Exploring the Effects of Traumatic Brain Injuries on Cranial Nerve Injury.

BACKGROUND: Traumatic brain injuries (TBI) are a significant and growing health issue, leading to over 200 000 hospitalizations annually in the United States. Cranial nerve (CN) injuries accompanying TBI can severely impact patients' quality of life. This review aims to address the gap in research regarding the severity, mechanisms of injury, and associated intracranial injuries, emphasizing the importance of early detection and intervention. METHODS: A comprehensive literature search was conducted across databases such as PubMed and Ovid using key terms, including "cranial nerve injury," "cr

Effective management of traumatic venous sinus injuries in a resource-limited health system: a retrospective cohort study.

OBJECTIVE: Traumatic venous sinus injury (VSI) is a rare but serious complication with high rates of morbidity and mortality. In resource-limited health systems, restricted access to advanced imaging and high trauma volumes further complicate care. The objectives of this study were to quantify the effectiveness of a resource-constrained imaging protocol, evaluate its impact on outcomes, and identify predictors of VSI. METHODS: The authors conducted a retrospective cohort study of adults with skull fractures overlying dural venous sinuses at Tygerberg Academic Hospital, a resource-limited publ

Traumatic intraventricular haemorrhages: clinical indicators determining morbidity and mortality in paediatric patients.

UNLABELLED: Intracranial haemorrhages and traumatic brain injury are significant causes of morbidity and mortality in paediatric patients. Traumatic intraventricular haemorrhage (tIVH) is the least common type of intracranial haemorrhage and typically occurs in association with other intracranial injuries. This study aimed to describe the clinical characteristics of children diagnosed with tIVH, and to identify risk factors associated with poor prognosis. Paediatric patients diagnosed with tIVH in the paediatric emergency department between January 2010 and December 2024 were retrospectively r

Anti-N-methyl-D-Aspartate Receptor Autoimmune Encephalitis Due to Ovarian Teratoma in a 17-Year-Old Young Woman: Case Report and Review of the Literature.

Many cases of encephalitis are caused by an autoimmune response, and some are associated with underlying malignancies. Here we present the case of a 17-year-old female patient who was admitted to the pediatric ward with acute gastroenteritis, seizures, and behavioral changes. A brain computed tomography (CT) scan ruled out acute infarction, haemorrhage, mass lesions, and other intracranial injuries, while the initial EEG showed no epileptic discharges. In the absence of a definitive diagnosis, midazolam was administered to treat tonic-clonic seizures, while ceftriaxone and acyclovir were start

Sickness absence > 14 days following sport-related traumatic brain injuries: a nationwide register-based study in Sweden.

BACKGROUND: Sport-related traumatic brain injuries (SR-TBI) have received increasing concerns regarding potential long-term consequences. For adults in the general population, one of these consequences is inability to resume daily activities, most notably sickness absence (SA) from work. The aim of this study was to investigate how often SR-TBI is followed by a registered period of SA. METHODS: A nationwide register-based study was conducted. Source population: working aged individuals (18-63 years), living in Sweden during 2014-2016. Using the national patient register we included patients w

Trends of anti-seizure medication prescribing pattern in traumatic brain injury patients for the prevention of posttraumatic seizure in Taiwan.

Traumatic brain injury (TBI) patients are recommended to receive anti-seizure medication (ASM) as posttraumatic seizure (PTS) prophylaxis. However, the utilization of ASM, including the prescription patterns and associated clinical characteristics, is limited in Taiwan. Thus, this study aimed to investigate the ASM trends and clinical characteristics. This retrospective cohort study enrolled TBI patients who received levetiracetam, phenytoin, and valproic acid during hospitalization using the National Health Insurance Research Database between 2012 and 2019. The primary outcome was the trend o

Long-term outcomes of major trauma patients with concussion.

INTRODUCTION: Concussion may be sustained in the setting of injuries to multiple body regions and persistent effects of concussion may impact recovery. This project aimed to evaluate the association between concussion and 6-month and 12-month functional outcomes in survivors after major trauma. METHODS: This was a registry-based cohort study that included adult patients with major trauma who presented to hospital between 01 Jan 2008 and 31 Dec 2017 and survived to hospital discharge. We excluded patients presenting with a Glasgow Coma Scale score <13 and those diagnosed with other intracrania

Acute Outcomes of Isolated Pneumocephali in Children After Minor Blunt Head Trauma.

OBJECTIVES: We aimed to determine the prevalence of and adverse outcomes caused by pneumocephali in children with minor blunt head trauma who had no other intracranial injuries (ie, isolated pneumocephali). METHODS: We conducted a secondary analysis of a public use dataset from a multicenter prospective study of pediatric minor head trauma. We included children younger than 18 years with Glasgow Coma Scale (GCS) scores of 14 or 15 and non-trivial mechanisms of injury who had cranial computed tomographies obtained. Patients with isolated pneumocephali were those without other traumatic brain i

Routine repeat head CT may not be necessary for patients with mild TBI.

BACKGROUND: Routine repeat cranial CT (RHCT) is standard of care for CT-verified traumatic brain injury (TBI). Despite mixed evidence, those with mild TBI are subject to radiation and expense from serial CT scans. Thus, we investigated the necessity and utility of RHCT for patients with mild TBI. We hypothesized that repeat head CT in these patients would not alter patient care or outcomes. METHODS: We retrospectively studied patients suffering from mild TBI (Glasgow Coma Scale (GCS) score 13-15) and treated at the R Adams Cowley Shock Trauma Center from November 2014 through January 2015. Diffuse axonal injury (DAI) in moderate to severe head injured patients: Pure DAI vs. non-pure DAI.

OBJECTIVE: Diffuse axonal injury (DAI) is known to be associated with poor outcome. DAI often associates with other intracranial injuries but their distinct features have not been established. In this retrospective cohort study, we compared clinical outcomes between pure and non-pure DAI patients. PATIENTS AND METHODS: Total of 1047 traumatic brain injury (TBI) patients visited our institute between 2011 and 2017. Age ranged between 15-85 years old and Glasgow coma scale (GCS) score less than 13 were included. DAI was diagnosed in 45 patients using CT and MRI and their clinical features and o

Clinical Outcome of Epidural Hematoma Treated Surgically in the Era of Modern Resuscitation and Trauma Care.

OBJECTIVE: Patients from contemporary populations with traumatic brain injury (TBI) resulting from epidural hematoma (EDH) may differ regarding age, comorbidities, and coagulation status. We therefore analyzed predictors for the clinical outcome of patients with EDH treated surgically regarding modern approaches to resuscitation and trauma care. METHODS: A retrospective observational analysis was carried out. All patients included underwent surgery. The indication for surgery followed international guidelines. Retrospective data evaluation considered data reflecting the effectiveness of traum

Pediatric abusive head trauma and stroke.

OBJECTIVE Despite established risk factors, abusive head trauma (AHT) continues to plague our communities. Cerebrovascular accident (CVA), depicted as areas of hypodensity on CT scans or diffusion restriction on MR images, is a well-known consequence of AHT, but its etiology remains elusive. The authors hypothesize that a CVA, in isolation or in conjunction with other intracranial injuries, compounds the severity of a child's injury, which in turn leads to greater health care utilization, including surgical services, and an increased risk of death. METHODS The authors conducted a retrospective

Clinical Presentations and Outcomes of Children With Basilar Skull Fractures After Blunt Head Trauma.

STUDY OBJECTIVE: We describe presentations and outcomes of children with basilar skull fractures in the emergency department (ED) after blunt head trauma. METHODS: This was a secondary analysis of an observational cohort of children with blunt head trauma. Basilar skull fracture was defined as physical examination signs of basilar skull fracture without basilar skull fracture on computed tomography (CT), or basilar skull fracture on CT regardless of physical examination signs of basilar skull fracture. Other definitions included isolated basilar skull fracture (physical examination signs of b

A National Survey of Traumatic Brain Injuries Admitted to Hospitals in Sweden from 1987 to 2010.

BACKGROUND: With an increasing and aging population, there is a global demand for improving the primary prevention strategies aimed at reducing traumatic brain injuries (TBIs). The objective of the present epidemiological study was to evaluate the pattern of TBI in Sweden over a 24 years period (1987-2010). METHODS: The Swedish Hospital Discharge Register was used, where in-patient care with a main diagnosis of TBI according to ICD9/10 was included. External factors, age and gender distribution was evaluated. RESULTS: A decreasing number of annual incidence was observed, that is, from 230 to

Antithrombotic therapy and outcomes of cervical arterial dissection in the trauma patient: a case series.

BACKGROUND: The use of antithrombotic therapy (anticoagulants and/or antiplatelets) in the setting of traumatic cervical arterial dissection (CAD) for the prevention of stroke remains controversial. This issue is further complicated by the frequent co-existence of intracranial hemorrhage (ICH) and other intracranial injuries, and also the wide variability in treatment due to a lack of evidence-based guidance. To address these controversies, a registry in a major Level I trauma center was created. The purpose of this investigation was to compare the safety of antithrombotic therapy in post-trau
