: Antiplatelet medications are frequently assumed to increase the risk of radiographic progression following traumatic intracranial hemorrhage (ICH), influencing guidelines that recommend prolonged observation and repeat imaging. However, antiplatelet use does not uniformly result in functional platelet inhibition, and the clinical relevance of therapeutic response remains unclear. METHODS: We conducted a retrospective cohort study of adult patients with traumatic ICH and a Glasgow Coma Scale (GCS) score of 13-15 who were on antiplatelet therapy and underwent platelet function testi
